3 Course Objectives JOINT PROVIDERSHIP This live activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Washington University, Continuing Medical Education and BroadWater, LLC. is accredited by the ACCME to provide continuing medical education for physicians. CME ACCREDITATION designates this live activity for a maximum of 14 AMA PRA Category 1 Credits for the main program and 6.75 AMA PRA Category 1 Credits for the optional lab. Physicians should claim only the credit commensurate with the extent of their participation in the activity. At the conclusion of this activity, participants should be better able to: Analyze, discuss, and recommend optimal surgical strategies for treating common spine conditions, including adult and pediatric deformity, cervical and lumbar degenerative, and thoracolumbar tumors. Explain approaches for avoiding and treating complications in complex spine surgery. Demonstrate familiarity with pedicle subtraction osteotomies and MIS lateral techniques in the treatment of spinal deformity and spinal degeneration. Registration Fees Surgeons $575 Residents/Fellows/Allied Health $425 Late Registration Fee $50 (after January 15) Optional Lab Registration Fee $595 Registration includes breakfasts, lunches, course reception, syllabus materials, and scientific sessions on Thursday and Friday. Lab registration includes breakfast, lunch, and cadaveric instruction on Saturday. Main course registration is required in order to register for the cadaveric lab. Registration Procedure Register early as the limited space will fill rapidly, especially for the hands-on lab. Surgeons who register after the course is full will be placed on a wait list and contacted if space becomes available. Refund Policy A full refund will be provided, less a $100 administrative fee, dependent upon written notice being received by January 15, Refunds will not be made after this date. Substitutions are allowed at any time but must be submitted in writing. Cancellation Policy: In the unlikely event of insufficient enrollment, we reserve the right to cancel the course up to 21 days prior to the course. Registrants will be notified by certified mail and registration fees will be refunded. Airline and other costs incurred by the registrant are the responsibility of the registrant and will not be refunded. Attendance: The course chairmen reserve the right to refuse admission to anyone whose presence is considered to be incompatible with the course objectives.
