Five years later, his parents still have the sedan, two bullet holes rusting in the hood and fender. They also have file folders documenting years of letdown in their pursuit of Tommy's killer. Sitting around the kitchen table in a suburban split-level, they say Harvey police lied to them, yelled at them and made them feel like a bother. A state police agent who reviewed the case told the Tribune that Harvey's handling of the investigation was a "travesty." 2 November 7, Semi truck crashes on NB McLoughlin Blvd. PORTLAND -- McLoughlin Boulevard has been reopened, following a rollover crash involving a semi-truck early Wednesday afternoon. The crash was reported just after 1:30 p.m. The truck rolled into a bridge. Northbound lanes were initially closed as the scene was cleared and the bridge is inspected, said Kimberly Dinwiddie of the Oregon Department of Transportation. But all lanes were re-opened in time for the evening commute. A detour was initially put into place at Southeast 18th Avenue, to Southeast Schiller Avenue, Southeast 17th Avenue and back to McLoughlin Boulevard. November 6, dead, 3 injured in crash on the Dan Ryan One person was killed and three others were injured in a crash near 85th Street on the Dan Ryan Expressway as at least four other accidents slowed South Side traffic during the morning commute, authorities said. One of the injured was a child, who was taken to Comer Children's Hospital. Three adults were taken in serious-to-critical condition to Northwestern Memorial Hospital, according to authorities. The Cook County medical examiner's office said one of the injured later died but had no other information. The accident happened about 5 a.m. Other crashes were reported in the inbound lanes at 43rd Street, 47th Street, 63rd Street and on Interstate 57 at Wentworth Avenue. One person was injured in the 43rd Street crash, and the driver of a semi that caught fire on I-57 suffered non-life-threatening injuries. 2 / 36

3 November 3, Truck Driver Was on Facebook at Time of Fatal Crash, Cops Say The trucker who crashed his semi into parked emergency vehicles on an Arizona highway was looking at photos of scantily clad women on Facebook at the time of the crash, according to investigators who examined dashcam video of the incident. Arizona Department of Public Safety officer Tim Huffman was killed on May 6 when Juan Espinoza drove his empty fuel tanker down Interstate 8 highway at 65 miles an hour and plowed into three police cars and two fire trucks that were responding to a roadside accident. Video of the incident shows Espinoza at the wheel of the 18-wheeler at 5 p.m., but his wallet was blocking the view of the dash camera that was pointed at him. According to documents from the investigation, police say the 33-year-old trucker intentionally placed his wallet there to block the camera while he was "looking at or manipulating his phone." November 1, Fatal truck-car crash closes northbound Dan Ryan near I-57 A fatal crash involving a truck and a car has closed the northbound Dan Ryan Expressway on the South Side this afternoon. Illinois State Police were at scene of the fatal traffic crash on northbound Interstate 94 at 91st Street that happened about 2 p.m. All northound lanes were closed as of about 2:45 p.m., State Police said. Interstate 57 northbound traffic was being diverted at Halsted and Interstate 94 northbound traffic is being rerouted via I-57, 3 / 36

