1 YOUTH FOOTBALL & CHEERLEADING PROGRAM February 1, 2017 To Parents/Guardians of Football Players: Thank you for allowing your child to play football. We have met many of you, and are looking forward to meeting new parents and kids. We are extremely excited about helping your children expand their understanding of the great game of football. Our teams consist of ingoing K/1 st, 2 nd, 3 rd, 4 th, 5 th and 6 th grade students. Little Cards is apart of The Greater Miami Youth Football Conference. To satisfy the league requirements, in addition to providing a copy of birth certificate, you will also need to provide proof of completion of grade. A copy of the front page of your child s report card where it states that he/she has been promoted to said grade will be sufficient. This can either be mailed to me or given to me at the concession stand on the first day of practice. The Registration form as well as the Release of Liability for Minor Participant's, Parent Code of Conduct, are included in this packet. It is mandatory for any new football players to be at one fitting. Fittings dates are listed below. There will be a $25 fee for any returned checks. 3 Equipment and Uniform Care I agree to properly care for and maintain all equipment and uniforms provided to me by the Little Cards Youth Football Program, Inc. I fully understand that these items are the property of the Little Cards Youth Football Program, Inc. I agree to return, upon request, the equipment and uniform(s) issued to my child promptly and in good condition, except for normal wear and tear. I fully understand that I may be subject to legal action taken by the Little Cards Youth Football Program, Inc. if any equipment and/or uniform is not returned 30 days from the end of the season. Subsequently, I will not be permitted to register for the next season until the equipment and/or uniform(s) is/are returned along with a $200 equipment deposit for the 2017 season. I understand that I am financially responsible for the replacement cost of said equipment and/or uniform(s) for damage that may occur from the participation in activities that are not sponsored by the Little Cards Youth Football Program, Inc. I also understand that I am financially responsible for the replacement cost of said equipment and/or uniform(s) if said equipment and/or uniform(s) is lost or stolen. No equipment will be distributed until registration fees are paid in full. I/We certify that to the best of my/our knowledge he/she is physically fit to participate in football/cheerleading. I have identified all medical problems the Little Cards Youth Football Program, Inc. should be aware of and have provided special instructions in the Medical Information section. I further certify that I do have insurance coverage for this boy/girl for medical expenses incurred as a result of any injury received from football/cheerleading with the Little Cards Youth Football program, Inc. I understand that a seventy-five percent refund (75%) of the registration fee will be refunded if a child quits prior to receiving equipment. A refund of fifty percent (50%) of the registration fee will be refunded if a child quits after equipment has been issued but prior to the player's eligibility having been approved by the league. Once a player/cheerleader's eligibility has been approved by the league, no refund will be given. A full refund (100%) will be given to any player deemed ineligible by the league and who chooses not to move up to the next grade group. No equipment will be distributed until registration fees are pain in full. Parent/Guardian Obligations The Little Cards Youth Football Program, Inc. is completely organized through the support of volunteers. In order to continue this support, all Parents/Guardians are Required to assist in some capacity during the season. The team mom will be assigning tasks to each family throughout the season. Game Day Activities include: Concession, Grill, Parking Lot, Chain Gang, Split the Pot, Announcer, Field Clean Up (6th Grade only), Field Set Up ( K/1 only), Press Box Spotter, and Time Keeper Photo/Web Site/Video Permission Form To promote activities of the Little Cards Youth Football Program, Inc., often still photos and videos are taken of your child. Some will appear in publications and others may be submitted to community newspapers. Your child's photo and/or name may appear on our website. For safety reasons, we are asking that you sign below authorizing us to include your child in any of the above projects. Your child's safety is of the utmost importance to us. We must have your signature below for your child to be included in the projects above. My son/daughter, has permission to be included in photos, videos or on the website. I understand that photos may be taken to submit for publication or taken by professional photographers for print or broadcast. 4 RELEASE OF LIABILITY FOR MINOR PARTICIPANTS READ BEFORE SIGNING IN CONSIDERATION OF my minor child/ward, being fully (child's name) permitted to participate in any way in the Little Cards Youth Football program, Inc., related events, activities, the undersign acknowledges, appreciates, and agrees that: 1. The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation, and, 3. I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention to the nearest official immediately; and 4. I myself, my spouse, my child and on behalf of my/our heirs assigns, personal representatives and next of kin, HEREBY RELEASE THE other participants, board of directors, organizers, supervisors, coaches, sponsoring agencies, sponsors, advertisers, persons transporting my child to and from activities, officials, and if applicable, owners and lesser of premises used to conduct the event, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 5. I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. 5 Little Cards Youth Football Parent Code of Conduct The essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship and six core principles: trustworthiness, respect, responsibility, fairness, caring, and good citizenship. The highest potential of sports is achieved when competition reflects these "six pillars of character." I therefore agree: 1. I will not force my child to participate in sports. 2. I will remember that children participate to have fun and that the game is for youth, not adults. 3. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety +of others. 4. I will learn the rules of the game and the policies of the league. 5. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other sporting event. 6. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures. 7. I will not encourage any behaviors or practices that would endanger the health and well being of the athletes. 8. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence. 9. I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, color, sex or ability. 10. I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game or his/her performance. 11. I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time. 12. I will never ridicule or yell at my child or other participant for making a mistake or losing a competition. 13. I will emphasize skill development and practices and how they benefit my child over winning. 14. I will promote the emotional and physical well-being of the athletes ahead of any personal desire I may have for my child to win. 15. I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game field, and will take time to speak with coaches at an agreed upon time and place. 16. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events. 17. I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team. 18. The Board reserves the right to remove anyone without warning, if the nature of the offense justifies removal. No refunds will be given. 19.I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: Verbal warning by official, head coach, and/or head of organization Written warning Parental game suspension with written documentation of incident kept on file by organizations involved Parental season suspension Permanent organization suspension (Parent/Guardian Signature )
