1 Grantee: 1199SEIU Training and Upgrading Fund Award: $37,400 Summary of Proposal: 1199SEIU, in partnership with Lynn Community Health Center and Union Hospital, will complete a planning process to develop a training program for frontline workers, which will allow them to gain skills in safely and effectively managing interactions with behavioral patients. Grantee: Anna Jaques Hospital Award: $48,923 Target Population: Nurses. WIB Region: Merrimack Valley Summary of Proposal: Anna Jaques Hospital will create a plan to transition their nurse workforce to higher educational preparedness as a key strategy in their efforts to drive patient errors toward zero. They will assess the current pilot RN to BSN curriculum for how well it exposes BSN candidates to measurable outcomes standards central to healthcare cost containment. They will also explore two options for upgrading the educational preparedness of their workforce: 1) interest of non-nursing employees in pursuing a nursing degree; and 2) potential for partnering with a college or university to offer post-baccalaureate education for bachelor s degree nurses. Grantee: Baystate Medical Center Award: $47,138 Target Population: Clinicians. Summary of Proposal: Baystate Medical Center will develop curriculum and a governance structure for a Nurse Practitioner and Physician Assistant residency in primary and specialty care. This project will also support the development of a program to be used to educate existing staff and physicians on the changing role of advanced practitioners under Chapter 224. Grantee: Berkshire Medical Center Award: $49,802 Target Population: Certified Nursing Assistants. WIB Region: Berkshire Summary of Proposal: Berkshire Medical Center, along with affiliates Fairview Hospital and Berkshire HealthCare, will develop plans for creating a Certified Nursing Assistant (CNA) career ladder training program, aimed at improving CNA retention rates and addressing Berkshire County s health challenges more efficiently. Their planning efforts will focus on: 1) conducting a needs assessment among CNAs, registered nurses and senior management to determine the structure, content and potential impact of the program on hospital operations; 2) researching other CNA career ladder training models; 3) developing curriculum; 4) establishing an application process and training plan; and 5) developing goals and anticipated outcomes. Grantee: Beth Israel Deaconess Medical Center, Boston Award: $49,995 Summary of Proposal: Beth Israel Deaconess Medical Center will develop a more efficient and effective process for care delivery in the hospital. Building on an existing pilot process within one hospital unit, the grant will support continued review and analysis of that pilot. Once a new standard process has been developed, a plan will be created to share this new process with other hospital units and support team members to make sure they have the training and skills to be successful. Grantee: Boston University Center for Aging & Disability Education & Research Award: $50,000 Summary of Proposal: The BU Center for Aging & Disability Education and Research (CADER) will partner with Commonwealth Care Alliance (CCA) to conduct a needs assessment of CCA staff to identify skill gaps in preparation for the implementation of an enhanced model of primary care. This model will provide for integrated, interdisciplinary teams, which can be utilized to achieve cost containment goals and maintain quality care. The results of the needs analysis will be used to develop standardized, consistent training for all CCA staff around integrated/interdisciplinary primary care, to ensure the successful implementation of the model. Signature HealthCare Award: $49,586 WIB Region: Brockton Summary of Proposal: Signature Healthcare is currently transitioning their primary care sites into a patient centered medical home (PCMH) model of care. As clinical and administrative roles are expanded and redefined with the PCMH model, it is critical that all staff possess the necessary knowledge and skill sets. During the planning process, Signature will identify the specific training required to address gaps in knowledge and skills among current primary care staff. Grantee: Cape & Islands Workforce Investment Board Award: $25,762 WIB Region: Cape & Islands Summary of Proposal: The Cape & Islands Workforce Investment Board will engage in a planning and needs assessment process to determine local community health center and hospital workforce training needs related to the implementation of Chapter 224. Grantee: Central Massachusetts AHEC Award: $49,085 Target Population: Community Health Workers. Summary of Proposal: Central MA AHEC, the Boston Public Health Commission, MA Association of Community Health Workers, and the Kennedy Community Health Center have partnered to ensure that the CHW workforce, employers, and health providers are