Access Alliance of Michigan (AAM) and Riverhaven Coordinating Agency (RCA) are divisions of the Bay-Arenac Behavioral Health Authority. AAM and RCA manage public mental health and substance use disorder services. Bay-Arenac Behavioral Health Authority is the Pre-paid Inpatient Health Plan (PIHP) for persons with Medicaid. The AAM also includes Huron Behavioral Health, the Montcalm Center for Behavioral Health, Shiawassee County Community Mental Health Authority, and Tuscola Behavioral Health Systems. 7 Emergency Services Page 3 Emergency and After-Hours Access to Services A mental health emergency is when a person is experiencing a serious mental illness, or a developmental disability, or a child is experiencing a serious emotional disturbance and can reasonably be expected in the near future to harm him/herself or another, or because of his/her inability to meet his/her basic needs is at risk of harm, or the person s judgment is so impaired that he or she is unable to understand the need for treatment, and that their condition is expected to result in harm to him/herself or another individual in the near future. You have the right to receive emergency services at any time, 24-hours a day, seven days a week, including holidays, without prior authorization for payment of care. An emergency service is a crisis intervention program with staff trained to help persons with an emergency mental health condition. In the event of an emergency, you have the right to use any emergency service provider or any other setting for care. If you are in a life-threatening situation, you should call 911 right away, or you can go to your local hospital emergency room. We have emergency services available for each of our counties. If you have a mental health emergency, you should seek help right away. 8 Page 4 About this Provider Directory About this Provider Directory What are behavioral health services? What types of behavioral health services are available a residential group home, outpatient mental health treatment and more? What behavioral health services are located in my community? Why a Directory? This directory has been created to help you make an informed decision in selecting a service provider. You have probably already been working with your current provider to decide what services will work for you and suit your unique and specific needs. Then, you must locate a provider to make those services available to you. A provider is any organization or licensed independent practitioner currently meeting the credentialing guidelines and under contract to deliver behavioral health services to the persons we serve. It s up to you to decide who you want to provide the services you need. You can choose the providers you and your service provider like and determine to be most suitable for you, or you can make your own informed choice. It s up to you.

9 About this Provider Directory Page 5 You have the freedom to know about and choose from any of the appropriate contracted service providers in our region. These providers work beside you as you start making your own Person-Centered Plan which maps out how you will get better (recovery), leads you to have as much freedom and choice as possible, helps you find ways to become part of where you live and work, and helps you reach having the best life that you possibly can. and possibly the type of funding that is used for your services. Many of the community mental health and substance use disorder services and providers are available to individuals of all ages (infancy through elderly). The directory is to help you find what you are looking for. It shows you all of our available behavioral health services, providers and where they are located. This directory answers the question, If I need a service such as outpatient therapy, which ones are available and is there one near me? So, you can look up what you need by the type of service needed and see where the services are located. This directory also includes information about non-english languages and sign language spoken by our providers. Please note that this information is not meant to imply that staff are certified language interpreters. However, at no cost to you, an interpreter can be arranged on your behalf if necessary for you to access care. This directory includes information about many different types of service providers. It is important to know that all primary care providers (pages 10-11) are accepting new patients. In addition, inpatient hospitalization services are available to all persons on a 24 hour per day/7 day per week basis. However, the availability of specific hospitals or other specialty service providers (pages 10-26) will vary depending on individual provider capacity This directory gives you options to help you be sure that you ve made the best decision about your services based on what is best for you and available. 10 Page 6 Primary Care Providers Primary care providers are responsible for providing direct assessment, person-centered planning, intervention, periodic monitoring and evaluation to persons receiving mental health or developmental disability services. This includes outpatient therapists, physicians, case managers, supports coordinators, assertive community treatment and home based programs. Primary care providers are the most important components of care as they also serve as the gatekeeper for access to the larger continuum of specialty mental health services and supports. The focus of treatment is to support each person s one-of-a-kind needs in reaching his or her own recovery goals. Inpatient Psychiatric Care Community Mental Health Service Definitions Community Mental Health Service Definitions impairments and manifesting a level of clinical instability (risk) that either individually or collectively are of such severity that treatment in an alternative setting would be unsafe or ineffective. Post-Stabilization Services These are services related to a recent emergency mental health condition that are provided after a person has been stabilized in order to maintain the stabilized condition or improve or resolve the person s condition. Specialized Residential Services These are personal care services provided to a person living in a licensed Adult Foster Care Home, certified by the State to provide a Specialized Residential Program. The residential services are individualized depending upon the needs and preferences of the person, but typically include additional staffing resources for purposes of treating physical, psychiatric and/or behavioral conditions. Inpatient Psychiatric Care may be used to treat persons who require care in a 24-hour medically structured and supervised facility. The Severity of Illness (SI)/Intensity of Service (IS) criteria for admission are based upon the assumption that the person is displaying signs and symptoms of a serious psychiatric disorder, demonstrating functional Community Living Support Services Community Living Supports (CLS) are supports used to increase personal self-sufficiency, facilitating personal achievement of goals of community inclusion and participation, independence or productivity. The support staff may be provided in the person s residence (i.e., licensed facility, own home, family home, apartment) or community settings.

