Sealed quotations are invited by Bihar State AIDS Control Society from specialized and experienced agency/ Company for providing below mentioned basic services to Bihar State AIDS Control Society. The Contract Period is effective from date of awarding the Contract to extendable on same terms and condition on monthly/yearly basis by another month or year on the basis of satisfactory performance. Sealed quotations are invited for onsite Annual Maintenance Contract (AMC) of Computers, UPS, Printers & other peripherals and Trouble Shooting of Local Area Network (LAN) in Bihar State AIDS Control Society, Patna, for the period effective from the date of award of the Contract to extendable on same terms and condition on monthly/yearly basis by another month or year on the basis of satisfactory performance. The agency putting up the quotation should have minimum 2 (two) years of experiences in rendering similar type of services. S.N. Brief Description of the work Expected Quantity 1. Onsite Annual Maintenance Computer- 38 Contract (AMC) of Computers, UPS- 38 Laptops, UPS, Printers & Printer-30 peripherals and Trouble Shooting of Local Area Network (LAN) for the period effective from the date of award of the work to EMD Amount Installed at 10, Bihar State AIDS Control Society, Sheikhpura. Patna Note: - The quantities of items indicated may be increase or decrease without any change in the unit price or any other terms and conditions. 1. Bid price a) The contract shall be for the full quantity as described above. Corrections, if any, shall be made by crossing out, initialing, dating and rewriting. b) All duties, taxes and other levies payable by the contractor under the contract shall be included in the total price. c) Sales Tax/Service Tax/VAT and should be clearly mentioned separately in terms of percentage and amount in the price sheet. d) The rates quoted by the bidder shall be fixed for the duration of the contract and shall not be subject to adjustment on any account. e) The prices shall be quoted in Indian Rupees only. f) Each bidder shall submit only one quotation. g)financial Bid must be submitted on the letterhead of the bidder in the format provided with this document (Annexure-I). Those bid/s which are not submitted on the letter head will not considered for financial evaluation.

3 2. Eligibility Criteria 1. The agency putting up the quotation should have minimum 2 (two) years of experiences in rendering similar type of services. 2. The agency putting up the quotation should having its registered Office/ Shops within the Patna Municipal area. 3. Scope of Work: 3.1. Annual Maintenance Contract (AMC) of Computers, Laptops, UPS, Printers & peripherals: Upkeep and maintenance of the hardware and installed software Repair to be carried out at the location of the equipments The software maintenance includes loading/reformatting of operating system/ application software with software like Windows 7 (Professional/Home), Windows XP (Professional/Home), Windows 2000, UNIX, Linux, Microsoft Office software, Internet Explorer, Netscape, Outlook Express, Anti-virus software, Data retrieval from corrupt Hard Disks whenever necessary and installation of new/used PCs, removal of any other software purchased by Society from time to time. It also includes removal of virus and reinstallation of software, if corrupted. Support for users and troubleshooting of commercial software packages mentioned above The Annual Maintenance Contract (AMC) shall cover maintenance of computer/laptop etc and reloading of licensed operating systems, application software, data retrieval, data back-up, virus cleaning and repair of hardware, if necessary of all parts and accessories installed in the CPU, Monitor and Printer, including Motherboard, Processor, PCB, SMPS, HDD, FDD, Optical Disk Drive, RAM, Teflon, data card, printer head, Roller, power strips, connection cords etc. The all parts and software must be repaired by the firm/ agency within the cost of AMC. If the part/s is/are beyond the repairable conditions, the part/s will be provided by the Society To attend any problem related to Computer, Laptops, UPS, Printers and Peripherals Under the provision of contract you have to provide weekly routine visit (at least two days in a week) and breakdown visit as and when required during the working hours (10.00 AM to 6.00 AM) from Monday to Friday in the Society for day to day complaints and breakdown. Breakdown of the machine must be attended by your service engineer within 24 hours after receiving the complaint from Officers/ Employees of the Society. In the event of service required after above hours or on holidays, the agency should be prepared to provide services with no extra charges. 3.2 The following would be the scope of work with regard to trouble-shooting of Local Area Network: Maintenance on comprehensive basis of PC s and PC peripherals including trouble shooting The agency has to ensure uninterrupted connectivity of LAN in all components till end users. It includes installation of Ethernet card drivers, monitoring of optical fiber/ UTP segment from time to time to check proper connectivity, crimping of connectors, configuring and assigning of IP addresses of all nodes. The agency will also under take cable laying activities as and when required by different components to extend network connectivity. The agency will further ensure that all users are directly connected with the network and are able to send or receive mails, browse the internet and are able to share files through networking Computer and Network virus Removal To check the port readiness of network switches and Hubs with the help of LAN Tester To check the connectivity of the cable with the help of LAN Tester To re-crimp a LAN node for any PC as and when required and able to identify LAN and internet faults To replace and re-install any corrupted networking system files. The all parts must be repaired by the firm/ agency within the cost of AMC. If the part/s is/are beyond the repairable conditions, the part/s will be provided by the Society Setting Network configuration across different version of Windows OS like Window 98/XP/2000/2000 Server etc Trouble shooting network with respect to accessing network Laser Printer or any other sharable resources.