effectively prepared for implementation of CHW certification across the State, including: assessment of CHW curricula to identify needed modifications to meet the CHW Core Competencies, as well as employer-identified training needs; develop, pilot, and evaluate a CHW Skills-Based Core Competency Assessment tool; and provide technical assistance to CHWs to navigate the certification process. Grantee: Central Massachusetts Workforce Investment Board Award: $25,186 Summary of Proposal: The Central MA Workforce Investment Board and its partners from a broad sector of the region's health care industry will establish a Health care Leadership Roundtable (CMHCLR) to 1) identify cross-sector issues, needs and policies; 2) share best practices and information regarding the resources and programming available; 3) explore the concept of an employee retention support system; and 4) identify cross-sector training programs. The CMHCLR is aligned with the goals of MA Chapter 224: health care worker retention, employment of low-income individuals and low-wage workers, and training/education career ladder services for currently employed or unemployed health care workers. Grantee: Clinical and Support Options Award: $49,119 Target Population: Mental health staff. WIB Region: Franklin/Hampshire Summary of Proposal: Clinical Support Options will create a program of education and training that develops general skills for Mental Health and Substance Abuse practitioners/professionals. This program will enable staff to target patients coping with chronic illnesses that put their health and/or social functioning at risk. Trained staff will be able to engage patients, promote their activation to improve their own health, connect them to appropriate services and resources, address questions, and support integrated treatment plans. Grantee: Community Health Programs, Inc. Award: $37,463 WIB Region: Berkshire Summary of Proposal: Community Health Programs will develop a training plan to increase the e-health related skills of front line workers and build the capacity of internal staff to use the e-health system. They will also develop a training plan to increase the skills of Medical Assistants, LPN and RN staff to be able to provide coaching support to patients related to their wellness plans. Grantee: Community Healthlink, Inc. Award: $36,008 Summary of Proposal: Community Healthlink proposes to develop a training plan to integrate primary health care delivery with behavioral health services. They will identify knowledge, skills and competencies required to fill the gap to meet current business challenges, and then develop a plan of action, identifying necessary resources and curricula to address the gaps. 3 Grantee: Cooley Dickinson Health Care Corporation Award: $49,587 Target Population: Care Managers. WIB Region: Franklin/Hampshire Summary of Proposal: Cooley Dickinson Health Care and partners will 1) assess current care managers skill levels and develop a training plan to fill competency gaps; 2) define roles and work processes of care managers in community care management program; 3) assess and create orientation and onboarding curriculum for future care managers; 4) develop ongoing professional development curriculum; and 5) assess Information System requirements to determine a training plan for the care management team. These tasks will ensure comprehensive training and readiness of care managers and care management team members for assured optimal patient outcome and cost savings. Grantee: Dorchester House Multi-Service Center Award: $27,660 Summary of Proposal: Dorchester House will conduct an internal assessment of staff and create a Training Center to address ongoing skill development needs among staff. The Center will be developed in partnership with MA League of Community Health Centers and Boston University School of Medicine. Planning grant funds will be used to assess resources needed to start up this Training Center. Grantee: Fellowship Health Resources, Inc. Award: $40,225 Target Population: Frontline clinical staff. Finance staff. WIB Region: Greater New Bedford Summary of Proposal: Fellowship Health Resources (FHR) is aware of the changes needed to meet Chapter 224 requirements and acknowledges that a change in attitude toward services delivery, as well as managing costs of delivery, will be necessary. FHR will develop a training curriculum for its direct care staff on how to provide preventative education and interventions for primary health care issues with the Serious and Persistently Mentally Ill population. They will also develop a training curriculum for their business staff in alternative healthcare billing options. Grantee: Gandara Center, Inc. Award: $49,564 Target population: Frontline clinical staff. Summary of Proposal: Gandara Center will work to develop credit-based curricula at Holyoke Community College and Urban College for direct