2 Student Health Services Eligibility and Requirements The Health Center serves currently registered students with a PEHR on file, when the college is in session. In order to utilize all Health Center Services, students are required to submit a completed Health Form and Physician's Certificate (medical form) one month before classes begin. This medical form is to be completed by the student and their family physician. The physician's office should mail the completed form directly to the Health Center. The Health Center provides non-emergency acute care with emphasis on preventive health care and health education. NYS Public Health Law - Immunization Requirements New York State Public Health Law requires students born on or after January 1, 1957 to provide proof of immunity to Measles, Mumps and Rubella. Also since August, 2003 all students have to submit a completed Meningitis Information Response Form after reading an accompanying document. Proper documentation for both laws is to be received in the Student Health Center before the first day of classes. Documentation that is not received within 30 days from the start of your first semester at New Paltz can lead to de-registration, and prevent a new registration. Allow three to five working days for data to be processed after it has been received. Appointments It is recommended that you call the Health Center for an appointment in the early stages of your illness. To assist you in obtaining the earliest available appointment, please call the Health Center before 10:00 a.m.

3 If you have an urgent medical problem, we will advise you if medical care can be provided at the Student Health Center or must be sought elsewhere. If necessary, we will refer you to a local hospital emergency room. After-Hours Help for Urgent Medical Issues If you call the Health Center after hours because you have an urgent medical problem, you will be connected to our answering service. The particular health insurance plan that you have will determine where you are referred for medical advice and/or care. You should expect to be asked to identify your health plan for the answering service. Students who identify themselves as having the University s health insurance plan (Maxon), or Medicaid will be connected by our answering service to a medical provider at the Family Practice Center of New Paltz. All of the students who have some other health insurance plan will obtain assistance from the physician on-call for the Student Health Center. Education You don't have to be ill to call the Health Center. As a student you're the person most responsible for your own health. The Health Center has information on a broad range of topics to help you live a healthier life. We encourage you to ask us how you can achieve and maintain good health. Records Your medical records are confidential. They are separate from your academic records. Civil law limits access to your medical records. Requests for copies of your medical record need to be made in writing. Within 10 working days we will respond by mail. In urgent situations, records will be faxed to designated medical providers with your written consent.

4 Laboratory Some medical conditions require special tests. We'll explain them to you if you need them. Your personal physician may arrange laboratory tests by calling Dr. Peter B.T. Haughton, the Health Center Director. Medications When you're seen for an illness, you may be given a limited supply of commonly used medications for your immediate relief. When you are well enough to fill any prescription given by the Health Center Staff, you may do so at a pharmacy in town. This expense may be partly covered by your health insurance either directly to the pharmacy or by reimbursement to you. Because of this new information, we strongly recommend that freshmen see their family physician or pediatrician and obtain this

5 vaccine before moving onto campus. The vaccine that is currently in use is very safe and its widespread use would have been anticipated to prevent 50% or more of the cases that occur in this age group each year. Public Health experts strongly urge that we continue to bring this information to you and that you act on it in a timely manner with your family and personal physician. Responsiveness At the Health Center, we take your feedback seriously. Your evaluation is essential to improving our services. Please write down any suggestions you have and send them to the Health Center. Follow-up care and continued treatment depend on your compliance. If you are unable to keep a scheduled appointment, please call as soon as possible to cancel so that the appointment can be assigned to another student. Health Insurance Every student needs some form of health insurance. You don't need insurance to visit the Health Center. Health Insurance is needed for prescription medications, lab tests, x-rays, specialty physician fees, hospital stay, or Emergency Room referrals. If you have purchased the University Student Health Insurance or the Health Insurance for International Students, our staff will help you complete the claim forms and answer questions concerning your insurance coverage. If you have other insurance, including Health Maintenance Organizations, you will need to contact that company s agent.

6 All claims, medical bills and receipts should be processed as soon as you get them. If you have any questions about these, please call and ask us for help. Some insurance plans, including the Student Health Insurance available through the University, provide partial coverage for prescription medicines. We encourage you to confirm that a local pharmacy accepts your prescription plan and to check your individual plan for particulars of coverage. You should understand your insurance coverage and/or prescription plans. You should have available the necessary cards, policy numbers, phone numbers for use in the event of illness or accident. Non-Emergency Transportation The Health Center does not provide transportation to off-campus medical facilities. When you are referred by the HLC to an off-campus medical facility you may use vouchers from the HLC for taxi service to get you to the recommended medical treatment facility. You must reimburse the Campus Auxiliary Service for these vouchers within thirty days. After hours, when this transportation is authorized, vouchers may be obtained from University Police.
