It is our goal to serve your family in a way that best meets your needs. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. All pricing included in this General Price List includes a discount of 3% for payment by means other than credit card. 2 GENERAL PRICE LIST Our Payment Policy It is customary that accounts are to be paid in full at time of arrangement. Alternatively, payment in full will be required 48 hours before services. If you anticipate any difficulty meeting this obligation, please discuss it with your director at the time of the arrangement. Method of Payment For your convenience, we accept: Personal checks Cash Visa, Master Card, American Express and Discover Card (Discount for cash payment, reversed 3%) Insurance assignments (requires validation with insurance company) Goods and Services The goods and services shown are those we can provide to our customers. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services. If legal or other requirements mean you must buy any items you did not specifically request, we will explain the reason in writing on the statement we provide describing the goods and services you select. Professional Services Basic Services of Funeral Director and Staff This fee for our basic services and overhead will be added to the total cost of funeral arrangements you select. This Fee is already included in our charges for direct cremation, immediate burials, forwarding or receiving remains. This fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to the initial call. Arrangement conference. Coordinating service plans with cemetery, crematory, church, pastor, and/or other parties involved in the final service and disposition of the deceased. Securing and recording the death certificate and disposition permit with local authorities. This charge covers a portion of overhead, including facility maintenance, equipment and inventory cost, insurance, administration expenses, and governmental compliance. $1,195 Other Professional Services Filing Insurance Claims (assignment to funeral home) Minimum assignment -less than $2000 (or 8% of total) $160 Filing a death certificate and /or acquiring a permit Special service $100 Filing forms for Military Marker (Non-government private cemetery) $100 Concrete base required for Bronze Military Marker $250 Prepare and submit forms for National Cemetery (Includes Flag Case) $150 Documentation/Consular Fee forwarding remains outside U.S. $495 Assisting with Published Obituary $100 Page 1
3 Preparation/Care of Deceased GENERAL PRICE LIST Embalming Except in certain cases embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. $400 Other Preparation/Care of Deceased Dressing, Cosmetology and Casketing Remains $250 Sanitary Care and Handling of Unembalmed remains $300 Repair of Remains Following Autopsy, or Anatomical Donation $300 Additional Restoration (per hour) $150 Pacemaker removal and disposal $100 Refrigeration & Storage of Remains (per day) $75 Use of Facilities, Equipment and Staff Our services include: Setup of facility for service, handling flowers, coordination & supervision of service and use of necessary equipment. Funeral Service at the Funeral Home or Other Facility -3 hrs. $450 Memorial Service at the Funeral Home or Other Facility -3 hrs. $450 Viewing or Visitation at the Funeral Home or Other Facility -3 hrs. $450 Graveside or Committal Service $450 Additional Hourly for Services > 3hrs $150 Facility Usage for Previewing / Cosmetology Coordination $195 Weekend, Holiday, After Hours Service Surcharge (Ending later than 5PM) $295 Reception Services-2 hrs. 4 GENERAL PRICE LIST Automotive Equipment All transportation listed is quoted at 30 miles or less from MVFH Mileage in excess of 30 miles will be at a rate of $2.00 per mile / Each Vehicle This mileage fee also applies to more than one attempt at acquiring required Physician and County authorizations. Adult Caskets $595 to $5,800 Alternative Containers - Cardboard $95 to $250 Rental Wood Casket $825 Special Charges Forwarding of Remains to another Funeral Home Includes basic services, transportation of remains from the place of death, embalming, obtaining permit, coordinating with receiving funeral home. Our charges exclude: fee for other preparation of the remains, shipping charge, shipping container, fees that may be required and transportation to common carrier. A $ credit will be applied when a casket at $l,l00.00 or more is purchased from MVFH casket price list. $1,895 Receiving Remains from another Funeral Home Includes basic services, transportation from Ontario Airport, custody and care of remains for up to 5 days, obtaining a local permit, transporting remains to local cemetery or crematory. This service does not include visitation, ceremonies or preparation of remains. $1,495 Page 4
6 Cremation GENERAL PRICE LIST A cremation container is required by the crematory. If you arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are cardboard containers. Our charge for direct cremation (without ceremony) includes: basic services, transfer of remains from place of death (within 30 miles) during normal business hours, transportation requiring one person, transportation to crematory, care of remains, necessary authorizations, 5 days of refrigeration, and the crematory fee. 7 GENERAL PRICE LIST Packaged Services Packaged Service Pricing is designed to ease the decision making process. The Majority of families we serve select their service from the Packaged Service Pricing. The packaged prices are greatly discounted and we cannot offer credit for deletions. Additions are acceptable. Services over 30 mile radius, will incur additional mileage charge. Traditional Service Packages do not include: Merchandise, Cash Advance Items, Sales Tax or Limousines. These packages are available only when a casket or urn is selected from MVFH. A.
