To challenge a Clinical Competency Examination you must have achieved a certain minimum clinical experience and obtained appropriate faculty approval. Failure in any one part of a Clinical Competency Examination constitutes a failure of the examination as a whole. This Clinical Competency Examination specifically addresses the following GHSU-CDM Competencies: 1. Applies advances in modern biology and new medical knowledge to clinical practice. 2. Applies the principles of behavioral science that pertain to patient-centered oral health care. 3. Provide care to a diverse patient population 4. Communicate with patients, staff and colleagues concerning oral health care matters 5. Applies the ADA s Principles of Ethics and Code of Professional Conduct and the Laws of Georgia and the Georgia Board of Dentistry s Rules to the provision of oral health care services. 6. Use information technology for professional development and comprehensive patient care 7. Obtain informed consent from a patient, parent, or guardian. 8. Prescribes and administers pharmacological agents for anxiety, pain, infection and caries prevention. 9. Obtain and assess diagnostic information from clinical and radiographic procedures. 10. Develop ideal sequenced treatment plans and alternatives based on etiologies, prognoses and patient circumstances. 11. Manage periodontal diseases. Sr. Dx/Tx Plan Competency Page 1
2 The Senior Periodontal Case Competency is a test of the student s overall clinical diagnostic and treatment skills in periodontics. This competency will examine the student s ability to perform an accurate and thorough periodontal examination; determine a periodontal diagnosis and an overall and tooth-by-tooth periodontal prognosis; formulate a periodontal problem list and a treatment plan to address each problem; educate and motivate a patient in appropriate techniques to control etiologic factors; perform appropriate non-surgical and surgical periodontal therapy; re-evaluate a patient s periodontal status at appropriate intervals and determine whether the patient s periodontal problems have been successfully resolved; and design and implement an appropriate periodontal recall program for the patient. Prior to initiating treatment on the patient selected for this competency, students must have passed the Year 3 Periodontal Diagnosis and Treatment Planning Independent Performance Examination, and the Year 3 Periodontal Reevaluation Independent Performance Examination. Procedural Requirements: Case Assignment: Assignment of the Senior Periodontal Competency Case occurs in the Junior year following completion of at least 4 units of root planing, the Diagnosis and Treatment Planning and the Periodontal Re-Evaluation Independent Performance Examinations. Patient Selection: Ideally patients for the Senior Perio Case should present with a diagnosis of moderate to severe chronic periodontitis. There should be at least 2 quadrants in the mouth with pocket depths in the 5-6mm range with accompanying bone and attachment loss in the range of 3-5mm. Any patient chosen for use as a Senior Periodontal Competency must be approved by a full-time Periodontics faculty before the data base examination is initiated, and the Senior Periodontal Competency Case work-up and treatment plan must be approved and graded by a full-time Periodontics faculty before treatment is started. Senior Periodontal Competency Case Expectations: 1. Case must be approved by Periodontics faculty before work-up is performed. Axium D0180 note template will be used. 2. The Senior Periodontal Case Competency is evaluated at three times over the course of treatment resulting in three separate competency examination grades that must completed in this sequence: o Senior Periodontal Case Diagnosis & Treatment Planning Competency o Senior Periodontal Case Re-Evaluation Competency o Senior Periodontal Case Student Self Assessment Sr. Dx/Tx Plan Competency Page 2
3 3. The Senior Periodontal Case Diagnosis & Treatment Planning Competency includes a complete periodontal examination, formulation of a periodontal problem list, and treatment plan. All of which must be completed before any treatment is begun. 4. Students are expected to perform all periodontal treatment procedures (including maintenance) on their Senior Periodontal Competency Case patients. No periodontal treatment step transfers are allowed, referral to graduate periodontics for surgical treatments is permissible. Restorative treatment performed prior to completion of the reevaluation examination should be limited to those procedures which are associated with contributing local etiological factors (overhangs, etc.). 5. Senior Periodontal Competency Case patients should be appointed in the Periodontics Clinic at least once a month during the active phase of periodontal therapy (from the initiation of root planing to the post-surgical evaluation). 6. Full mouth assessment of plaque and inflammation is required documentation for each periodontal appointment and should be used for patient education and motivation purposes. 7. Oral hygiene instruction should be provided at every appointment. Re-instruction and motivation for the patient s oral hygiene practices should be given at each appointment. Oral hygiene instructions for the patient should include a combination of toothbrushes and appropriate interproximal oral hygiene aids. 8. Scaling and root planing should be completed by quadrant using both hand and ultrasonic instruments. No more than 2 units of root planing may be completed within a single week. 9. The following additional procedures should be completed before the patient is reevaluated Extraction of hopeless teeth Endodontic treatment Restoration of carious lesions encroaching upon the gingival tissues Periodontal Re-Evaluation of the Senior Periodontal Case should be performed after completion of all non-surgical periodontal therapy and those procedures listed above. Remediation: Students must pass all components of this competency. A failure of up to two specific criteria may be remediated at the time of examination. A failure of more than two evaluation criteria will necessitate repeating an examination. Sr. Dx/Tx Plan Competency Page 3

Provides thorough, appropriate use of information technology (electronic health record, digital radiography) for comprehensive patient care. Faculty Evaluation Obtains informed consent from a patient, parent, or guardian. Prescribes and administers pharmacological agents for anxiety, pain, infection and caries prevention. Specific Evaluation Criteria Faculty Evaluation Periodontal Examination is complete and accurate. Radiographic interpretation of periodontal findings is complete and accurate. An accurate Periodontal Diagnosis is established and documented. A Periodontal Prognosis is established for individual teeth and for the dentition as a whole. A complete Periodontal Problem List if formulated. A Periodontal Treatment Plan is formulated that addresses all of the periodontal problems. A Surgical Phase of treatment is proposed if indicated.
