Des Plaines - Taxi Subsidy Program Hours: Monday- Sunday: 24 hours per day can access taxis Cost: Each voucher has a $3.00 value. Only one ticket will be accepted for a one-way ride. Must be age 65 or older or be disabled. Des Plaines must be a start or end point. Elk Grove Township Dial-a-bus Hours: Monday Friday: 9:30 a.m. - 4:00 p.m. Cost: no cost but donations are accepted Must live in Elk Grove Township, be age 55 or over. Need an emergency card filed before you can use the service. Can accommodate mobility limited riders (those using walkers, wheelchairs, etc.). One registered caregiver allowed to ride with patient. Please let dispatcher know if you have a disability at the time you make reservation. Curb to curb service. Reservations required a minimum of one day advanced notice. The bus will take you anywhere within Elk Grove Township: to doctor offices plus Northwest Community Hospital. Must live in Elk Grove Township, be permanently disabled between the ages of 18 and older or seniors over 55. You can use this service for any reason, traveling inside and outside of the township. No lifts provided. Call or visit the website for an application. With the application please note that proof of residency, proof of age, and proof of disability completed by your doctor will be required. IL etsnw.org Hours: Monday -Saturday 8 a.m. 8 p.m. Cost: $12 donation for each round trip will be appreciated. An invoice will be sent the month after services are provided for the donation. Volunteers do not accept tips. Volunteers use their own cars to drive seniors (60+) to medical/dental appointments, wait for them and bring them home again, stopping at the pharmacy, if necessary, on the way home. Call for an application and it will be mailed to you, or visit the website and download and print it off. Once the application is received and processed, services will be available. Please allow at least one week for this processing to occur. Seniors must be able to get in & out of a car without physical assistance. Portable, foldable walkers & wheelchairs are acceptable. A week s notice is preferred. First Transit Hours: Monday through Friday: 8 a.m. to 5 p.m. This service is for Public Aid recipients going to medical appointments. First, call a transportation company to see if they can take you to your doctor s appointment. If you need help finding a transportation company, call First Transit at Second, call First Transit to get your trip approved. Have a pen and paper ready, along with your Medicaid ID#, and appointment information. Write down the reference number given. Third, again call the transportation company to give them your reference number from First Transit, and schedule your pick-up time. Harmony Home Care 316 W. Northwest Hwy. 3 This is a home health agency that provides door-to-door transportation. They have a shuttle bus with a wheelchair lift to take people to the doctor. Please call at least one week in advance to schedule a ride. Medicare does not cover. Northwest Community Hospital Courtesy Van Hours: Monday - Friday: 6:45 a.m. - 5:30 p.m. Cost: $5 per direction ($10 for round trip total) Provides transportation between home and the following locations: Northwest Community Hospital, Busse Center for Specialty Medicine, Kirchoff Building, Wellness Center, Buffalo Grove Fitness Center, Rolling Meadow Outpatient Rehab & Therapy, and Schaumburg Treatment Center. To schedule a ride call 24 hours in advance. For medical visits the van serves any age but must be going to the hospital for medical treatments, tests, etc. (not to visit patients or for doctor visits in the Busse Center for Specialty Medicine) and have no other means of transportation. Must live within boundaries of: Rt.22 (Half Day Rd) to the North, I-294 to the East, Route 59 to the West, and Thorndale, the Elgin/O Hare Expressway, or Irving Park Rd to the South. Can accommodate wheelchairs but the person must be able to maneuver wheelchair independently. One ambulatory companion may ride with a frail patient to provide assistance. Pace Northwest Cook County ADA Paratransit Services Hours: Monday through Friday - 6:00 a.m. 6:00 p.m. Saturday and Sunday - 8:00 a.m. 5:00 p.m. Cost: $3.00 each way Provides a small bus with lift for door-to-door service within the Northwest Suburban Cook County (Townships of Elk Grove, Schaumburg, Palatine, and Wheeling, Niles, and Northfield). To register call to request an application from RTA. You must be approved before using this service. When approved, you will receive an ID card to show to driver. To schedule a ride call , TTY Call the day before ride is needed, early in the morning (5:30 a.m. 9:00 a.m.) as route gets filled up on a first call, first served basis. Must be unable to use a regular fixed route due to a physical or mental disability and be within ¾ mile of a Pace fixed route. Registered personal care attendants may ride for free. Attendants may accompany free of charge, but should be signed up first. Palatine Township operates a minibus curb-to-curb service for seniors (60+) and/ or permanently handicapped individuals living within the Palatine Township boundaries. In addition, the minibus goes to Arlington Heights on Tuesdays for medical appointments. Reservations required 3-14 days in advance. First-time riders need to go to the Township office to fill out a registration application. Palatine Township Taxi Subsidy Program Hours: Monday- Sunday: 24 hours per day Cost: Taxi Cab Vouchers for seniors that are worth $4.00 off the metered fare. Anyone needing them can stop at the Township front office to fill out an application. 10 vouchers are provided at a time, allowed up to 10 per month maximum, and they have no expiration date. Schaumburg - Dial-a-Ride Transportation (DART) Hours: Monday Friday: 5:30 a.m. 9:00 p.m. Saturday: 8:30 a.m. - 5:00 p.m. Cost: $1.00 each way for age 65+, and disabled persons (ride passes: $9.50 for 10 rides) $2.00 for others, each way (ride passes: $19.00 for 10 rides) Curb to curb bus service for people living in Schaumburg and are open to the general public. Bus travels within the Village of Schaumburg Buses are equipped with lifts. Call 90 to 2 hours minutes in advance. Schaumburg - Taxi Subsidy Program Hours: Monday- Sunday: 24 hours per day Cost: Village subsidizes $2.70 off the metered rate per one way trip with coupon. Or $5.00 off for hospital and treatment center trips. Those who wish to participate must live in the village of Schaumburg and first be registered in the Benefit Access Program which is formerly Circuit Breaker ( ). 5 Schaumburg Township Seniors and Disabled Bus Hours: Monday - Friday: 8:30 a.m. 4:30 p.m. Cost: $1.00 ($15 ticket for 18 rides). Must live in Schaumburg Township and be over age 60 or ages 18 and older that are permanently disabled and travel within Schaumburg Township. Bus goes to Northwest Community Hospital 2 nd and 4 th Monday & Wednesday at 11:00 a.m. and back at 1:00 p.m. Buses are equipped with a lift. Reservations required with a minimum of 3 days advanced notice recommended. One registered caregiver allowed to ride with the patient at no charge. TRIPS Program - Inter-Township Medical Transportation Hours: Monday - Friday: 5 a.m. 9 p.m. Saturday: 7 a.m. 4 p.m. Cost: The fare is $5 per township boundary crossed, with a maximum of $10 each way. Some exceptions for certain hospitals are made, with a 10 set fee. TRIPS is a door to door medical transportation program that crosses township boundaries for seniors and persons with disabilities who live in Palatine, Wheeling, Elk Grove, Hanover or Schaumburg Townships. They will also travel to Barrington or Maine. Registration is necessary. Must be a senior over 60, disabled over 18 and signed off by a physician. Can get registration forms at the townships, by calling for it to be mailed, or through the website. It can take days for registration. Please make your appointments no earlier than 9:00 a.m. or later than 2:00 p.m. You must be finished by 3:30 p.m. to allow for a return ride. When the medical van schedule is filled, some riders choose to use the scheduled Dial-a-Bus service to travel to and from their medical appointments.