Video from the scene showed the cab of a semi-tractor dump truck heavily damaged, with a maroon or red sedan stopped just south of it, with moderate damage on its passenger side. Skid marks followed the truck from the right lane to where it had stopped crossing all lanes of northbound traffic, pointing toward the left shoulder. Check back for updates. October 29, Oswego Man Dies in Route 52 Accident A 53-year-old Oswego man died early Monday morning after colliding with a semi truck in Kendall County. The accident occurred at 7:54 a.m. at the intersection of Grove Road and Route 52. The Lisbon Seward Fire Department responded to the accident and called Minooka and Troy fire departments for assistance with the extrication accident. According to state police, the man was driving southbound on Grove and went through a stop sign onto Route 52, where he collided with a semi truck headed westbound. Those were the only two vehicles involved in the accident, state police said. The roadway is now cleared. October 28, Freeway lanes reopened day after fiery truck crash; two injured Two southbound lanes of a Los Angeles-area highway closed after a truck crash spilled thousands of gallons of burning fuel reopened Monday, officials said. 5 the lanes on Interstate 710 in Commerce about 5 a.m., nearly 20 hours after the accident, which injured two men and set fire to six other vehicles, the San Gabriel Valley Tribune in West Covina reported. No homes were reported damaged. Both sides of the highway were closed Sunday after the tanker truck hit an overpass guardrail, caught fire and dumped blazing fuel onto the highway below, the San Gabriel Valley Tribune in West Covina reported. The northbound lanes of the freeway were closed for about an hour. The driver of the truck and a man walking nearby received minor to moderate injuries, Los Angeles County Fire Department Inspector Brian Jordan said. A California Highway Patrol incident log indicated the truck driver may have lost control of the rig because of a blowout, the Los Angeles Times reported. October 26, One killed in rollover truck accident A 52-year-old man from out of state was killed Friday afternoon in a one-vehicle accident on U.S. 33, east of Celina-Mendon Road. For an unknown reason, a 2007 GMC Budget rental box truck traveling westbound went off the north side of the two-lane road, according to a press release from Mercer County Sheriff's Office. The driver overcorrected, brought the truck back onto the road and overcorrected again, causing the truck to overturn. The driver was ejected from the truck and became pinned under it. He was pronounced dead at the scene by Mercer County Coroner Dr. Timothy Heinrichs. A male juvenile passenger was also inside the truck and was transported to Joint Township District Memorial Hospital by Celina EMS with non-life-threatening injuries, according to the report. Mercer County Sheriff's deputies and emergency responders from Mendon and Celina fire departments responded to the call at 4:03 p.m. The accident is under investigation. As of press time, the victim's name had not been released pending notification to the family. No other injuries were reported. The accident marks the fourth traffic fatality this year in Mercer County. 5 / 36

6 October 22, Truck crash blocks Illinois Route 53 in northwest suburb A truck hauling garbage crashed on Illinois Route 53 this morning, blocking all southbound lanes of the major northwest suburban artery near Rolling Meadows and injuring the truck driver, authorities said. The tractor-trailer truck was hauling 72,000 pounds of garbage when it turned on its side south of Euclid Avenue near Rolling Meadows about 9:20 a.m., according to Illinois State Police. The driver was taken to Northwest Community Hospital with non-life-threatening injuries, according to State Police. The southbound lanes of Route 53 were open by a little after 1 p.m. October 22, Metra trains running on Milwaukee West Line after crash Metra trains were running on normal schedules this morning on the Milwaukee West Line, a day after a semi pulling a trailer of cars was struck by an inbound train at a crossing in Bartlett. At least one train car was derailed, but the crossing was cleared overnight and Metra said "normal operations" were expected during the morning rush. Two passengers suffered minor injuries. The accident happened around 3:35 p.m. Monday, sending cars flying off the trailer and striking cars in a Metra parking lot, police said. "We're really lucky no one was walking to their cars at the time," Bartlett Police Cmdr. Mike McGuigan said. The wreckage blocked both inbound and outbound tracks for hours. 7 October 21, Trains slowed but running after truck crash on Red Line A truck cab crashed into a barrier on the Dan Ryan Expressway and struck a section of the newly minted third rail of the Red Line overnight, but the damage was repaired and trains were running this morning. The accident happened in the southbound expressway near 68th Street around 10 p.m. Sunday, the first day of the reopening of the south branch of the Red Line after a five-month reconstruction. Trains were slowing to 35 mph through the crash site this morning. CTA spokeswoman Tammy Chase said the rail will be replaced Monday night, allowing trains to run up to 55 mph on that stretch Tuesday. The truck cab landed on the CTA side of the barrier, with most of the cab coming to rest between the barrier and the third rail, according to Chicago Fire Department spokesman Larry Langford said. A car carrier being pulled by the cab remained on the expressway side of the barrier. The driver was not injured, Langford said. October 17, Major truck crash closes I-64 westbound HUNTINGTON A crash involving two, tractor-trailer trucks has shut down all westbound lanes of Interstate 64 in Huntington, according to Cabell County 911. No injuries were reported. Dispatchers received word of the crash at 4:17 a.m. Thursday. The accident happened at the 9.5-mile marker, located just east of the 5th Street Road exit in Huntington. Officials were diverting westbound traffic off the Hal Greer and 29th Street exits. The crash involved a box truck owned by the U.S. Mail and a tanker truck carrying an unspecified substance. Dispatchers said the box truck overturned due to the crash s impact and was leaking fuel from its tanks. One of the trucks also hit a bridge and knocked off a chunk of its concrete wall. 7 / 36

