Advantage-Hennepin County Medical Center/NorthPoint is an integrated network of primary care, specialty centers, hospital, emergency care and convenience care that provides a high quality, yet affordable option for you and your family. Now that you re part of our network, this information will provide you with the details you ll need to understand the doctors, specialists and services in your Advantage network, out-of-pocket costs, referrals and other important information.

As a member of the Advantage-Hennepin County Medical Center/NorthPoint network, you have access to the entire Hennepin County Medical Center and NorthPoint Health and Wellness healthcare systems, including clinics throughout Hennepin County. We will be your partner to help you maximize your health and wellness by focusing on prevention, working with you to coordinate your care, and giving you current information to help you make informed choices. In addition to our primary care and specialty offerings, you have access to integrative health services at our downtown location and in many of our neighborhood clinics. 4 Member benefits. One phone call provides Concierge services. We provide a single phone number and personal resource for you to call to schedule appointments, answer questions about your network, assist with out-of-network referrals, and provide additional resources and information. The Concierge is available Monday-Friday, 7:30 a.m. 9 p.m. and Saturday Sunday 8:30 a.m. 5 p.m. Local Toll free No out-of-pocket costs for clinic visits at Hennepin County Medical Center/NorthPoint. Your outpatient clinic and urgent care visits at Hennepin County Medical Center and NorthPoint are 100 percent covered, which means there are no out-of-pocket costs to you or your covered family members for most outpatient services. The network provides for the first three office visits for in-network services at no cost. Remember, it s your responsibility to keep track of the three free visits throughout the year for yourself and your covered family members. If you are unsure how many of your free office visits you have used, contact PreferredOne customer service at Be Well Clinic offers expanded options and a pharmacy. The Be Well Clinic in the 701 Building provides access to primary care, an onsite pharmacy, and integrative healthcare services, including chiropractic and acupuncture for you and your covered adult family members. The clinic is connected by skyway to the Government Center, and features six exam rooms. Staff includes primary care providers, chiropractors, acupuncturists, and a pharmacist. The clinic offers expanded hours to meet the needs of Hennepin County employees. Call the Concierge line at to make an appointment. Free MDLive and E-Visits are available every day th Avenue South Online medical advice and treatment from a Hennepin County Medical Center provider is available 7 days a week for non-urgent conditions such as cough, diarrhea, heartburn, red eye, and more. Hennepin County Medical Center nurse practitioners respond to your health concerns online Monday-Friday between 9 a.m. 5:30 p.m. and weekends 10 a.m. 5:30 p.m. There is no co-pay for this service for you and your covered family members as part of the Advantage-Hennepin County Medical Center/NorthPoint network. If you already use MyChart, log in and go to the Messaging tab, and select Get Medical Advice. If you don t already use MyChart, it is easy to sign up by going to HCMC.org and selecting the MyChart home page and following the simple instructions. Members also have access to telephone or secure video consultations with board-certified doctors to treat non-emergency medical conditions through MDLive.

5 MyChart online access for your convenience. You have secure, direct online access to your health information. Your lab results, appointment information, medications, immunizations, and more are all available to you at your desktop or mobile device through our convenient MyChart app. You can be in communication with your doctor s office, renew prescriptions, send messages, and schedule appointments all online. Student and travel coverage. Students and individuals traveling outside the coverage area still have access to in-network benefits. Access to MultiPlan PHCS Healthy Directions Network Approximately 900,000 contracted providers across the country available at the in-network level Visit or call Free parking downtown and neighborhood clinics. Parking is free at HCMC clinics, NorthPoint Health & Wellness, and downtown in two parking ramps for Hennepin County employees, retirees, and their immediate family members when accessing HCMC services. Parking is validated in both the HCMC Ramp (615 S. 6th Street) and the Hospital Ramp (8th and Chicago), which are both connected to the hospital campus by skyway or tunnel. Ask for a free parking validation ticket when you check in for your appointment. Resources. There are many resources available to you and your covered family members with the Advantage-Hennepin County Medical Center/NorthPoint network. The following services and benefits are available at no cost. Online health and wellness resources. You can access the patient education health portal to help you make informed, smart decisions about leading a healthier lifestyle. Many resources are available, such as symptom and drug interaction tools that can help you identify symptoms of common conditions and manage medications effectively and safely. Visit to learn more. Nurse line. Members are able to speak with a specialty-trained nurse who will talk to you about your symptoms or medical concerns and provide a recommendation for care, including home care instructions, clinic visit, urgent care, or emergency department. 6 What happens next? 1. After you enroll you will receive a new member packet and a call from the Advantage-Hennepin County Medical Center/NorthPoint network concierge service welcoming you and assisting you with using the network. 2. Visit to learn more about your network and what s available to you. Use this site to find important resources including forms, links to the online provider search tool and more information about the network. 3. Establishing a primary care relationship is an important step to managing your wellness. Refer to the map and locations list in this Member Guide to see your primary care options at Hennepin County Medical Center and NorthPoint Health & Wellness or visit to see your options. 4. If you haven t already, sign the consent form included in your new member packet and return it using the enclosed envelope. If you do not receive a form, you may be asked to sign one at your first clinic visit. 5. Schedule a wellness visit. Call the Concierge line at to schedule your appointment.
