1 HEALTH CARE 1. FIRST LINE HEALTH CARE 1.1. General Information Free choice In Belgium patients have free choice of doctors. In general, choices are made based on others people recommendation, distance, gender, knowledge of English, etc. Often it is matter of coincidence: one needs a doctor urgently. The doctor on guard is called and becomes from that moment on the family doctor GMD - Globaal Medisch Dossier (Global Medical File) In order to limit medical shopping the government rewards patients financially who stick with one general practitioner (GP). The whole of the patient s medical history is then centralized in one databank, the GMD. GP s do charge a small fee for this service which, however, is completely refunded by the Health Insurance. Patients are entitled to extra benefits such as a 30 % reduction on their co-payment RATES AND REFUNDS The fees doctors charge for a specific service is determined by a convention between the Doctors Unions and the government represented by the Health Insurance Companies. Doctors who respect this convention are called geconventioneerd/conventionné. Patients know exactly how much they will be charged and how munch refund they will receive. Doctors who are following trainings on a regular basis to keep up with the newest developments, (geaccrediteerd/accredité) are allowed to charge a small supplement After paying the doctor, patients receive a receipt to submit to the Insurance for refund. 2 IMPORTANT: Although most doctors do, there is no obligation for doctors to comply with the convention. They can ask any amount they wish. So please in case of doubt check with the doctor whether he/she is geconventioneerd/conventionné and agrees to respect the official tariffs. Doctors can respect the convention when practicing in a hospital but may not do so when seeing patients in their private practice General practitioners In Belgium GP s are not civil servants. Most are self employed. Currently, for the Brussels Region, roughly, one doctor is available per 178 inhabitants (1 per 244 inhabitants as for the whole of Belgium). Lists can be found in the yellow pages of the phone book under the section Huisartsen (Dutch) and Medecins Généralistes (French): sartsen/?contenterrorlinkenabled=false Fill a keyword (here huisartsen ) in the first search field and your location (here: Elsene commune) in the second. TIP: The following website in English has a search engine to locate a GP in your neighborhood: Group practice In order to guarantee a round the clock service and still have a social life there is a tendency for starting GP s, especially in densely populated area s to work in group practices. In case of emergency there s always a doctor available. You still can choose to stick to a particular doctor but every now and then you ll end up with a colleague. Patient files are centralized and can be consulted by the whole team. VUB has a group practice on the main campus (see section 1.8.) 1.4. House Calls When choosing a GP please check whether the service includes house calls. Some doctors do not make house calls, others only within a specific perimeter. A supplement is charged for this (see section 1.1.3) Evening and weekend permanence When the assistance of a GP is required evenings or during weekends please call: Dutch speaking permanence: 02/ (Vlaamse Wachtdienst) French speaking permanence: 02/ (SOS Médecins) or 02/ (Medi Garde). 3 If, in case of real emergency and no doctor can be reached within a reasonable time, it is possible to turn to the emergency ward of the (nearest) hospital or call 112, the international emergency number. (see also section 2.4) 1.6. Free Clinic The concept of Free Clinics hails from the 1960 ties and had the ambition to offer free medical care for all. Its initial goal was to promote family planning and make anticonception available. The Free Clinic in Matonge exists since The services are open to the public 54 hours a week. A medical permanence without appointment is available. The clinic has a staff of 37 professionals of various disciplines: general medicine, family planning psychological assistance and legal assistance. Free Clinics only charge refund rates. Since no co payment is due the patient pays nothing Psychological and psychiatric care Centra voor algemeen welzijnswerk (CAW ) First line psychological assistance is provided by a network of by the government subsidized organizations, called CAW (Centrum Algemeen Welzijn). CAW offers professional help to people facing various problems that have an impact on their general wellbeing. For example people facing loss, loneliness, anxiety, depression, phobia, stress, compulsive behaviour, etc. Other issues like legal and social problems can also be addressed. Important is a low threshold approach with no waiting lists, easy contact, availability nearby, confidentiality and often free of charge. The multi disciplinairy team consists of social workers, physicians, psychologists and psychotherapists. Sometimes lawyers and psychiatrists are also available. Specific fields of action are: trauma due to domestic violence or crime in general. Mozaik is a CAW centre located on campus (see section 1.7). 3