8 Crews, just before 6:30 a.m., were waiting for another truck to arrive to offload contents of the mail truck, which remained on its side and needed turned upright. October 16, Downtown crash affecting Lake Shore Drive traffic Authorities have stopped southbound traffic on Lake Shore Drive after a city truck crashed near East Walton Street. Police are diverting southbound traffic at North Avenue, and a northbound lane is also shut down for about a block, police said. About 2 p.m., a semi truck belonging to the Department of Streets and Sanitation flipped in the southbound lanes, Chicago Police Department News Affairs Officer Michael Sullivan said. No injuries were reported, Sullivan said. Authorities have brought the truck upright again and expect the closed lanes to reopen by about 3:30 p.m. October 16, Semi-truck accident near Oelrichs Approximately 25 cattle perished in a semi-trailer truck accident late Thursday night, south of Oelrichs near mile-maker 8 on Hwy According to Fall River County Sheriff Rich Mraz, the cattle truck, driven and owned by Wilford Kurtz, 45, of Miles City, Mont., was southbound when it rolled onto its right side as it neared the railroad crossing through a construction zone. The 911 call was made by a passing northbound motorist and called into Fall 8 / 36

9 River County dispatch at 9:54 p.m. The cattle truck came to rest on the west shoulder of the highway and also on railroad tracks, blocking both one lane of traffic and the DM&E railline. Sheriff Mraz said when he arrived on the scene, personnel spotted an oncoming train in the distance but were able to notify the railroad in time to stop the train and avoid a collision. There were a total of 94 head of cattle in the trailer at the time of the accident. The driver had to be extricated by members of the Oelrichs Fire Department. He was then transferred to Chadron Community Hospital. Sheriff Mraz said Kurtz was reported to be pretty banged up but he is not believed to have sustained life-threatening injuries. October 15, injured when UPS truck crashes, spills packages Three people suffered non-life threatening injuries when a UPS semi-tractor truck was involved in a crash with two other vehicles that caused scores of packages to spill out onto the Reagn Tollway, authorities said. The crash on westbound Interstate 88 about 7:15 a.m. just east of Illinois Route 83--Kingery Highway--sent three people to local hospitals, said Illinois State Police Master Sgt. Sergio Ortiz. Lanes were blocked to pick up packages spilled when one of the trailers on the semi split open when it fell on its side, Ortiz said. 10 SHIAWASSEE COUNTY, MI A 43-year-old Morrice man suffered serious, but non-life threatening injuries after being struck head-on by a semi-truck during a multi-vehicle accident, according to a news release from the Shiawassee County Sheriff's Office. Police say the accident occurred on M-52 near Grand River Road at about 4:30 p.m. Friday, Oct. 11 in Bennington Township. The semi-truck was traveling southbound in the northbound lane of M-52 when it struck a 2000 Ford Taurus head-on, which was traveling northbound on M-52, according to the release. The semi-truck then crossed M-52 and struck two trees on the west side of the road. Perry Fire and Rescue freed the driver of the Taurus using the Jaws of Life after he had been entrapped in the vehicle, police say. He was taken to Sparrow Hospital in Lansing having suffered serious injuries. October 11, Truck accident causes fuel spill on I-20, stalls traffic for hours An 18-wheeler jackknifed and spilled 100 gallons of diesel fuel shutting down eastbound Interstate Highway 20 on Thursday afternoon. Around 4 p.m. Dallas Fire-Rescue were called out to the scene where the truck hit two other vehicles, causing an additional 50 gallons of motor fuel to leak onto I-20 at Highway 67 in Red Bird. The crash trapped a woman in a car. It took 10 minutes for crews to free her and she was taken to an area hospital with non life-threatening injuries. No one else was hurt. Crews have just begun the process of moving the vehicles. A HazMat Team responded to the scene and absorbed what they could of the fuel, but had to pump 50 gallons from the 18-wheeler s tanks before it could be moved. 11 southbound exit ramp LAS VEGAS (FOX5) - Troopers said a tractor trailer rolled onto its side as it was trying to exit the freeway near the Spaghetti Bowl interchange late Wednesday night. The crash happened about 10 p.m. on Interstate 15 southbound at the U.S. Highway 95 southbound exit ramp. According to the Nevada Highway Patrol, a fluid leaking from the truck was identified as a cleaning solution. The highway was closed for several hours while crews worked to right the semi. No injuries were reported. October 3, Route 80 west closed due to tractor trailer and dump truck crash, State Police say PARSIPPANY An accident involving a dump truck and a tractor trailer has shut down Route 80 west as of 9:20 a.m., State Police said. Route 280 east parsippanya crash involving a dump truck and a tractor trailer has shut down westbound lanes on Route 80 near Route 280, the State Police said.google Maps Update: All lanes are open on Route 80 west. The crash occurred near Exit 47A for Route 280 in Parsippany at about 8:45 a.m, State Police said. No injuries have been reported, but all lanes are closed, State Police said. The state Department of Transportation reported on 511nj.org a delay of 35 to 40 minutes as of 9:45 a.m. and that traffic is currently passing in the shoulder. 12 October 1, Fiery Semi Crash Closes Southbound I-55 Two semi trucks collided, causing an explosion early Tuesday morning along Interstate 55, according to Illinois State Police. The accident happened just before 7 a.m. in the southbound lanes of I-55 south of U.S. Route 6 in south suburban Channahon, police said. One of the trailers caught fire after the crash, which then spread to the other trailer, officials said. State Police believe one of the trailers was carrying natural gas. No injuries were reported as of Tuesday morning but southbound I-55 was shut down from Route 6 to Bluff Road. September 27, Two truck crashes cause I-65 traffic concerns A fiery wreck early Friday involving two semis on Interstate 65 snarled traffic in the northbound lanes near exit 188 for the morning commute, prompting Indiana State Police to encourage northbound traffic to find an alternative route. By 11:50 a.m., Indiana State Police reported that all northbound lanes were open after more than 11 hours of investigation and cleanup. James S. Wilson s northbound semi developed mechanical problems just before 1:30 a.m., so he pulled onto the interstate s shoulder south of the Brookston/Fowler exit and turned on his flashers, according to state police. Wilson, 32, from Missouri, got out of his rig and was standing by the passenger side of the truck near the front of the tractor when it was struck by a northbound semi driven by Vivian George Rose, 59, of Illinois, according to state police. 12 / 36

