3 Whether the search for senior housing is prompted by a serious medical condition or the desire for a lifestyle change, finding the right place to live can be challenging for both the individual and their family. The earlier you assess the individuals current needs and how those needs may evolve over time, the more choices and control individuals and families have.

4 Staying at home has the advantage of keeping the individual in familiar place where they know their neighbors and community. There are a wide range of in-home care services that can help the individual maintain their independence including: Private duty Homecare (Companionship or Personal Care) Medicare Skilled Homecare (Nursing, PT, OT, ST, SW, HHA) Adult Day Care Home repairs or modifications to make life easier and safer Alternate transportation services Meals on Wheels

5 INDEPENDENT LIVING General name for any housing arrangement designed exclusively for seniors. Other terms include retirement communities, retirement homes, senior housing, and senior apartments. These are more compact, easier to navigate, and includes help with outside maintenance. Recreational centers or clubhouses are also normally available on site.

6 1. INDEPENDENT LIVING These enable active seniors to remain in their own home environment without having to rely on family and friends. Individuals can access specialized programs and services, such as transportation to the grocery store or doctor s appointments, home healthcare, various help with ADLs, as well as a network of social activities / programs with other community members. Suggesting Independent living would be appropriate if you see an individual needs assistance with ADLs, they would like a place requiring little upkeep, and/or like the idea of socializing with peers and having activity options nearby. Maybe they are isolated and battling depression...

7 2. ASSISTED LIVING - ALF Also known as residential care, board and care, congregate care, adult care homes, adult group homes. These ARE NOT Nursing Homes. Assisted Living (AL) is for those who need more support with a number of ADLs, including help with medications. Costs vary according to the level of help required, although staff is available 24 hours a day. Some AL facilities provide apartment-style living with scaled down kitchens, while others provide studio rooms. Shared rooms are available in most AL facilities to save on cost.

8 2. ASSISTED LIVING - ALF (cont.) Most have a group dining area, provide all meals, and have common areas for social and recreational activities. Transportation and staff are provided to help individuals with shopping, outside events, doctors appointments, etc. Suggesting an AL option would be appropriate for those needing more clinical or personalized care services than are feasible at home or in an independent living setting. Memory care is also supported in many AL communities as they offer specialized programming and staff that are trained and have experience working with cognitive impairments. They focus on the individual s Quality of Life and Daily Moments of Success.

9 3. NURSING HOME - SNF Skilled Nursing Facilities and Nursing homes are one in the same. The truth of the matter is there are 2 sides to every one of these facilities. There is a Nursing Home or Intermediate Care Facility side Hospitals make arrangements for follow-up patient care after an acute hospital stay, a la stroke or knee replacement surgery. When released from the hospital, a patient transfers to the skilled nursing home to receive hands-on care from nurses, physical therapists, occupational therapists, speech therapists, or any combination thereof until they re able to go home. This is traditionally covered by your Medicare part A benefit for the first 20 days then a percentage thereof beyond.

10 This side provides a room, meals, and 3. NURSING HOME - SNF (cont.) help for individuals with activities of daily living needs. Residents living in this area usually have physical or memory problems that keep them from living on their own or in an Assisted Living Facility. An individual without the financial resources to afford an Assisted Living Facility, even after all financial benefits have been explored, has no family in the area, but needs 24/7 care, a skilled nursing facility will meet their needs. It can be extremely expensive if paid for privately, but can be paid for with Medicare benefits, Medicaid benefits, and health insurance plans.
