Further, I hereby grant permission in case of injury to have an athletic trainer and/or doctor residing in the United States to provide me with medical assistance and /or treatment. I certify that I am in good physical condition and have no disease or injury that would impair my performance or physical condition in training for and participating in the competition. I agree to accept any and all financial obligations incurred as a result of any medical assistance, treatment and related expenses, provided in connection with any injuries which I may received in the competition. It is agreed that it is compulsory and mandatory that this liability waiver, release, and indemnification agreement be fully completed as a precedent to this official entry form being accepted, and that the completed liability waiver, release, and indemnification agreement is incorporated by reference as a part of the official entry form. All other events any belt is okay o All players will wear one of the following belt colors according to USA Taekwondo KEUB Ranking. o Following Belts with colored stripes are acceptable to wear (i.e. Red Belt W/ white stripe down middle). o Recommended black and Deputy Black Belts will need to wear the Red Belt. Sparring Equipment: o All competitors will provide their own sparring equipment o All competitors must wear a clean WHITE uniform, v-neck only style for sparring. o All competitors must wear full sparring gear during the match: o White Head gear, chest pad, Fore-arm pads and shin pads or matching colors for gear only BLUE or RED o Clear mouth piece and groin guard (worn under the uniform pants.) o No Taekwondo shoes allowed during sparring matches. o Uniform MUST cover the sparring pads for safety. o A $10 Electronic sock rental fee applies for all those who use the KP&P EVO E.B.P. system. * TOE AND FINGER NAILS MUST BE CUT SHORT IN ACCORDANCE WITH THE WTF RULES FOR SAFETY!! Coaches: o Coaches will follow the USA Taekwondo s code of conduct rules and regulations. o Players will need to have their coach ready when their match is called. o Matches cannot and will not be delayed for coaches for any reason. o A player will have one minute to get a coach or they will compete without one or be disqualified. o Misconduct by a coach/parent will affect their player s match including up to disqualification. Video Taping and Photography: o Parents are welcome to video tape their players and fellow students. o Videotaping and photography will be done in designated areas outside the boundary markers. o Any photography or taping that interferes with the tournament operation is forbidden. o SPECTATOR VIDEO IS NOT ALLOWED TO CHALLENGE THE DECISION OF ANY OFFICIAL OR OUTCOME! --Please do not bring it to the tournament directors as you will be turned away! Sound System: o The ballroom sound system is only for the tournament staff for the function of the overall tournament. o Creative and weapons forms are forbidden to use the ballroom sound system for individual use. o Demo Teams will be allowed use of the sound systems. 3 Demo Team: Sparring 4 TH EVOLUTION EVENTS: o Each team will have 8 minutes to perform their routine. o Set-up and take-down is counted as a part of their 8 minutes. o Each team will provide their own: music, props, boards and support staff. o NO FIRE OR WATER WILL BE ALLOWED! NO Confetti or Fruit o Judging will happen on overall creativity, entertainment, and rule following. Players are bracketed by: gender, weight, and belt rank (in this order). NO KNOCK OUTS ARE ALLOWED AT ALL FOR ANY AGE OR RANK. Players who weigh less than 60 lbs. will NOT use the EVO EBP Armor. (Please bring own sparring gear) We will be using the WTF Olympic Sparring rules and USA Taekwondo s Junior Safety rules for head kicks. (10 yr + Blackbelts head kicks 12 yr + colored belts head kicks) Colored belt matches: two 60 second rounds W/30 Sec. break in between. Black Belt matches: two 90 second rounds W/ 30 Sec. break in between. Current WTF point gap will be used (12 points or more in the 2 nd or 3 rd rounds). Sudden death and superiority of judges will be used to determine a winner in the event of a tie. Poomsae All WTF and ITF poomsae s are allowed; no Japanese or Chinese forms permitted. Poomsae Division is a single elimination event. Players will compete two at a time, but individually scored. Players may choose which Poomsae to perform but more points will be awarded to those who choose a Poomsae that reflects their current rank more. NO music or boards will be allowed in the Poomsae division. Poomsaes will be judged based on the following criteria: o Correct body posture o Power o Balance between rhythm and speed. o Focus o Continuity (what you do to the left is what you do to the right) Weapons Forms Players will provide their own weapons for use. Time limit of 1 minute and 45 seconds per player. Weapons forms are a single elimination event. Players may use music and boards only if they provide the music player and boards. Use of fire, water, fruit, vegetables and/or confetti IS STRICTLY FORBIDDEN IN THIS TOURNAMENT. Weapons players will not have mats under them for their performance, players should plan accordingly. Players will be judged on their skill with the weapon and the overall showmanship. Speed Kicking The kick used will be the round or round-house kick at waist level. Page 3 of 5

4 Speed Kicking is a single elimination event. 12 years and younger will kick for 30 seconds, and 13 years and up will kick for 45 seconds. Kicking pattern will alternate between right and left leg in sets of two (right-right-left-left) Players with the most kicks win the match. A sudden death round of 15 seconds will be used to break any ties. Tiger Tails 8 years and younger will have a 30 second match, all others will be 45 seconds. Tigers Tails is a single elimination event. All players will be given 3 tails to place in their belts. NO purposely falling down or crouching to the floor is allowed. NO holding, tripping or pulling the player s actual belt is allowed. The player who keeps the most tails on them is the winner. If a tail falls out during the match it is considered pulled. Ties will be decided by the number of tails on the player and the number of captured tails. Foam Swords Long sword division is for 12 years old and up with some experience or more. Short foam sword division is for less than 12 years old with no experience. Foam swords are a single elimination round. NO clubbing or wild swinging/slashing of the sword is permitted. A helmet with a full face shield and the swords will be provided by the tournament hosts. Legal scoring areas include: the arm from shoulder to elbow, thigh to knee, head, and the trunk of the body above the belt. Illegal parts of the body: groin, center of back, hands, forearms, face, knees and below. Each match will be 45 seconds long for long sword and 30 seconds for short sword. Minor violations of the rules will result in a warning; the 4 th warning will be a disqualifying act. Gross violations of the rules will result in point loss; a second warning will result in disqualification. Disqualifying acts: Players who break any rule set forth by the tournament and as defined within. Any non-martial arts conduct or misbehavior while waiting for or during any event. Missing any of equipment specified above. Submitting your tournament application and fees the day of the event. Any announced rules added during the competition. Any inappropriate behavior from parents or spectators will have their player disqualified and face ejection from the tournament. For the first time ever in a Utah Taekwondo competition, The Utah Martial Arts Commission is please to bring in Dr. s Jeff Rocco and Dann Byck from Utah Orthopaedics of South Ogden. Dr. s Rocco and Byck specialize in sports medicine for the shoulder to the foot. The Utah Martial Arts Commission is dedicated to keeping all athletes in their best physical shape and empowering the parents with knowledge and tools to keep their athletes healthy for life. Utah Orthopaedics will be providing this year s competition s medical control and support for our players in this revolutionary tournament. Having highly skilled and specialized medical staff on hand will bring the very best in medical treatment and injury prevention like no other. Utah Orthopaedics will be presenting special 10 minute free seminars to address: Concussion awareness Total ankle and joint health Sports medicine ImPACT concussion testing. We will have several of these free seminars scheduled throughout the tournament. Utah Orthopaedics is dedicated to athlete s wellness as well as recovery. The Utah Martial Arts commission along with Utah Orthopaedics is dedicated to safe and fun martial arts competition environment.