13 September 26, Williamstown man injured in New London accident NEW LONDON, Mo. -- A Williamstown man was injured in a Wednesday morning accident involving a semi on U.S. 61 at New London. The Missouri State Highway Patrol reports a semi truck driven by Joshua E. Justice, 31, of Williamstown, and a 2003 Volvo tractor driven by John M. Alberts, 52, of Hannibal, were southbound on U.S. 61 at 8:20 a.m. The Volvo entered traffic from Route A and was accelerating when it was struck in the rear by the truck. Justice was transported by Marion County Ambulance to Hannibal Regional Hospital. The patrol was assisted by the Ralls County Sheriff's Department and Marion County Ambulance. September 24, Semi hits cow on I-380, causing crash GILBERTVILLE Black Hawk County sheriff's deputies have released more information about the semi rollover that closed down Interstate 380 near Gilbertville Tuesday morning. The crash happened about 4:30 a.m. near mile marker 64. A semi being driven by William Bruner, no age given, of Illinois, struck a cow in the southbound lanes of I-380. Bruner's semi was damaged, but he was able to safely stop his truck on the side of the road. He was uninjured. A second semi, also southbound on I-380, being driven by Michael O'Neal of North Liberty struck two other cows. This caused him to drive through the median before his truck rolled onto its side and came to rest blocking both northbound lanes of the interstate. O'Neal received minor injuries; he was treated at the scene but was not transported to the hospital. 13 / 36

