2 Offense-Defense Football League National Championship IT S TIME TO PLAY SOME FOOTBALL!! You ve practiced all season, now the REAL GAMES begin. The Offense-Defense Football League National Championship brings the best-of-thebest from all over the US to battle it out for the ODFL Championship Title! The top teams in each age group from a variety of leagues will come to play the National Championship in Orlando, FL during the Offense-Defense All American Bowl Week between Christmas and New Years. Bring your best Play your hardest Cause this game is FOR REAL! GENERAL INFORMATION Dates: December 30, 2014 January 4, 2015 Ages: 15&Under 12&Under 10&Under 8&Under 6&Under *Teams are AGE PURE. A player may play up an age but a player may NOT play down an age. There will be no exceptions. Players must present birth certificate at registration to verify age. 3 REGISTRATION Cost $300 PER TEAM ($400 for non-odfl members) Once a team books an entire teams block of rooms through O-D in one of the specified O-D host hotels, that $300 deposit will go toward the teams hotel rooms block. These rates will be cheaper than if you book on your own. REGULATIONS AND REQUIRMENTS All teams must have a birth certificate for each player, Current Photo taken during this calendar year, a current sports physical and a roster available at the field each day. To accommodate any necessary re-verifications, allow for adequate warm-up and ensure that the games start in a timely manner, all teams must check-in at the appropriate field two (2) hours before their first game, and one (1) hour before each additional game. 3 of 9

4 The ODFL National Championship will take place Dec 30, 2014 to Jan 4, 2015 during the O-D All-American Bowl Week in Orlando, FL. These are the points of interest concerning the event: Director of the event will be Craig Smith - ODFL Commissioner. The location of the event for the 2014 season will be the Apopka Recreational Complex north facility in Apopka, FL. The event will consist of five (5) division age groups (6U, 8U, 10U, 12U, 15U) (with four teams playing in each division). The teams will be limited to 25-players and 4-coaches per team. Each team that enters the championship will be guaranteed at least two (2) games. The cost for an ODFL team participating is $ That fee will be transferred to the team lodging cost if the team resides in an O-D event hosted hotel. For ODFL teams that resides elsewhere the registration fee will also be $ non-refundable deposit. For non-members of the ODFL the fee will be $ non-refundable deposit no matter where they stay. The teams will take part in the Bowl Week Award Ceremony where the team will be recognized and each player will receive a medal for attendance. The event winner and runner-up for each age group will receive a team trophy and each member of the winning team will receive an individual trophy. 4 of 9

7 OFFENSE-DEFENSE SPORTS Waiver and Release of Liability Minor ASSOCIATION NAME - READ BEFORE SIGNING IN CONSIDERATION OF, my child/ward, being allowed to participate in any way in Offense-Defense Sports and related athletic sports programs, events, and activities, the undersigned acknowledges, appreciates, and agrees that: 1) The risk of injury to my child/ward, myself, from the activities involved in these programs is significant, including the potential for permanent disability, paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2) FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLICENCE OF THE RELEASEES or others, and assume full responsibility for my child/ward s participation; and, 3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant concern in my child/wards', readiness or, hazard during my presence or participation, and/or in the program itself, I will remove my, child/ward, from participation and bring such to the attention of the nearest official immediately; and, 4) I, for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Offense-Defense Sports, their officers, directors, officials, volunteers, agents, and/or employees, other participants, sponsoring agencies, tournament host, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ( RELEASEES ), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, incident to my child/wards', involvement or participation in these programs, Whether arising from the negligence of the releasees or otherwise, TO THE FULLEST EXTENT PERMITTED BY LAW. 5) I, for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child/ward s involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. 7 of 9
