1 COMPREHENSIVE JOB DESCRIPTION Dungeness Courte Director of Nursing Services (DNS) Reports To: Dungeness Courte Executive Director Position Summary: Responsible for planning, implementing and supervising resident care and services in a residential setting, which may include a dementia unit. Duties may include the following: Interviewing, hiring, evaluating and disciplining resident services staff members. Establishing time schedule for nursing staff on specific shifts. Assigning and delegating responsibilities to various nursing staff for specific residents and their care needs based on the qualifications of the staff and the needs of the resident. Ensuring that each resident receives services and prescribed or requested in accordance with federal and state regulations and facility policies and procedures. Assuring prompt, appropriate action in cases of injury, illness, fire and/or other emergencies. All duties will be performed in accordance with established facility policies and procedures, resident service objectives, and resident rights policies and objectives. Essential Job Functions: Listed below is an outline of the essential job function requirements. This listing of essential job function expectations in no way implies that an employee is expected to do the physical abilities specified on a daily basis. It is simply a definition of the physical expectation requirements should the need arise. All newly hired Director s of Nursing Services are expected to be able to meet or exceed the essential job function requirements. Must be physically able to lift up to 25 pounds on a frequent basis, 50 pounds on an occasional basis and 100 pounds on a rare basis. Must be physically able to carry up to 10 pounds on a frequent basis and up to 25 pounds on a rare basis for up to 100 yards. Must be physically able to stand and walk up to 95% of the workday. Must be physically able to push/pull the medication cart and transport residents in wheelchairs occasionally throughout the workday. Must be physically able to climb stairs and climb onto and off of a three-step stepstool. Must be physically able to twist, bend, stoop, crouch and kneel occasionally to frequently throughout the workday. Page 1 of 5

2 Must be physically able to handle, grasp and reach occasionally to frequently throughout the workday. Must be able to hear and speak as necessary to communicate with residents, coworkers, visitors and guests. Must be able to see as necessary to do their job safely. Must be able to smell as necessary to discern resident s needs and/or building emergencies. Must be honest, trustworthy, patient, dependable, and courteous. Must have the ability to work well with others and be able to follow instructions (oral and written) accurately, timely and efficiently. Must maintain a professional appearance while on-site and/or in the course of employment. Must be drug and alcohol free and have a clean police record. May be subject to random or specified drug and alcohol screening (see personnel manual). Must be free of communicable diseases. Duties and Responsibilities: Listed below is an outline of the duties and responsibilities that the employee may be required to perform. This listing of expectations is not to be construed as an all-inclusive list and in no way states or implies that these are the only duties the employee will be required to perform. Personnel: Ø Assists in the recruitment, interviewing and hiring of quality staff to meet staffing requirements while staying within budgetary constraints and without a need for temporary or agency help. Ø Counsels and disciplines employees when required in a fair and consistent manner per facility policies and mandated rules and regulations. Ø As directed, schedules nursing staff and assures correct number of staff members are on duty pursuant to resident s needs and budgetary guidelines. Ø Replaces absent, scheduled employees as needed per resident care and facility needs. Ø Coordinates assignments with other support personnel as needed/indicated. Ø Provides a written assignment for each employee consistent with the employee s experience and training. This assignment outlines residents to be cared for and other duties to be executed during the assigned shift. Ø Supervises personnel and evaluates performance on an ongoing basis to ensure residents receive appropriate and required care. Ø Evaluates, discusses plans of action and follow through involving issues in the nursing unit. Ø Assures documentation on resident records by unit personnel is complete and accurate per regulatory and facility requirements. Ø Reviews and assures that all nursing staff have appropriate and required licenses, training, CPR and First Aid documentation as required by state regulations. Ø Provides staff in-service training to as indicated by the facility and resident needs to assure that all staff has received the appropriate and required training necessary to accomplish their assignments and provide the correct resident care. 3 Ø Keeps nursing staff informed of any changes involving resident services, policies and procedures. Ø Instructs nursing staff on the use of equipment as needed. Resident Services: Ø Prepares a completed and specialized dementia care resident service plan on all new residents prior to admissions with the resident and other involved parties input. Ø Prepares a specialized dementia care assessment for each resident admitting to the dementia care unit. Ø Reviews resident s charts on a routine basis and prepares updates on resident care needs no less than bi-annually (may be more frequent based on the needs of the residents). Ø Participates in the development and implementation of resident services and care plans for all residents, and specialty care plans for dementia residents. Ø Encourages resident and family/surrogate participation in care planning by explaining approaches and involving the resident and their families in teaching programs. Ø Makes resident rounds frequently to assure that each resident is given timely and appropriate care (includes medication, treatments, prescribed diet and nourishments, and skin care) and is kept clean and comfortable. Ø Observes residents symptoms, reactions to medications and/or treatments, changes in condition and unexpressed needs. Ø Takes appropriate action and notifies physician and family/surrogate decision maker when significant changes in a resident s observed condition occurs. May include writing incident reports as indicated and following-up on incidents from other shifts as well as those occurring within the Director of Nursing Services designated shift. Ø Prepares, administers and documents medications and treatments as ordered by the resident s physician and in accordance with federal requirements, state regulations and facility policies and procedures. Ø Orders and receives medications from pharmacy. Ø Contacts resident s physicians and record physicians verbal orders. Ø Conducts monthly drug re-cap according to facility policy and procedure. Ø Counts controlled drugs and disposes of discontinued medications according to state regulations. Ø Performs necessary procedures and works with equipment per facility policies and procedures. Ø Monitors each residents physical and mental functioning, psychological wellbeing and nutritional status to detect deviations from baseline and to gauge progress. This includes obtaining and documenting vital signs, weight, intake and output, diet supplements and replacements, etc according to facility policies. Ø Ensures universal precautions are used properly throughout the unit. Ø Maintains work area in a clean, safe and sanitary condition at all times. Ø Ensures residents are treated with dignity and respect. Ø Investigates complains of residents and family/surrogate and takes appropriate action as indicated. Ø Makes necessary arrangements for consultants, laboratory and x-rays as necessary. Page 3 of 5

