Seniors, juniors, and sophomores who: Have participated in ski club for AT LEAST 1 year, and Can safely and comfortably ski black diamond terrain at Seven Springs.

2 INFORMATION PACKET: Dates and Deadlines Friday, 10/21/16 Registration begins Your $ non-refundable deposit to Mrs. Susa secures your spot! - We have room for 40 students. Spaces will be filled on a first-come, first-served basis. When spaces are filled, we will begin a waiting list. - Disciplinary or academic records may be reviewed with an administrator, and can prohibit students from participating in the trip. Friday, 12/16/16 DUE: Final payment of $330 due to Mrs. Susa Parent Permission Form, Medical Information Form, and Security Form Thursday, 02/16/17 7:00pm Luggage checks, load bus (USCHS Theater Entrance) 8:00pm Depart USCHS for Lake Placid! (times subject to change) Monday, 02/20/17 ~ 2:00 AM Arrive at USCHS (time subject to change) Make checks payable to Upper St. Clair High School. Return all forms and payments to Mrs. Susa in Room 122 or the English planning room. Missing deadlines may be cause for removal from the trip without a refund. Any student found in the presence of, possessing, or using alcohol or drugs will face discipline by both the New York police and high school administration. If a student is found using or possessing drugs or alcohol, the student will be prohibited from participating in trip activities and parents will be responsible for providing immediate, alternate transportation home from New York.

3 INFORMATION PACKET: Tentative Itinerary Thursday, 02/16/17 7:00 pm: Arrive at high school theater parking lot for bag checks and to load bus Space is limited! You may bring a total of 3 bags: 1 suitcase with clothing, food, etc. to go below the bus. This bag will not be available until we arrive at the hotel in Lake Placid on Friday morning. 1 equipment bag, also to go below the bus. DON T FORGET skis/board! 1 carry on bag with the homework you will do on the bus, your current novel in English class, money, headphones, snacks. 8:00 pm - Depart for Lake Placid! Do NOT be late. You WILL be left without a refund. Friday, 02/17/17: ~ 6:00 am: Arrive at the Summit Inn, 2375 Saranac Avenue, Check in (if possible) and enjoy the slopes at Whiteface! Dinner on your own in Lake Placid Return to the hotel by 9:00 pm. 10:00 pm. Room check and lights-out. Saturday, 02/18/17: 7:00 am: Breakfast (included). 7:45 am: Depart hotel. Enjoy the slopes at Whiteface! Dinner on your own in Lake Placid Return to the hotel by 9:00 pm. Curfew - 10:00 pm. Room check and lights-out. Sunday, 02/19/17: 6:00 am - Check out of rooms. Breakfast (included). Clean room, prepare for room check, turn in keys, load bus * Pack a change of clothes in your carry on bag for the bus ride home. 7:30 am - Depart hotel. Enjoy the slopes at Gore Mountain! 4:00-4:30 pm Change clothes, begin loading busses for trip home 5:00 pm Depart for USC. Dinner stop on the way home! Monday, 02/20/17 (No School):

INFORMATION PACKET: Security Form & Medical Release The Summit Inn in Lake Placid, New York does not require a security deposit. However, there will be an initial and final room check at this hotel. Upon the initial room check, students are to report any damages that they notice in the room. Any subsequent damages found on the final inspection may result in additional charges by the hotel. It will be the student s responsibility to pay for these damages upon returning from Upper St. Clair. All students in that room will split the cost equally. The club will pay for the damages and the students involved will be responsible for reimbursing the club. Failure to pay for these damages will result in a removal from the club as well as a disciplinary action at USCHS. The most common causes for the security deductions include, but are not limited to: damage beyond what is considered reasonable wear and tear, extra cleaning, noise complaints/ violations, loss of keys, failure to vacate units on time, inventory loss, tampering with locked spaces, broken windows, etc. Security Agreement By my signature below, I indicate that I have read, understand, and agree to abide by the information above. If my son/daughter causes any damage or is staying in a room involved in any noise complaint resulting in a fine, my son/daughter is responsible for his/her share of the additional costs. Furthermore, if my son/daughter does not reimburse the club for damages, I agree to assume all costs to repair or replace the damaged item(s) at the hotel. 6 INFORMATION PACKET: Permission and Release Form Name of Student: By my signature below, I indicate that I do hereby allow my son/daughter to attend the Ski Club trip to Lake Placid. The Undersigned parents or guardians and student are aware that any sport or activity involves some risks, and understand that participation in the Ski Club, or travel on Ski Club trips to mountain resorts, is not required, but rather is voluntary. The Undersigned hereby consent to the participation by the student identified above ("Student") in the Ski Club activities, and assume all risks of harm arising out of this activity. In consideration of the agreement of the Ski Club and the School District to include the Student in Ski Club activities and/or travel relating to the Ski Club, the Undersigned hereby release the Ski Club, the faculty sponsors, any other sponsors or chaperones, the Upper St. Clair School District, and all current and former directors, officers, employees and agents of Upper St. Clair School District (collectively "Releasees"), from any and all claims, liabilities, demands, actions, causes of action, expenses and attorneys fees, whether for negligence or otherwise, and/or for personal injury, property damage or otherwise, arising directly or indirectly out of participation in Ski Club activities, including travel (collectively "Claims"). In further consideration of the agreement of the Ski Club and the School District to include the Student in this activity, the Undersigned also hereby agree to indemnify, defend and hold harmless the Releasees from any Claims which any of the Undersigned cause or are involved in, and to reimburse the Releasees for any losses, damages, costs, or expenses which any Releasee may incur by reason of any such Claim. By his/her signature, the Student hereby agrees to abide by all applicable Upper St. Clair School District rules, including the General Rules as well as the local laws, ordinances and rules of Whiteface Mountain, Okemo Mountain, and/or the hotel. If my son/daughter fails to abide by any of these rules, he/she may be removed from the trip without being entitled to a refund. Furthermore, he/she may be subject to Upper St. Clair School District disciplinary proceedings and/or local law enforcement proceedings. Student name: Signature: Date:

