1 GREATER GEELONG: BUILDING OUR COMMUNITY THROUGH ENTERPRISE, OPPORTUNITY AND INNOVATION IN A QUALITY ENVIRONMENT POSITION TITLE: POSITION NUMBER: 2624 DIVISION: DEPARTMENT: CLASSIFICATION: Band 7 REPORTS TO: DIRECTLY MANAGES: INTERNAL LIAISONS: EXTERNAL LIAISONS: HACC Occupational Therapist Community Life Aged & Disability Services Senior HACC Planning & Innovation Officer N/A (provides clinical supervision to the Allied Health Assistants) Department staff DATE: March 2016 POSITION OBJECTIVES: Residents, specialist agencies and organisations, local health services, other local and state government departments This position will facilitate the development and delivery of the City of Greater Geelong Allied Health Assistance program, and ensure the program provides innovative, responsive community care options for aged and disabled residents. POSITION RESPONSIBILITIES: Key Responsibilities: 1. Develop and implement the Allied Health Assistance program in partnership with regional health services. 2. Provide clinical supervision to the City of Greater Geelong Allied Health Assistants in accordance with the Department of Health and Human Services Supervision and Delegation framework for Allied Health Assistants. 3. Implement a care coordination model of which shared care, case conferencing and client reablement is a focus. 4. Liaise with other service providers, government and non-government agencies and consult with extended family members/significant others as appropriate in the design and delivery of consumer care. 5. Facilitate integrated services and care arrangements via effective liaison with relevant service providers and coordinate effective care plan implementation with Council s contracted provider. 6. Consult with and share information with local community health services to ensure clients receive prompt occupational therapy intervention from the appropriate organization. 7. Establish and maintain effective relationships with external stakeholders. 8. Provide input into forward planning in organisational policy and service development 9. Adhere to all Council policies and procedures relevant to this position. 1

2 10. Carry out other duties as deemed reasonable and appropriate to the role as directed from time to time. Delegate tasks that are in the scope of the Allied Health Assistant as outlined in the Department of Health and Human services Supervision and Delegation Framework for Allied Health Assistants. The following general physical and functional requirements may apply to this position. Specific physical requirements will be attached if applicable. Manual handling tasks. Prolonged periods of inactivity eg. sitting at the computer. Regular keyboarding associated activities. Long / short distance travel between sites. Dealing with difficult clients and situations. Demanding deadlines. KEY SELECTION CRITERIA: Essential: 1. Qualified Occupational Therapist with substantial experience in the delivery of community based occupational therapy. 2. Sound understanding of the Department of Health and Human Services, Allied Health Supervision and Delegation Framework for Allied Health Assistants and experience in clinical supervision and delegation of allied health tasks. 3. Demonstrated knowledge and understanding of home based care, National Disability Insurance Services, Commonwealth Home Support and the Commonwealth Government s Aged Care reform. 4. Demonstrated experience in care coordination, goal directed care planning and capacity building approaches. 5. Experience in using web based technology programs in care coordination, client data base systems and Microsoft software applications. 6. Knowledge of program planning, development and innovation. 2

3 7. Demonstrated understanding of, and sensitivity to, the support needs of older people, people with disability and their carers, including the needs of people from diverse backgrounds. 8. Demonstrated ability to manage and prioritise a variety of operational tasks concurrently with minimal supervision. 9. Experience in building and maintaining networks within the Aged Care and Health Service field. Desirable: 10. Current AHPRA membership 11. Experience in the use of S2S E-referral systems. 12. Experience in working in a multidisciplinary team including the capacity to work across the Aged and Disability Department service delivery teams. ACCOUNTABILITY AND EXTENT OF AUTHORITY: The position is responsible for providing professional Allied Health services to clients through the supervision of Allied Health Assistants and maintaining accurate client records. The incumbent will be required to assist with the development of policies, procedure and strategies relating to service delivery in particular the Allied Health Assistant program. The position will be required to provide clinical supervision and monitor the clinical performance of Allied Health Assistants. The incumbent will delegate tasks, identify and facilitate Allied Health Assistant training and ensure that clinical practice is compliant with relevant legislation and the scope of practice of allied health practitioners. Accountable for creating and capturing accurate and complete records of the business activities related to this position, in accordance with approved policy and procedures. JUDGEMENT AND DECISION MAKING: The incumbent is responsible for decision making at the program/operational level, and is required to make decisions around sound clinical practice. The incumbent is required to exercise professional and independent clinical judgement in client care and developing positive working relationships with contractors, clients and staff. The incumbent is required to participate and contribute to the Home and Community Care reablement approach in program and policy development. SPECIALIST SKILLS AND KNOWLEDGE: Knowledge of the impact of an ageing population and an understanding of legal and ethical responsibilities of service provision for the Home and Community Care target group. Understanding of the local needs of older community members, people with disabilities and their carers and the Community Care environment. Sound expertise in assessment and service delivery within the philosophy of active service and re-ablement principles. Ability to participate in the development, implementation, monitoring and review of policies, procedures and strategies relating to Community Care Services. Ability to manage data management systems. Demonstrated ability to be able to be flexible, show initiative and inspire others in the development of the Allied Health program. MANAGEMENT SKILLS: Ability to motivate a team of staff and provide support to the development of the Allied Health program. 3

4 High level of stakeholder engagement skills. Ability to manage a variety of operational tasks. INTERPERSONAL SKILLS: Excellent communication and presentation skills. Demonstrated ability to relate effectively with a diverse range of people including the capacity to handle conflict, deal with sensitive matters, and resolve disputes. Demonstrated ability to be flexible, show initiative and inspire others in the development and delivery of service models. Ability to influence team members and gain cooperation from others. QUALIFICATIONS AND EXPERIENCE: Bachelor of Applied Science (Occupational Therapy) or equivalent Eligibility for membership to the Australian Association of Occupational Therapy Substantial clinical experience. Current Victorian Driver s Licence Current Police Check Current Working with Children Check Experience in the supervision of Allied Health Assistants ADDITIONAL INFORMATION: The incumbent may be required to work in different locations within the City of Greater Geelong local government area.
