1 DODGE COUNTY TITLE: HEALTH EDUCATOR BAND C GRADE 4 SUBGRADE 1 DIVISION: ACCOUNTABLE TO: CLASS CODE: Public Health Director of Public Health PRIMARY LOCATION: Public Health Office NORMAL HOURS: 8:00AM to 4:30PM Evening/Weekend hours as needed DATE OF LATEST REVISION: 8/1/2012 JOB PURPOSE: Under general supervision, the Health Educator is responsible for conducting assessments, analysis and evaluation of community health educational needs. The Health Educator is responsible for planning and implementing health educational programs, delivering health education to County residents, planning and implementing outreach activities for Child and Teen Checkup, and planning and implementing statewide health improvement program. Position responsibilities include the promotion of health programming and the preparation of grants or the seeking of supplemental revenue to assist in the support of and development of educational health programming and services. Duty Essential Duties Frequency B/G No. 1. Prepares, promotes and delivers health educational programming at community events, worksites, schools, senior dining sites or other events requested by Dodge residents. 20% 2. Conducts community health need assessments for various target populations for health promotion through the research, collection and analysis of survey data Researches health topics and available resources for Dodge residents or PHN staff. Determines how best to communicate and disseminate health information to the public. 4. Researches grant or funding opportunities and writes grants that can support and enhance the objectives and goals of Dodge County Public Health. Daily 10% Quarterly 1

2 Duty Essential Duties Frequency B/G No. 5. Formulates and develops advisory groups for different target Population groups to gain feedback, support, or ideas concerning health programming, services and health education Collaborates and serves as the Adolescent Health Coordinator/Grant Manager to better coordinate the delivery of service and better utilize existing resources. 7. Prepares progress and activity reports for grant programs outlining th objectives, activities and accomplishments of the program. 8. Develops/implements the C& TC Outreach Program. 9. Performs duties of the lead County Resource Management Team (CRMT) 10. Performs WIC duties as needed. (For this duty a degree in Dietetics is required.) 11. Performs case management for community and nursing home residents for certain health plans. Daily Quarterly Daily Performs other duties of a comparable level or type. MINIMUM QUALIFICATIONS: Requires minimum of Bachelor s Degree (BA/BS) in community health education, public health, nursing or related area. Will consider a 2 year RN, or a combination of 2 year degree in related field and experience. Prefer Masters Degree in Public Health and certification as a Certified Health Education Specialist. Valid Driver s License. KNOWLEDGE/SKILLS/ABILITIES REQUIRED: (At entry level) Knowledge of: Principles, practices, issues, trends and techniques in the field of community health education and public health. 2

3 Fundamentals, methods and approaches used in developing and implementing health promotion programs. Research methods and techniques used in assessing, evaluating and documenting the performance of health needs, promotional and educational efforts and programs. Community issues, organizations and funding sources available for health education. (At entry level) Skill in: Identifying, formulating, and writing grants for alternative funding. Designing, developing and implementing community based health education programming and services. KNOWLEDGE/SKILLS/ABILITIES REQUIRED (CONTINUED): Planning, promoting and collaborating with citizen groups, advisory groups, citizens and other county agencies in designing health programs and services to better meet the needs of citizens. Researching health prevention and health related topics. Communicating and presenting health information in a clear, concise and effective manner in both writing and presentation. Writing grant applications and funding reports and program evaluations. Public relation activities designed to promote, communicate and foster interest in health issues and topics relevant to Dodge County. Using office equipment, computers and software applications. Perform tasks in a safe and efficient manner while using appropriate safety equipment, clothing, and devices. PHYSICAL ACTIVITIES/REQUIREMENTS: Light physical work. Duties involve exerting force up to 25 pounds to 50 pounds occasionally. Physical requirements and essential duties involve walking, standing, seeing, sitting, talking, hearing, and using hands to handle, feel or manipulate objects.
