2 APPLICATION INSTRUCTIONS AND PROCEDURES July 15 Deadline for application for Fall start; Application fee $55 (US) The admissions process is on-going and applicants will be admitted on a rolling basis. The accuracy, completeness, and neatness of your application are determining factors in our admission decision. Send all materials together in one packet. The admissions committee will not evaluate an incomplete application. 1. Application Form Type or print clearly all information requested on the application form. A separate application to the University is not required. Your signature is required to validate the information you provide. 2. Transcripts Submit one sealed, official transcript with all current grades from each college and graduate school you have attended. Do not open transcript. Place the sealed transcript in your application packet. A final transcript of any course(s) in progress will be required, including graduation verification if applicable. OT program must have been completed with a 3.0 or higher GPA 3. Letter from Employer or Immediate Supervisor A secure letter of employment verification from a supervisor or employer is required. Include the letter, secured in an envelope and signed across the seal, in your application package. 4. Applicants must have earned a master s degree in occupational therapy (or a related field). 5. Letters: A. Letter of Intent 1) Provide a statement of intent to enroll in the Doctor of Occupational Therapy (OTD) program. 2) Describe your goals for earning a doctorate and how your degree will shape your future practice. 3) If you are a registered occupational therapist and not currently working as an OT, indicate in the letter the community partnership that you have created to allow for access to a setting that provides services to a client population with which you intend to work in the future. B. References: Two letters of reference from current or former employers or professional colleagues. 6. Copy of License Include a copy of your current occupational therapy state-issued license. If from a state that does not license OT practice, explain this in your letter of intent. 7. Copy of Initial Certification Include a copy of your initial certification issued by the National Board for Certification in Occupational Therapy, Inc. (NBCOT) or its predecessor organization American Occupational Therapy Certification Board (AOTCB). 8. AOTA Membership Upon enrollment, students must submit documentation of current membership in the American Occupational Therapy Association. This is required to access member-only materials used in courses. 9. Resume or Curriculum Vitae Please include a copy of your most recent resume or CV. 10. Application Fee Please submit a non-refundable application fee of $55 (US) with the application materials. Make the check payable to Pacific University. If someone else writes the check, write your name on the check.