4 Facility: Ø Complies with budgetary guidelines as laid out in the facility budget. Ø Discusses any potential budget overages with the Executive Director prior to approving the expenditures. Ø Verifies labor hours daily per budget guidelines. Ø Reviews and verifies that all nursing personnel are using the time clock appropriately and taking breaks as indicated by policy and regulations. Ø Maintains good public relations with families/surrogates, residents, visitors, contractors, community members and other facility personnel. Ø Responds quickly to all emergency situations. Ø Responds quickly to all emergency situations. May include: o Assisting in fire control and resident evacuation procedures during fire/disaster. o Assuring unit personnel know actions to be taken during fire/disaster. o Perform CPR as necessary. Ø Must meet continuing education requirements when necessary for state licensing/registration. Ø Assists nursing personnel as needed to maintain continuity and efficiency of services to residents. Ø Perform related work as required/requested. Education and Special Requirements: Must be currently licensed with the state board of nurse examiners as a Registered Nurse. Must have supervisory and managerial experience. Must have a current CPR certification. Must have current documentation of First Aid, HIV training. Must have professional appearance and excellent work habits/work ethic. Must posses the ability to acquire and retain ongoing industry related knowledge and a working understanding of federal and state rules and regulations governing board and care/congregate facilities. Must have excellent problem identification and resolution skills. Must have the ability to teach and/or make oral presentation to individuals and groups. Must display a genuine interest in the needs of the aged. May be asked to successfully pass a medication pass audit upon hire. Must be able to demonstrate competency in medication administration terminology and abbreviation. Must be able to read, write and speak the English language in an understandable manner. Must have the ability to relate positively, effectively and appropriately with residents, families/surrogates, community members, contractors and all levels of personnel. Must be self-motivated and well organized. Must demonstrate integrity, imagination and initiative while showing appropriate manner and conduct. Must have the ability and desire to relate well to persons of varying backgrounds, abilities and expectations. Page 4 of 5

5 Must have the ability and knowledge to use and demonstrate the use of all resident services equipment. Must have the necessary manual dexterity to manipulate equipment. Must have the ability to direct, delegate and follow-up on services/care of the residents. Must have the ability to provide positive motivation of all team members. Must have documentation of TB testing in compliance with facility policy and State law. Must have Washington State Patrol background check with no convictions in violation of RCW Must understand and work at all times with Standard Precautions and prevent the spread of infections and disease. Must become familiar with and follow facility policies and procedures. Must be able to work independently, make appropriate decisions in emergencies and safeguard resident safety and wellbeing while on duty. May be required to work weekends, holidays and after-hours as needed to assure resident s needs are met and staffing requirements are full. Must be able to be on-call for facility emergencies and resident consultation services with on-duty staff members. Must attend mandatory meetings and in-services. May be subject to falls, exposure to odors, etc. including the potential exposure to HIV and Hepatitis B viruses.