4 Centra voor geestelijke gezondheidszorg (CGG) This kind of centre focuses solely on the treatment of psychological illnesses. Unlike a CAW, a GGZ team not only consists of psychologists and psychotherapists but also of psychiatrists which also allows them to prescribe psychopharmaca. There is a GGZ linked to VUB (see section 1.7) Tele onthaal You can call Tele Onthaal regarding all your personal questions and problems. It is completely anonymous and is always reachable (24 hours during all 7 days of the week) The numbers are 106 (Dutch) and 107 (French). 02/ Other organisations offering specific assistance Child Focus: Child Focus deals with all aspects of child abuse and missing children (abduction as well as fugue). It issues and distributes posters of missing children that can be seen in shop windows and on public transport vehicles. It also offers support to parents of missing children Suicide Prevention Phone Line: This organization gives assistance (free of charge) for everyone confronted with suicide: People considering committing suicide People who in the past have tried committing suicide Afraid someone you know may be suicidal Next of kin needing assistance after a suicide Contact details: Centrum ter preventie van zelfdoding (Dutch): 1813 Centre de prevention du suicide : (French) 0800/ Facilities at VUB Main campus has its own group practice and CAW. Specialists Specialists can have a private practice or exercise in polyclinics and hospitals. Often they combine both options. In general, patients are expected to have an introduction letter but it is possible to make appointments without the intervention of the GP. You can easily make an appointment with disciplines such as gynecology or dermatology without a referral of a doctor Polyclinics Polyclinics are departments within the hospital that offer a wide array of disciplines. Patients can go to polyclinics for a medical consultation or a small treatment but it doesn t include hospitalization Hospitals Origins Hospitals in Europe originate from charity practiced by religious orders in the Middle Ages. This is why, even today, many hospitals are managed by catholic organizations and bear names of catholic saints and may have Christian symbol present in the buildings. Hospitals can be managed by the private sector as a commercial venture or linked to a Medicine Faculty of a University. The latter are referred to as Academic Hospitals. (VUB s academic hospital is the UZ Brussels) Other hospitals are an initiative from the public sector like communes. In some cases the ideology can have an influence on medical practices offered or refused by a hospital. Catholic hospital may refuse life shortening/ending interventions, abortions, artificial assisted conception or treatments that include the use of stem cells Checking in Checking out When checking in for hospitalization, the hospitals reception will require proof of identity, proof of health Insurance (ask your Health insurance company) and will charge a down payment. Proof of additional private Insurance can be submitted (hospitalization). When checking out you will be asked to sign a release form. 5
6 2.3.3 Rates and refunds It is nearly impossible to determine exactly in advance what a hospitalisation will cost since complications can always occur. You can ask the surgeon for an estimate of the cost for the intervention or treatment. In most cases getting an exact price in advance will not be possible. A recent survey published by the Belgian consumers organisation Test Aankoop/Test Achat revealed that an enormous difference exists in what hospitals charge for the same intervention. Hospital bills are composed of 3 categories of cost: - Part paid by the Insurance company - Co payment to be paid by the patient - Supplements to be paid by the patient Hospitals will forward the bills directly to the insurance company. The patient will receive a bill only for the co payment and the supplements. Important remarks: - When a patient specifically asks for a single room or accepts one without medical necessity, the hospital can charge whatever amount they want and are no longer bound to the convention. - Supplements can proof to be an unpleasant surprise. It comprises items like technical material (seringes, bandages, medication, disposable materials) and things like water, food other than the usual meals, etc. - Invoices can take a long time to arrive. In some cases bills