11 Community Mental Health Service Definitions Page 7 Psychosocial Rehabilitation Services Psychosocial rehabilitation services is a community-based program in which the beneficiaries (also called clubhouse members ), with staff assistance, are engaged in operating all aspects of the clubhouse, including food service, clerical, reception, janitorial and other member supports and services such as employment, housing and education. In addition, members with staff assistance participate in the day-to-day decision-making and governance of the program and plan community projects and social activities to engage members in the community. Through the activities of the ordered day, clubhouse decision-making opportunities and social activities, individual members achieve or regain the confidence and skills necessary to lead vocationally productive and socially satisfying lives. Supported Employment Services Supported Employment Services provide job development and initial and ongoing support services to assist persons in obtaining and maintaining paid employment that would be otherwise unachievable without such supports. Support services are provided continuously as needed throughout the period of employment. Capacity to intervene to provide assistance to the individual and/or employer in episodic occurrences of need is included in this service. Supported/integrated employment is provided in integrated work settings where the beneficiary works alongside people who do not have disabilities. Skill Building Services Skill-building assistance consists of activities that assist a person to increase his/her economic selfsufficiency and/or to engage in meaningful activities such as school, work and/or volunteering. The services provide knowledge and specialized skill development and/or support. This includes but is not limited to: assistance with acquisition, retention, or improvement in selfhelp, socialization, adaptive skills and supports services. Services are aimed at preparing a person for paid or unpaid employment, but that are not job task-oriented. Activities included in these services are directed primarily at reaching habilitative goals, such as improving attention span and motor skills, not at teaching specific job skills. Peer Delivered or Peer Operated Support Services Peer delivered or peer operated support services are programs that provide individuals with opportunities to learn and share coping skills and strategies, move into more active assistance and away from passive patient roles and identities, and to build and/or enhance self-esteem and self-confidence. These services may be provided in many different settings, including the home or community.

12 Page 8 Substance Use Disorder Outpatient Services Outpatient Therapy may include one-on-one meetings with a clinician and the person seeking substance use disorder (SUD) treatment. Family members and other people close to this person may also be there if he/she chooses. Group Therapy Substance Use Disorder (SUD) group counseling is led by a therapist in a group setting of people dealing with many types of SUD issues. Group Therapy usually includes educational talks, helpful discussions, and other group activities. Substance Use Disorder Prevention Services Prevention Programs are for people who do not yet require treatment for substance use disorders (SUD). Funded prevention services must be provided in many different settings for both the general community and for those who may be at a high risk for developing a SUD. Sub-acute Detox Services Sub-acute Detox Service is medically supervised care provided in a substance use disorder residential setting to help a person stop using alcohol or drugs (withdrawal). As one goes through withdrawal, one usually feels physically sick. The detox services give a person a safe setting to manage these side effects. Substance Use Disorder Service Definitions Substance Use Disorder Service Definitions Methadone/Other Medication Supported Services Methadone and various other medications may be used to help a person be successful in recovery. They may be used to assist in detoxification and/or for continued medication maintenance. Medications are usually used once a person has tried other treatment and is not successful in staying in recovery. Medications such as Methadone or Suboxone are used for opiate dependence. These medications are used with individuals that have developed serious addictions to opiates. They may be used for either detoxification (getting the drugs and/or alcohol totally out of a person's system in a short amount of time in a supervised inpatient setting) or maintenance (allowing a person to continue to take the drug over a longer period of time, while working with them to slowly get off of the medication in an outpatient setting). These medications may have serious side effects and may be dangerous if not used according to doctor's orders. Methadone and Suboxone are usually only used once other options have been exhausted. Methadone and/or Suboxone programs usually require an individual to travel on a daily basis to receive the medication, as well as other services

13 Substance Use Disorder Service Definitions Page 9 such as: doctor visits, individual counseling sessions, group counseling sessions, urine drug screens. An individual that is thinking about entering into treatment with methadone or Suboxone must have reliable transportation and a serious commitment to their recovery. Residential Treatment Residential Treatment is a supervised live-in substance use disorder program. It includes several types of therapy: individual therapy, group instruction, and peer support all within this supportive environment. There are residential programs for men, women, and adolescents. Some specialized women s residential programs allow children to live with them during treatment. Case Management The focus of case management services for people with substance use disorders is to help find, link, and use needed community resources. These resources improve one s long-term recovery. The community resources may include such things as housing, health care, employment opportunities/trainings and other social services. Case management staff work alongside the person and the treatment team; together they support the one-of-a-kind needs to help a person reach his/her recovery goals. Recovery Support Services Recovery Support Services are most often services provided in the community outside the usual treatment agency. These services are intended to support long term recovery of a person with a substance use disorder whether that person is at the beginning, middle, or maintenance stage of recovery. This may include linking a person to community resources, assisting a person with basic needs (food, heat, clothing, etc.), providing support (talking with, listening to, and providing guidance) to this person, and acting as a positive role model. Ancillary Services Women s Specialty Services This is specific substance use disorder (SUD) treatment and other therapeutic interventions for women with SUD who are either pregnant or have dependent children. It helps with such issues as relationships, sexual and physical abuse, parenting and child care. Women with children have many barriers to treatment. With Women s Specialty services, those barriers are decreased. Women s Specialty services include: referrals to primary medical care for women, referrals for pediatric care for children, child care for the children while the mother is receiving services, transportation to and from treatment, gender specific treatment, therapeutic interventions for children, advocacy, case management services Dual Diagnosis/ Co-occurring Treatment Co-occurring disorder treatment are services provided to persons who have both substance use disorders and mental health issues. All contracted providers are trained and have skills to treat both issues at the same time for the best treatment success.
