8 Term Made Simple Available Riders Disability Income Rider: Pays a monthly benefit to the insured in the event the insured becomes totally disabled: Defined: Disability which keeps you from being able to perform the major duties of your regular occupation as a result of such injury or disease. Issue Ages: Monthly Benefit is 2% of the Face Amount, not to exceed $1,500 (if annual income is below $25,000, the benefit cannot exceed $900 per month) The Benefit Period Runs for 2 Years 60 Day Elimination Period and the Benefits are not Retroactive Ineligible Occupations listed in Agent Guide Rider terminates at age 60 (unless benefit period is in effect)
9 Term Made Simple Available Riders Critical Illness Rider: Provides a cash benefit to the insured upon the diagnosis of a covered critical illness: Available at 25%, 50% or 100% acceleration of death benefit Requires disclosure form no to be left with the applicant Issue Ages: Rider coverage expires at age 70 Covered Illnesses: - Heart Attack - Coronary Artery Bypass Graft (10% of DB) - Stroke - Invasive Cancer - Kidney Failure - Major Organ Transplant Surgery - Paralysis - Blindness - Terminal Illness - HIV contracted performing duties as professional healthcare worker
10 Term Made Simple Available Riders Waiver of Premium Disability Agreement: Waives the payment of each premium (including riders) in the event of total and permanent disability. Rider coverage expires at age 60 (unless rider is in effect). Issue Ages Waiver of Premium for Unemployment Rider Waives the payment of each premium (including riders) in the event the insured becomes unemployed (receiving state or federal unemployment benefits) while the policy is in force. Rider coverage expires at age 65 or at the end of the policy level premium paying period (unless rider is in effect). Issue Ages: 20 60
11 Term Made Simple Available Riders Children s Insurance Agreement: Provides $3,000 per unit (max 5 units) on each child until age 25. Convertible up to 5 times the initial coverage. Benefit expires at the earlier of primary insured s age 65, or the child s age 25. Issue ages: Children 15 Days 17 years Primary Insured Accidental Death Benefit: An additional benefit paid to the beneficiary if the insured dies as the result of an accident. Available up to $200,000 or 5 times the face amount of the policy, whichever is less. Benefit expires at age 65. Issue Ages 18-64
12 No-Cost Rider Terminal Illness Accelerated Benefit Rider: Insured can elect to receive up to 100% of the death benefit when diagnosed with a life expectancy of 24 months* or less. Automatically included with every policy issued (you do not need to apply for rider on the app) Remember to leave disclosure statement Form 9474 with the applicant at point of sale. * 12 months in CA, CT, DC, DE, FL, ND & SD
13 No-Cost Benefit Accelerated Benefits Rider - Confined Care*: Triggered if Insured becomes a Full Time, Permanent Resident in Nursing Home Insured can elect to receive a Fixed Monthly Payment equal to 2.5% of policy face amount After each payment is received, the remaining death benefit and the Base Premium are reduced accordingly Automatically included with every policy issued, where available* (you do not need to apply for rider on the app) Remember to leave disclosure statement Form 9675 with the applicant at point of sale * Not available in: CT, DC, IN, MA, NJ, VA or WA.
14 No-Cost Benefit Chronic Illness Accelerated Death Benefit Rider With this benefit a portion of your death benefit (up to 95%) can be accelerated early if an authorized Physician certifies that the proposed insured is chronically ill. After each payment is received, the remaining death benefit and the Base Premium are reduced accordingly Chronically Ill being defined as: 1) Unable to perform, w/o substantial assistance from another person, at least two ADL s for a period of at least 90 consecutive days due to loss of functional capacity; or 2) Requiring substantial supervision for a period of at least 90 consecutive days by another person to protect oneself from threats to health and safety due to Severe Cognitive Impairment Automatically included with every policy issued, where available (you do not need to apply for rider on the app) Remember to leave disclosure statement Form 3230 with the applicant at point of sale * Not available in: CT, DC or FL

18 PREFERRED UNDERWRITING GUIDELINES To be eligible for Preferred class, the proposed insured must answer NO to the following questions: Have you used tobacco or nicotine products in the past 36 months? Using the Height & Weight Table below, does your weight exceed the minimum or maximum weight corresponding to your height? In the past 10 years, have you taken medication to treat high blood pressure or an elevated cholesterol level?* In the past 10 years, have you had, been tested for, received treatment or been told by a medical professional that you have diabetes, cancer or cardiac disease (heart attack, myocardial infarct, angina, cardiac insuf ficiency, cerebral thrombosis or coronary artery disease)? Has more than one member of your family (father, mother, brother or sister) died before age 60 from breast, colon, intestinal or prostate cancer, or from cardiovascular disease (heart attack, myocardial infarct, angina, cardiac insufficiency, cerebral thrombosis or coronary artery disease)? In the past 10 years, have you been treated for alcohol abuse? In the past 10 years, have you been treated for drug abuse or used any drugs not prescribed to you? In the past 5 years, have you had more than 2 moving motor vehicle violations or any DUI/DWI convictions? In the past 5 years, have you been convicted of a felony or misdemeanor? *Note: These are guideline criteria. We may consider an exception to one of these guidelines (i.e., elevated blood pressure or cholesterol but not both) if the condition is under control and the applicant has no other impairments.

22 Term Made Simple: Telephone Interview (Required with every sale) If cannot be completed Point-of-Sale Indicate on Application that Interview has NOT been completed, and provide Applicant s Phone #, and a Best Time to Call Vendor (EMSI) will contact Applicant to complete Interview or schedule a time to complete Results of Interview Transmitted to Home Office electronically
23 Term Made Simple Underwriting Medical Information Bureau (MIB) Member Companies report conditions to MIB database during the underwriting process Our Company accesses the MIB database on all Term Made Simple applications MIB replies can trigger additional underwriting requirements if additional information can not be obtained (i.e. APS s, Exams, etc. )