care, children's behavioral health workers. They will develop clinician training to focus on the integration of behavioral and medical health services. Grantee: Greater New Bedford Workforce Investment Board Award: $30,348 WIB Region: Greater New Bedford Summary of Proposal: The Greater New Bedford Workforce Investment Board, in collaboration with the Bristol Workforce Investment Board, will engage with local healthcare employers to determine current training needs related to their implementation of Chapter 224. This work will inform the activity of the South Coast Healthcare Collaborative and support the development of a focused training plan. Grantee: Greenfield Community College Award: $27,981 Target population: Frontline clinical staff. WIB Region: Franklin/Hampshire Summary of Proposal: Greenfield Community College proposes the creation of new and enhanced credit and credit-free curricular pathways for Home Health Aides, Certified Medical Assistants, Personal Care Assistants, and Certified Nursing Assistants, in order to ensure the curricula address the needs to area employers. Grantee: Hallmark Health Award: $47,839 Target Population: Pharmacy Technicians. Summary of Proposal: Hallmark Health System (HHS) will develop a Pharmacy Technician Certificate that would allow them to train current employees, and create a pipeline for future hiring. As HHS has a School of Nursing and two acute care hospitals, they are in a unique position to simultaneously teach and provide needed clinical experience to build a broader base of qualified Technicians, while providing unskilled employees with the opportunity to build a better career. 4 Grantee: HealthFirst Family Care Center, Inc. Award: $21,500 Target Population: Community Health Workers. WIB Region: Bristol Summary of Proposal: HealthFirst is a member of the The South Coast Community Health Workers Collaborative, a coalition of thirty organizations in southeastern Massachusetts convened by United Interfaith Action. Led by Healthfirst, they intend to establish a local Training Collaborative that will provide core competency and disease-specific training, as well as continuing education and networking opportunities in the South Coast region essential to building, retaining and strengthening the professional development and professional identity of a highly trained and highly qualified CHW workforce. Grantee: Holyoke Community College Award: $24,255 Target Population: Certified Nursing Assistants. Frontline clinical staff. Summary of Proposal: Holyoke Community College (HCC) will work in partnership with the Soldiers Home and Loomis Communities to assess the efficacy and relevance of HCC s current credit-based curriculum for training direct care workers and Certified Nurses Aides (CNAs). In addition, HCC will assess workforce job satisfaction, job retention, and job expectation/job actuality at both Loomis Communities and the Soldiers Home in relation to the relevance, quantity, and quality of HCC s existing for-credit CNA training program. Lastly, HCC will explore development of clearer career ladders for both incumbent and new CNAs. Grantee: Home Care Aide Council Award: $44,569 Target Population: Home Care Aides. Summary of Proposal: Home Care Aide Council will research and create an enhanced Mental Health Supportive Home Care curriculum, allowing for the future training of advanced home health aides who serve the personal care, and difficult and complex emotional and behavioral needs of patients with mental illness. Grantee: Jewish Vocational Service Award: $41,933 Nurses. Summary of Proposal: Jewish Vocational Services (JVS), in partnership with Hebrew Senior Life, will assess the training needs for leadership and process improvement among Certified Nursing Assistants and Registered Nurses. In addition, JVS, in partnership with Brigham and Women's Hospital and Children's Hospital Boston, will assess the need among administrative support workers for skills that improve workers' ability to increase patient engagement. For both projects, assessment findings will be compiled in a report that will inform curriculum development and a plan for delivering training related solutions. Grantee: Justice Resource Institute Award: $28,094 WIB Region: Metro South/West Summary of Proposal: Justice Resource Institute will develop and pilot trauma training to existing staff to ensure they are better able to address trauma in their client population. Staff training will lead to an improved and efficient delivery of services to clients. This will help prevent clients from unnecessarily seeking emergency room services, and support a continued focus on the provision of appropriate levels of care in the community. Grantee: Lahey Clinic Award: $49,967 Target Population: Medical Assistants. Summary of Proposal: Lahey Hospital & Medical Center s (LH&MC) will conduct a multi-pronged analysis to evaluate the skills of their primary care workforce, vis-à-vis the core competencies needed to deliver a Patient-Centered Medical Home (PCMH) model of care. This analysis will inform the development of an in-house training curriculum for LH&MC s Medical Assistants. This project will investigate the opportunity to expand the role of Medical Assistants while reducing reliance on more expensive providers, and provide a career ladder for low-wage Medical Assistants by advancing them in developing PCMH roles. LH&MC will partner with Middlesex Community College, leveraging their expertise in curriculum development. Commonwealth Corporation Page 4