arrive months later, after students are long gone. In order to avoid legal problems contact the hospital regarding this matter before leaving and give them an address in your home country. More information on refunds is available in the info file Insurance 2.4. Emergency In case of emergency it is possible to go to the hospital s emergency ward. However be prepared to spend a lot of time there under sometimes uncomfortable conditions. The medic at the reception will proceed to a procedure called triage in order to determine what patients need taking care of first. Patients with life threatening conditions or injuries will be given priority Ambulance In Belgium, ambulances called via the national emergency number 100 or the European emergency number 112 will take you to the hospital considered to be the one best equipped for your condition (possibly but not necessarily the nearest one). So, transport by ambulance, following a 100 call, will not allow you to choose the hospital to be taken to. 6
7 Non-emergency transport by ambulance (in case of reduced mobility) does offer the possibility to choose your destination. Transport by ambulance is not refunded by the Health Insurance s general policy but often covered by the complementary policy. 3. TREATMENT, THERAPY AND MEDICATION 3.1. Pharmacy In Belgium, the sale of drugs is strictly regulated. Pharmaceutical products can normally only be sold in Pharmacies. We do not know the concept of drugstores selling medication like in the US or UK, especially for medication that requires a prescription from a doctor. Even Pharmacies are not allowed to sell these without prescription. Foreign companies escape government control but for just that reason compose a serious health hazard. Drugs purchases through these channels may just be fake but harmless or downright dangerous. Night and evening permanence for pharmacies can be found on posters in pharmacists windows or by calling: 0900/ or 0900/ There is also a search engine available (only in French and Dutch): For security reasons, during the night, pharmacists will not let you in but will use a small secured window. Make sure to have cash money Paramedical care HOME NURSING After hospitalization people may need some care by a professional nurse for a while (wound care, help with bathing, etc. This kind of assistance may be permanent in case of reduced mobility caused by old age or handicaps. Regular visits of a paramedic are available from independent nurses or from organizations like White Yellow Cross or Health Insurance companies. Some of the services offered may also be non medical and include small domestic chores (laundry, tidying, doing the dishes, etc.) and cleaning LOGOPEDY, PHYSIOTHERAPY, DIETIST In order to qualify for refunds form the Health Insurance a prescription form a GP is required ALTERNATIVE TREATMENTS (homeopathy, acupuncture, chiropractor, etc.) The Ministry of Health is monitoring very closely the landscape of alternative medicine and applies very strict rules on any practitioner who claims to be able to cure deceases, illnesses or handicaps. Practitioners of alternative therapies may own an official doctors degree or not. 7
8 Growing popularity of alternative medicine made Health Insurance Companies decide to accept limited refunds under the complementary insurance policy. 4. COMPLAINTS LEGAL ACTIONS PATIENT Some hospitals have intercultural mediators who offer the following assistance: - Translators - Cultural brokerage : explaining the culture of the hospital and of the physician to the patient and the world of the patient to the physician - Practical assistance - Emotional support - Mediation in case of conflicts - Outreach assistance - Providing information A patient who wishes to file a formal complaint against a physician is advised to seek assistance from the juridical department of the Health Insurance. In Belgium taking a case to court is not as common at all as it in the US. Procedures can drag on for years. 5. FOUNDATIONS People suffering from, mostly, chronic diseases or handicaps can turn to a variety of specialized initiatives. Some of these organizations or foundations exist since a long time and are well known by the public because of their regular fundraising activities in the media. Many of them used to be bear names starting with Belgian or National but virtually all of them have been divided in Dutch speaking and French speaking sections. All of the following medical conditions have their own organizations that offer information, support and practical assistance: Cardio vascular problems, Multiple Sclerosis, Aids, Crohn s disease, Alcoholism, Diabetes, Epilepsy, etc. Often patients have started self-help groups allowing members to exchange experiences and rely on each other for support in moments of crisis.
