1 HEALTH AND SAFETY SERVICES INCIDENT REPORTING - GUIDANCE NOTE Reporting of Injuries, Accidents & Dangerous Occurrences to Health and Safety Services This Guidance Note is intended to assist you in completing the University of the West of Scotland Incident/Dangerous Occurrence Report (Accident Report). The procedure below tells you how to complete each part of the form. Part A - Place, date, time of Incident Give as much information as you can. If you do not know the exact details, give as close an estimate as possible. Let us know that it is an estimate however. Give a contact name and number. Part B The Injured/Affected Person Again, give as much information as possible about the name, address and category (eg student, employee) of the injured/affected person. If you cannot obtain an undergraduate student s address, then their name, course and year is sufficient information. Where someone is injured, it is important that you tell us both the type of injury and the part of the body injured, eg deep cut to palm of left hand. It is also important to let us know if First Aid was given, who gave it and if the person was taken to hospital. Part C Indicate What led to Injury or Condition You should tick one box only and this should show as accurately as possible what led to the accident. For example, if someone tripped on the stairs while carrying a large computer box then you would tick the box Injured while handling, lifting or carrying and not fall on stairs If you are not sure which box to tick then leave Part D blank, give an explanation of what happened when you complete Section E and we will decide which box to tick. Section D Circumstances You must complete this section. Give us the details of what actually happened and what you think caused the accident. Let us know who else was there when
2 the accident happened and what their role was. If anything has been done to prevent the accident from happening again, tell us. If you do not have enough room to give us this information then continue on a separate sheet or on the blank side of the accident form. Tick the appropriate box to let us know you have done this. Make sure you fax all of the information to us. Finally, complete the form by filling in your name and signing it. This does not mean that you will be blamed if the form is incorrect. It does mean that we can contact you to discuss the accident if necessary. Part E - Do not complete We will complete this in Health & Safety Services.
3 EXPLANATORY NOTES ABOUT ACCIDENT REPORTING An accident is an event that results in injury or ill-health. In addition the University also requires the reporting of any incident that, while not causing harm, has the potential to cause injury or ill-health. A dangerous occurrence which is a specific, reportable adverse effect as defined in the Reporting of Incidents Dangerous Occurrences and Diseases Regulations 1995 is an example of an incident. they happen to University staff or students at work (whether in the University or on University business, eg, fieldwork, consultancy) they happen to members of the public or visitors either in the University or because of something done by staff or students in the University or on University business, eg, fieldwork, consultancy University property was (or was nearly) damaged Other people s property was (or was nearly) damaged because of something done by staff or students in the University or on University business, eg, fieldwork, consultancy If you are not certain whether an accident should be reported, please report it and let us make the decision on whether to follow it up or not. We at Health & Safety Services are required by law to collect these accident reports and, if necessary, to report certain accidents to the Health & Safety Executive or the Local Authority s Environmental Health Services. Examples of accidents which would be reported are, where someone died or broke a bone. We have to keep a record of all accidents, no matter how trivial (cuts, bruises, broken windows etc). We rely on you to help us to do what the law demands. A separate accident report should be completed for each person injured or, where nobody was injured, for each incident. Please complete the form clearly in ink. When you complete the form, do not worry about blame. No-one will be criticised on the basis of the form alone. If you discover the information you gave on the form was incorrect, tell us and we will change it. Similarly, if we find out that what happened was not as you understood it to be when you completed the form, we will change what was recorded.
4 We need to get the information as soon as possible after the accident so please the completed form to any member of the Health and Safety Services team, fax it to us on or, if in the Paisley Campus, hand deliver it to Health and Safety Services in the Brough Building. If the accident is serious, for example if someone is taken to hospital, let us know immediately by phone. If there is nobody in the office then ask Paisley Campus Security (Ext 3168) to contact us by phone or pager. We need the accident report to arrive at our office no later than one working day after the accident has occurred. If you do not have all the facts of the accident at that time, give us what you do have we can add the other details as we get them. Do not delay informing us while your Department/Unit carries out its own investigation. When you send us an accident report, keep a copy and pass it to your Head of Department/Unit or Department/Unit Safety Convenor for their files and, if necessary, to let them investigate.