5 Grantee: Lowell Community Health Center Award: $26,863 Target Population: Medical Assistants. Community Health Workers. WIB Region: Greater Lowell Summary of Proposal: Lowell Community Health Center will develop a health care workforce development program in the Northeast region targeted at Medical Assistants and Community Health Workers. They will also partner with local colleges to explore opportunities to provide accredited courses where possible. Grantee: Mass Coalition for the Prevention of Medical Errors Award: $48,327 Summary of Proposal: The Massachusetts Coalition for the Prevention of Medical Errors will engage physician practice organizations to plan the implementation of training for clinical and administrative staff in primary care practices. Training will support them in using continuous improvement strategies to reduce costs and improve the quality and safety of care consistent with the goals of Ch 224. The MA Coalition will work with Hallmark Health Medical Associates to develop an implementation plan and manual that would meet the needs of its practices, and will recruit additional office practice organizations to implement the training plan in their practices. Grantee: Massachusetts Hospital Association Award: $49,977 Summary of Proposal: Massachusetts Hospital Association will conduct an assessment their members' needs and research educational programs to address those needs. The focus will be on identifying education and training needs that exist as a result of the shift in healthcare delivery with a particular focus on care management, population health management, and care transitions. Grantee: Merrimack Valley Workforce Investment Board Award: $23,056 Target Population: Certified Nursing Assistants. WIB Region: Merrimack Valley Summary of Proposal: Merrimack Valley WIB proposes to enhance the training of Certified Nursing Assistant in the region by focusing on team work, cultural sensitivity, communication, and specialized content in Dementia, Rehabilitation/Restorative, and End of Life Care. Grantee: Metro North Regional Employment Board Award: $32,500 Target Population: Frontline clinical staff. Summary of Proposal: Metro North Regional Employment Board will work to identify the roles of frontline healthcare workers, assess skill gaps, and identify needs for training for healthcare workers in emerging fields of the Accountable Care Organization and Patient Centered Medical Home care delivery. The focus will be on teamwork and coordination among healthcare workers, as well as engaging patients to manage their own health and chronic conditions. Grantee: Metro South/West Employment & Training Administration Award: $35,500 Target Population: Frontline clinical staff. Frontline non clinical staff. WIB Region: Metro South/West Summary of Proposal: Metro South/West ETA is planning to provide local healthcare facilities with the ability to meet their mission of providing quality healthcare by building the skills and careers of their employees, as well as providing a pathway for adults to enter the field of healthcare. They will partner with employers to develop a strategy for advancement of lower wage employees through new educational initiatives leading to credentials, including a career ladder that would advance lower level direct service workers and clerical employees. Grantee: North Shore Community Action, Inc. Award: $16,645 WIB Region: North Shore Summary of Proposal: North Shore Community Action Program, North Shore Community Health (NSCHI) and Salem State University will conduct a workplace needs analysis and develop a curriculum scope and sequence focused on assessing and enhancing the communication skills of entry-level and front-line employees at NSCHI. The purpose is to address workforce challenges occurring as a result of Chapter 224 implementation and, specifically, to determine opportunities for increased communicative competency, efficiency and enhanced patient engagement within the shift to the patient-centered medical home model. The needs analysis will primarily focus on front-line and entry-level staff, more than 50% of which are non-native English speakers. Commonwealth Corporation Page 5