4 3.2.9 To attend any problem relating to network under the provision of contract you have to provide weekly routine visit (at least two days in a week) and breakdown visit as and when required during the working hours (10.00 AM to 6.00 AM) from Monday to Friday in the Society for day to day complaints and breakdown. If the Society procures any additional Hardware, the selected bidder shall support configuration and integration of the same, according to the requirement 5. Documents required to be submitted along with the bid The bidder shall furnish the following documents along with the bid: a) Copy of registration certificate of the agency. b) Copy of the Income Tax return for last two years. c) Copy of PAN card. d) Document in support of the past performance. e) Copy of the VAT/ Service Tax Registration Certificate. f) Copy of profit and loss account and balance sheet (duly attested by the auditor) for last two years. g) The Bid should accompany a DD of Rs. 10,000/- (Rupees ten thousand only) drawn on any Nationalized Bank in the Name of Project Director, Bihar State AIDS Control Society, Patna payable at Patna towards Earnest Money Deposit (EMD). The EMD of the successful bidder will then be treated as security deposit and would kept till satisfactory completion of jobs stipulated, failing which it would be forfeited. EMD of unsuccessful bidder will be returned after reasonable time. h) Bidder shall submit an affidavit from Notary that the company has not been blacklisted/ de-registered/ barred by the Central/ State Government/ Society. 6. Validity of Quotation Quotation shall remain valid for a period of not less than 90 days after the deadline date specified for submission. 7. Evaluation of Quotations The purchaser shall evaluate and compare the quotations determined to be technically responsive i.e. these a. are properly signed; and b. conform to the terms and conditions and specifications. 8. Award of Contract a) The purchaser will award the contract to the bidder whose quotation has been determined to be substantially responsive and who has offered the lowest evaluated quotation price. b) The purchaser reserves the right at the time of award of contract to increase or decrease the quantities of items indicated by 20% without any change in the unit price or any other terms and conditions. c) Notwithstanding the above, the purchaser reserves the rights to accept or reject any quotation or all quotations at any time prior to the award of contract. d) The bidder whose bid is accepted will be notified of the award of the contract by the purchaser prior to the expiration of the quotation validity period. The terms of the accepted offer shall be incorporated in the purchase order. 9. Penalty a) For delayed in breakdown call - 1/2 % of the value of the contract per week or part thereof after expiry of the delivery period subject to maximum 10%. If the agency fails to attend or rectify any complaint of LAN as well as equipment/s, the Society may forfeit the EMD, blacklist the agency and the required work will be done from the other sources at the contractor s risk and cost.

5 10. Payment- No advance payment will be made. Payment will be released on basis of call attended by the agency's service engineer and generate the call report. The call report must be submitted for release of payment on quarterly basis or after the completion of the Contract. 11. Submission of Bid: The last date and time for submission of the completed bid is hrs on April, Tender Opening The bid will be opened on the last day of submission of bid (i.e. on April, ) at hrs in presence of the bidders or their authorized representatives who choose to attend. In the event of the date specified for bid receipt and its opening being declared as a holiday for BSACS, the due date for submission of Bids and opening of Bids will be the following working day at the appointed time.
