1 Department of Health Professions Application to AAS-PN Program Thank you for your interest in the Health Professions Programs at - UM. As you prepare your application for submission, there are a few items to consider. This application is for the Practical Nursing Program. Students can apply in the semester they are finishing their prerequisites. If you are not already enrolled at - UM, an application for admission to MC must be completed as well. Please follow the application directions carefully. In order to be eligible to apply to the Practical Nursing Program at - UM the following must be demonstrated: General Associate of Arts (AA) Program Requirements Students may apply in the semester in which they are finishing their requirements. Determination of program acceptance will be made after grades are submitted at the end of the semester. BIOH 201 & BIOH 202N Anatomy and Physiology I 4 BIOH 211 & BIOH 212N Anatomy and Physiology II For students who 4 completed the one semester (6 or 7 credit) Anatomy & Physiology course, you must complete BIOH 211 & 211N M 121 OR M121 College Algebra or 3 M115 Linear & Probability or M 152 Pre-Calculus with Algebra or M 153 Pre-Calculus with Trig or M 171 Calculus WRIT 101 English Composition 3 PSYX 100S Introduction to Psychology 4 CHMY 121 Introduction to General Chemistry 3 CHMY 122 Introduction to General Chemistry Lab 1 NUTR 221N Nutrition 3 NRSG 100 Introduction to Nursing 1 Completion of BIOH 201 & BIOH 202 (A&P I) and BIOH 211 & BIOH 212 (A&P II), with a minimum of a B grade within 2 attempts. 2 All other pre-nursing courses completed as outlined in catalogue with a grade of C or better. Maximum of two attempts per course. Most recent grade earned will be used to calculate GPA. Students in their last semester of General Associate of Arts (AA) prerequisites for the nursing program can apply for acceptance into the Associate of Applied Science- Practical Nursing (AAS-PN) level. However, all prerequisites must be successfully completed prior to entering into the nursing program. Accepted students will enter into the Nursing Program and begin with Practical Nursing (AAS-PN) Scope and Sequence. Your application will be evaluated for minimum eligibility and then ranked in two steps. Each step and category is carefully considered and valuable in the application process. The two steps of the application process: 1. Submitted application materials 2. Interview (of top 30 ranked applicants) Minimum eligibility requirements: Overall cumulative GPA of at least 2.75 or better. B or better in AP I and AP II in maximum of two attempts. Current Certified Nursing Assistant (CNA) Certificate. The CNA course must be completed by the end of the semester in which you are applying. Step 1 Ranked requirements: Cumulative GPA in the prerequisite courses (50% of application score). Completion of a proctored Test of Essential Academic Skills (TEAS) test (25% of application score). The TEAS test will be offered: i. Monday, November 9, 2015; 11:00 a.m. to 3:00 p.m. AD 16 ii. Tuesday, November 10, 2015; 2:00 p.m. to 6:00 p.m. AD 15 iii. Thursday, November 12, 2015; 2:00 p.m. to 6:00 p.m. AD 15 iv. Friday, November 13, 2015; 8:00 a.m. to 12:00 p.m. AD 14 Please call the Administrative Associate at to schedule your day for your TEAS test after you have turned in your application. You are allowed 4 hours to take the TEAS test. Please create an account with ATI before your test date at ATItesting.com. You will need to bring some type of electronic payment to purchase the test online. The cost is $ Step 2: The top thirty ranked applicants will be invited to an interview with a selection committee. Date, time and place to be scheduled (25% of application score). Rev. 09/10/15 2

3 The final application score is a cumulative score of the TEAS, GPA, and interview. Selection Process 1. Step I: Applications will be read and evaluated. All applicants will be notified regarding their selection status as soon as possible after final grades for the semester have been posted. This process takes approximately 1-2 weeks. Please do not call or . Notification letters to all applicants will be sent out. 2. Step II: The top 30 ranked candidates will receive an invitation to interview. Selected candidates are required to participate in an interview for consideration. The Nursing Program Selection Committee will then make final selections for acceptance into the Practical Nursing Program. The decision of the Selection Committee is final. The Selection Committee reserves the right to decline any nursing program applicant based on the application packet and interview process. 3. AAS accepted students will be notified by letter as soon as possible. Please note: Successful completion of the AAS Nursing Program does not guarantee a Montana State nursing license. Nurses cannot practice in the profession of nursing until they have successfully passed a National Certification Exam. Acceptance for taking National Certification Exams is approved or denied by the Montana State Board of Nursing on an individual basis. If you have a criminal conviction, you may be denied. 4 Application Steps Review the application requirements. Consult a nursing advisor or program director if you have questions. Compile and submit the requested materials listed below. PRACTICAL NURSING PROGRAM A letter stating which program you are applying to with your current address and phone number. Completed Nursing Program Application form (application to must be done online or at the Admissions Office prior to application to the nursing program). Transcripts, evaluation of transfer courses and/or waivers: Unofficial transcripts from all colleges and universities attended. Unofficial transcript of courses currently in progress. Completed Transcript Release Form for grade currently in progress at time of application; This form can be accessed on the web. 1. Go to The University of Montana webpage at: 2. At the top of the page under the word Transcripts select Request for Transcript and the form will appear. 5. Print the form, complete it and enclose it in the envelope with your application. (You do not need to pay the fee.) If you are unable to access the form online, you may obtain one from the Registrar s office. Course Waiver, Transfer and Substitution completed, if applicable. Please submit a copy of the form. You may contact the Registrar s office for this form. Certified Nursing Aide Certificate (CNA) Include a copy of current certification in the application packet or proof of enrollment in CNA course and certification by finals weeks of the semester in which you are applying. TEAS test score (from proctored exam on campus) after application is submitted. Date choices are listed above. Submit the completed application by Monday, November 2, 2015 by 12 noon. The application must be in a sealed 9 by 12 envelope with your name, address, phone, , and the program you are applying to on the outside of the envelope. 5 Fall 2015 Application for Nursing: Missoula This application is for students applying to the Practical Nursing Program starting January You must have completed, or will complete by the end of Fall semester 2015, all pre nursing courses. CNA certificate must be turned in by finals week, December 18.