6 Grantee: North Shore Workforce Investment Board Award: $48,500 Target Population: Medical Assistants. WIB Region: North Shore Summary of Proposal: The Patient Centered Medical Home (PCMH) model has been observed nationally as a cost saving/service enhancing model, involving the patient and family in partnership with the entire team of primary care professionals in the care of the patient. Primary care staff, with particular attention to the Medical Assistant as the hub of this team, requires extensive training with regard to these new systems. North Shore WIB will survey employers to understand current skill levels existing within physician practices in relation to PCMH standards, and develop curriculum for Medical Assistants and others that allows for the implementation of the PCMH across all employer partners. Grantee: Organization of Nurse Leaders, Association for Nursing Leadership, Science, and Education, Inc. Target Population: Nurses. Award: $49,500 Summary of Proposal: The Organization of Nurse Leaders, in partnership with Winchester Hospital and other key academic, community, and healthcare organizations, will work towards planning a statewide Massachusetts Nurse Residency Program (MNRP). This initiative will develop a needed state-wide framework for nurse residency for those newly licensed nurses who are currently unemployed or underemployed. New nurses, in transition from education to practice, will receive experience in acute, home and/or rehabilitation care settings. They will maintain skills, gaining valuable experience that will better prepare them for employment in our evolving healthcare delivery system while also improving nursing retention rates. Grantee: Partners Healthcare System Award: $31,085 Target Population: Frontline non-clinical staff. Summary of Proposal: Partners HealthCare will work to identify the necessary competencies for staff that works in care delivery settings where Patient Centered Medical Home (PCMH) is being implemented. Partners HealthCare will partner with College for America to develop an online health care certificate to enable non-clinical staff to acquire and apply skills needed to support the PCMH model. They will develop a plan to create and pilot the certificate in several PCMH practices across Partners HealthCare, including licensed community health centers and community-based primary care practices. Grantee: Partners Home Care Inc. d/b/a Partners HealthCare at Home Award: $27,366 Target Population: Clinicians. Summary of Proposal: Partners HealthCare at Home will develop a plan to provide enhanced training in palliative care to clinicians across the Partners Continuing Care Network, with guidance from the Partners Palliative Care Steering Committee. Grantee: Partnerships for a Skilled Workforce, Inc. Award: $32,883 Target Population: Medical Assistants. WIB Region: Metro South/West Summary of Proposal: Partnerships for a Skilled Workforce proposes to create a Senior Nursing-Medical Assistant role, who will enhance functions currently performed by nurses. Health care providers will reduce personnel costs and enhance patient care by upgrading the knowledge and skills of the workers closest to the patient and improving communication between nurses and the assistants on their teams. Building upon Hebrew SeniorLife s CNAs: Creating Frontline Leaders, a plan will be developed to expand to other health care settings, move instruction from health care business as trainer to community college faculty, and develop a return on investment model. Grantee: Quinsigamond Community College Award: $27,689 Summary of Proposal: Quinsigamond Community College seeks to create a bilingual healthcare career pathway that opens up opportunities for individuals whose employment options are currently limited, while also improving the access and quality of healthcare services for non-native English speakers living and working in Central Massachusetts. 7 Grantee: Regional Employment Board of Hampden County Award: $25,121 Target Population: Clinical Lab Staff. Summary of Proposal: Led by the REB of Hampden County, the Western Massachusetts Clinical Laboratory Workforce Analysis will engage employers, educators and workforce development agencies in the Pioneer Valley and Berkshires to address gaps between supply and demand for medical lab technologists, medical lab technicians and clinical lab assistants. The planning process will result in strategies that address an aging workforce, the lack of clinical placements for current students, and lack of updated staffing models to determine the most cost-effective ratio of lab professionals needed. Grantee: Riverside Community Care Award: $49,988 WIB Region: Metro South/West Summary of Proposal: Riverside Community Care is community-based behavioral health care and human service organization, committed to ensuring workforce and data systems are ready for effective collaboration with ACOs and Patient Centered Medical Homes. I order to support implementation of an integrated electronic