INFORMATION PACKET: Rules and Policies The following rules have been developed to help each student have a safe and enjoyable time. In addition to observing these rules, the student s conduct must not be contrary to the criminal code or laws of the state of New York, the local governing body, or its published or posted rules and policies. The student must also abide by the rules and regulations of the ski area. Students must also remember that they are at all times representing Upper St. Clair High School, and are expected to show courtesy and respect to all chaperones, employees of the ski resort, and other skiers. 1. IN ORDER TO ENSURE A SAFE AND ENJOYABLE EXPERIENCE FOR SKI CLUB PARTICIPANTS, ALL BAGS, BOOTS, JACKETS, AND OTHER CONTAINERS ARE SUBJECT TO SEARCH AT ANY TIME BY THE SKI CLUB CHAPERONES, SCHOOL ADMINISTRATORS, AND/OR SCHOOL POLICE. Should you or your child be in disagreement with this practice, please do not take part in Ski Club activities. 2. NO SMOKING OR USE OF ANY TOBACCO PRODUCT IS PERMITTED AT ANY TIME WHILE ON SCHOOL PROPERTY, THE BUS, AT THE HOTEL, OR AT THE SKI RESORT. 3. NO ALCOHOLIC BEVERAGES OR DRUGS OF ANY KIND ARE PERMITTED TO BE CARRIED OR CONSUMED WHILE ON THE TRIP. Any student who appears to be intoxicated while on the ski trip will be required to remain in the ski lodge with a chaperone and his/her parents will be contacted and required to provide transportation home from the resort for the student. 4. Bag lunches and sealed drinks may be taken on the bus. Only manufacturer sealed containers are permitted. NO OPEN THERMOSES, OPENED GLASS CONTAINERS, OR OPENED PLASTIC BOTTLES MAY BE TAKEN ON THE BUS. 5. All students attending ski trips must follow the USCHS dress code in the Student Handbook. 6. Students may not leave the ski slope and lodge area at any time between arrival and departure at the ski resort. 7. Students must attend school on Tuesday, February 17, 2016 to participate in future ski club activities. 8. Any damage that our students create or cause during the trip becomes the responsibility of ALL the students unless the identity of the individual or individuals responsible is known. 9. Students are not permitted to enter hotel rooms of students of the opposite sex. All room assignments will be single-sex. Students may socialize in common areas of the hotel. 10. The bus must be kept clean at all times. If the bus is left in poor condition, those responsible will not be permitted to participate in further Ski Club activities. 11. In case of injury, chaperones will contact the parent or guardian of the injured student. We will not transport home any student whose injury, in the opinion of the medical personnel at the ski area and the Ski Club sponsor, could be worsened by the ride on the bus. It is the responsibility of the parent or guardian to provide proper transportation of an injured student. 12. THE SKI CLUB SPONSORS, SKI CLUB OFFICERS, SKI CHAPERONES, OR ANY ADULT OR STUDENT INVOLVED IN THE PLANNING AND EXECUTION OF THE SKI TRIPS WILL NOT BE HELD RESPONSIBLE IN THE EVENT OF AN ACCIDENT. 13. ALL SKIERS AND BOARDERS MUST WEAR A HELMET AT ALL TIMES WHEN SKIING/RIDING. 14. REFUNDS CANNOT BE MADE FOR DEPOSITS PAID TO THE SKI CLUB. Please notify the Ski Club sponsor immediately if you do not think you will be able to attend the trip. 15. NON-COMPLIANCE WITH THE RULES CAN RESULT IN SUSPENSION OR EXPULSION FROM SKI CLUB WITH NO REFUNDS.