14 September 20, Semi accident on I-70 An accident sent a semi into the median of Interstate 70 at about 3:45 p.m. Friday east of Indiana 1 near the overpass over Jacksonburg Road. Wet pavement from afternoon downpours led to the accident. Senior Trooper Kyle West of the Indiana State Police said the semi was traveling westbound when it hit a patch of water on the west side of the overpass, hydroplaned and the driver lost control. The truck slid across the left lane and had a head-on collision with the guardrail adjacent to the eastbound lanes, stopping the truck from entering eastbound traffic. It could have been a lot worse, West said. The semi driver, Samantha Wilder, 47, of Interlachen, Fla., was taken to Reid Hospital for treatment but had no life-threatening injuries, West said. September 17, Galesburg woman killed in I-55 crash PONTIAC The woman killed in a head-on collision near Pontiac was identified Tuesday as Anne Sperry, 35, of Galesburg. Sperry s minivan was southbound on Interstate 55 when it crossed through the grassy median at 3:56 p.m. Monday and collided with a northbound semitrailer truck, which was pulling a trailer, just south of the Illinois 116 exit into Pontiac, state police said. Sperry was pronounced dead at 4:30 p.m. at the scene, Burke said. Truck driver James Westmoreland, 65, of Lakewood, Calif., was not hospitalized. The accident remains under investigation by the state police. 14 / 36

15 September 15, Chicago man killed in I-80 truck crash A Chicago man is dead after he crashed his truck on a bridge at 6:13 a.m. Saturday on Interstate 80 in Hubbard Township. Larry J. Johnson, 57, was driving the truck westbound on I-80, west of U.S. Route 62 when he went onto the left side of the roadway, hit the edge of a bridge rail and flew over the bridge, according to a press release issued by the Ohio State Highway Patrol in Trumbull County. The tractor-trailer came to a rest at the railroad tracks below the bridge and became engulfed in flames, the release said. It is still unknown what caused Johnson to travel off the roadway. Debris from the accident forced cars on I-80 to slow down, which caused numerous collisions. The eastbound side of I-80 was closed for an hour and a half for cleanup and for the Ohio Department of Transportation to declare the bridge safe for traveling, according to the release. September 12, One dead, three hurt in semi crash DOVER A man died instantly, and three other people were injured, after a Pontiac Grand Prix sedan was crushed under a jack-knifing semi truck on Indiana 32 in front of Pleasant View Church, just west of CR 500 W, Wednesday. Anthony Cowell, 60, of Crawfordsville, was identified as the victim of the crash. He was the only occupant of the Grand Prix. Investigators determined that the driver of a semi-truck, pulling an unloaded grain trailer west on Ind. 32, tried to stop quickly because of a slowing vehicle, said Boone County Sheriff s Office Cpl. Chris Burcham. The semi-truck skidded across the two-lane highway; Cowell was 15 / 36

16 unable to stop in time and crashed into the right side of the truck s frame, just ahead of its rear wheels. Cowell suffered massive injuries to the head and throat, Advance Fire Chief Jim Caldwell said at the scene. September 9, Yorkville Truck Driver Dies from I-80 Crash Injures A Yorkville man who lost control of his truck Monday morning on Interstate 80, near Minooka, has died from injuries sustained in the crash, Illinois State Police Master Sgt. Wilfredo Rivera said. Joseph J. Drwal, 35, died at Advocate Good Samaritan Hospital in Downers Grove, where he'd been airlifted following the 9 a.m. accident, Rivera said. Police still do not know what caused Drwal to cross from the westbound to the eastbound lanes of traffic, causing his semi-truck to roll and spill its cooking oil contents, he said. While no other vehicles were involved in the crash, pieces of debris did come loose from the truck and struck other vehicles, he said. No one else was injured. September 8, Semi truck driver killed in Indiana accident COVINGTON, GA - State police say a truck driver was killed when the semitrailer he was driving west on the Indiana Toll Road veered off the interstate and struck a concrete bridge deck. The victim has been identified as 48-year-old Michael Hall of Covington, Ga., about 16 / 36

17 35 miles east of Atlanta. State police say the accident happened about 9:35 a.m. Sunday in Porter County, about 5 miles west of State Road 49. Police say they don't know what caused Hall to veer off the highway. Hall was ejected and pinned under the box trailer. He was pronounced dead at Porter Regional Hospital in Valparaiso. The semi was loaded with frozen dinners that were strewn across the median and road. August 27, Semi Vs. Car Accident Closes Route 126 The Oswego Fire Department responded to the accident at Schlapp and 3:36 p.m. for an accident that involved a passenger vehicle and a semi-truck. The passenger vehicle sustained major damage with required extrication of three patients. A total of five patients were treated, reported the OFPD, although there is no word on the condition of the patients. One patient was transported via Lifestar Helicopter. The Oswego Fire Protection District was assisted by Plainfield Fire Department, Bristol-Kendall Fire Department and Aurora Fire Department. August 26, Three people injured, one critically, in Brooklyn truck accident Three people were injured, one critically, Monday in Brooklyn in a car crash, officials said. The motor vehicle accident left a gold sedan so smashed up that its entire top was missing. 17 / 36