health record system, they plan to: 1) determine what training their workforce needs to be able to effectively collect, analyze, and utilize health record and billing systems data for reporting, internal management, quality improvement and interface with HIEs; and 2) determine what training mechanisms will best enable them to meet their identified workforce training needs. Grantee: Roxbury Community College Award: $7,000 Target Population: Patient Care Coordinators. Summary of Proposal: Roxbury Community College will use the Healthcare Workforce Transformation Fund planning grant to design a Patient Care Coordinator curriculum. This new curriculum will be designed as a career pathway that can be accessed by high school students, Roxbury Community College students, and incumbent workers interested in retraining. Employer advisory board partners include Brigham & Women s Hospital and Massachusetts General Hospital. The curriculum will be designed to grant college certificate and degree pathways. Grantee: Royal Health Group, LLC Award: $46,050 WIB Region: South Shore Summary of Proposal: Royal Health Group seeks to address high turnover of Certified Nursing Assistants and other clinical and non-clinical staff by delivering periodic training, and creating a new Staff Development position to provide focused orientation and support to new hires. Grantee: South Shore Hospital Award: $19,449 Target Population: Nurses. WIB Region: South Shore Summary of Proposal: South Shore Hospital will create an innovative critical care training program for the 2-5 year medical/surgical Registered Nurse. Training will include the role of the bedside nurse in the implementation of Ch 224 by providing quality care that will prevent readmissions, delayed discharges, and lengthy hospital stays. Group training will reduce education costs, RN turnover, provide career ladder, and reduce payroll expense while providing quality care. Grantee: Southcoast Physicians Group, Inc. of Southcoast Health System Award: $49,704 Target Population: Medical Assistants. WIB Region: Greater New Bedford Summary of Proposal: Southcoast Physicians Group will identify a core framework of skills needed by Medical Assistant staff to address health management and prevention. They plan to develop a model for training that will be transferable to other positions. Grantee: Spaulding Rehabilitation Hospital Corporation Award: $11,660 Summary of Proposal: Spaulding Rehabilitation Network and Partners HealthCare at Home will collaborate on a project to improve career development services for current and prospective staff to improve retention at their affiliates and in the health care field. The affiliates involved in the program will conduct a needs assessment of staff at all levels. The needs assessment will help determine what the gaps are in career development services for staff, and how they can do a better job of helping staff see a long term future in health care. 8 Grantee: UMass Memorial Medical Center Award: $50,000 Summary of Proposal: In response to business changes, UMass Memorial has established these goals: 1) to improve patient access; 2) to improve patient flow; 3) to improve patient and employee experience; and 4) cost containment and financial recovery. To achieve these goals, UMass Memorial needs to objectively measure employee skills, identify areas for improvement, and provide access to learning resources to close skill gaps. UMass Memorial will implement an initial skills identification and assessment project to be used as a model for an organization-wide initiative. Grantee: VNA and Hospice of Cooley Dickinson Award: $50,000 Target Population: Occupational Therapists. WIB Region: Franklin/Hampshire Summary of Proposal: VNA & Hospice will develop a paid fellowship and preceptor program for new Occupational Therapists based on the existing VNA-RN fellowship model. Grantee: VNA Care Network Foundation Award: $16,560 Target population: Physical Therapists. Summary of Proposal: VNA Care Network will develop a residency program for newly graduated Physical Therapists that will attract and engage them in home care practice, and help them transition into home care clinical settings. Grantee: Western MA Public Health Training Center, Amherst Award: $49,586 Summary of Proposal: Western MA Public Health Training will collaborate with Caring Health Center, a community health center in Springfield MA, to do an assessment of organization and staff needs, and develop a training strategy to provide training on goal setting and cultural competency that will help improve patient engagement and selfmanagement. Grantee: YMCA Training, Inc. Award: $19,502 Target Population: Frontline non-clinical staff. Summary of Proposal: YMCA Training Inc. will develop a healthcare worker pipeline to address the staffing needs of managed care organizations. They will change and enhance an existing Training Inc.