18 The car collided with a truck on Bushwick Ave. and Eastern Parkway about 1:45 p.m., according to witnesses and officials. August 16, killed, including 4 kids, when Jeep crushed by semis Seven relatives, including four young children, were killed when their Jeep was crushed between two semis on Interstate 65 near Roselawn, Ind., about 70 miles from Chicago. The accident happened around 10 p.m. Thursday when a northbound semi and the Jeep stopped for a traffic backup near a work zone, according to Indiana State Police. A semi behind the Jeep failed to stop and plowed into the SUV, pushing it into the back of the other semi, state police said. The Jeep exploded into flames and the fire spread to both trucks, state police said. Initially, police and fire crews could only see two bodies in the Jeep because it was so badly damaged and burned, authorities said. It was hauled to the Keener Township Fire Department and taken apart, and, over the course of several hours, emergency workers discovered five more bodies. August 12, Semi rolls over on I-65 southbound near Lafayette The trailer of a semi truck flipped on its side Monday morning during a crash on Interstate 65 between the Indiana 38 and Indiana 26 exits near Lafayette. The crash happened around 18 / 36

19 7:10 a.m. in the southbound lanes when a semi tractor with two trailers swerved to evade a deer and another car in the roadway, Indiana State Police said. During the maneuver, the rear trailer tipped onto the driver s side and came to rest on the right shoulder of the road. August 7, Semi rolls over on Indiana highway Jeff Price paced along Interstate 65 Wednesday morning as he talked on the phone, his lower leg bleeding slightly from small cuts. About 30 minutes earlier, Price had been driving his semi north on the interstate and didn t notice traffic slowing in front of him. He tried to brake, and to avoid a collision, he drove onto the shoulder, where he lost control of the rig, Indiana State Police Sgt. Kim Riley said. The tractor-trailer rolled off the side of the interstate and onto the passenger side of the rig about 6:45 a.m., Riley said. Price, 42, of Joshua, Texas, was treated at the scene, Riley said. Price s co-driver, Goree Lyn Carr, 43, of Dallas, Texas, was asleep in the bed inside the truck s cabin when the accident happened. Carr, who suffered minor shoulder injuries, was taken to IU Health Arnett, Riley said. A hospital spokeswoman said Carr was treated and later released. August 5, Coal City cop dies in semi crash A recently hired Coal City police officer from Oswego died Saturday night after a crash in rural 19 / 36

20 Livingston County, according to a news release issued by Livingston County Deputy Coroner Marie Margherio. Heather K. Wandt, 30, was traveling northbound on Illinois Route 47, 1/2 mile south of Saunemin, shortly after midnight when she attempted to pass a semi-trailer and collided with an oncoming pickup truck in the southbound lane. Margherio said Wandt was pronounced dead at the scene. August 2, Greyhound bus hits semi-truck on I-94 CLIMAX, Mich. (WOOD) - Twenty-two people were injured, one critically, after a semi-truck and a Greyhound bus collided on I-94 late Thursday morning. The wreck happened around 10:30 a.m. in the westbound lanes near Exit 88 to 40th Street in Climax. Kalamazoo County Sheriff Richard Fuller told 24 Hour News 8 at the scene that the bus rear-ended the semi-truck and became impaled on the trailer. "The truck driver was able to guide the accident to the edge of the road, being that the two vehicles were connected," Fuller said. July 26, Truck Smashes Into Bridge; Cemetery Road Closed Until Further Notice A semi-trailer truck that was too tall struck and damaged a bridge on Cemetery Road east of Tabler Road and west of McLindon Road on Thursday afternoon in Aux Sable Township, not far from Channahon. An eyewitness said, "The truck driver drove under the bridge. But his truck was too tall for the bridge. It's 12 feet it's marked clearly on the signs. His truck had to be 13-7 or Anybody who drives a truck knows you can't make that." Illinois State Police, the Grundy County Sheriff's Department, Channahon Police and Minooka Police all responded to the accident and were working to divert traffic on the scene.
