What the service could do better The service is planning to improve the recording of its six monthly care plan review meetings. What the service has done since the last inspection The service continues to build its participation methods for service users and relatives. Conclusion The service works hard to give service users and family members chances to comment on the care home's services. 4 1 About the service we inspected Glenlivet Gardens Care home provides accommodation for up to 48 older people. The building is purpose built on two levels. There is an enclosed garden area to the rear. All bedrooms offer single accommodation with en-suite facilities. It is situated in a residential area of south Glasgow, near to public transport and community resources. The service is provided by JSL Care Limited. Before 1st April this service was registered with the Care Commission. On this date, the new scrutiny body, Social Care and Social Work Improvement Scotland (SCSWIS), took over the work of the Care Commission including the registration of services. This means that from 1st April 2011, this service continued its registration under the new body, SCSWIS, now known as the Care Inspectorate. 5 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection The evidence for this report was gathered by Drew Conlon and Sarah Gill, Inspectors. They visited the service on Tuesday 21st February from 9.30 a.m to 4.45 p.m. During the visit, they spoke with the depute manager, two staff nurses and three care assistants. They also spoke with seven service users and a number of family members who were visiting the home on the day of the inspection. Consideration was given to a range of policies and procedures and other related documentation including: Care plans. Care review records. Participation records. Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. Details of what we found are in Section 3: The inspection Inspection Focus Areas (IFAs) In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. 6 Fire safety issues Inspection report continued We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may consider what action to take. 7 What the service has done to meet any requirements we made at our last inspection The requirement The provider should improve their approach to managing complaints to meet statutory requirements to respond to complainants within 20 working days. A response in writing should go to a family after a complaint incident in August What the service did to meet the requirement The provider has amended their complaints timescale to the statutory number of days and a written response went to the complainant. The requirement is: Met The requirement The provider must give notice to the Care Inspectorate within 24 hours of any allegation of misconduct which warrants investigation, dismissal or disciplinary action. What the service did to meet the requirement During this present inspection, care home management were fully aware of the notice and timescale required. They confirmed there had been no such recent incidents requiring notification. The requirement is: Met The requirement The provider must demonstrate that the level of staffing is adequate to provide the assessed level of support to service users at all times. What the service did to meet the requirement The home has introduced use of the Isaacs and Neville dependency tool to measure service users care needs. This is being used to measure and meet present needs for staff cover and this will be regularly reviewed. The requirement is: Met Glenlivet Gardens Care Home, page 7 of 18

8 What the service has done to meet any recommendations we made at our last inspection Action plans were received to encompass two Recommendations arising from the last inspection report. Three Recommendations arising from complaints activity are covered in the body of the report above. The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We were satisfied with the way the service had completed this and the relevant information they had had given us for each of the headings we grade them under. Taking the views of people using the care service into account This service has up to forty eight service users at any one time. We spoke to seven service users during the course of our visit. 'It's alright in here, I get my paper everyday'. 'The staff look after me really well'. 'I like the trouble the staff go to arranging really good outings'. 'Residents have meetings they can go to and give their views'. Taking carers' views into account The views of relatives and family members were favourable. 'The staff could not be more attentive and always make me feel welcome as a visitor'. 'The kitchen staff are really good about my husband's meals'. 9 3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 4 - Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service strengths The home has a robust participation structure which includes meetings for service users and suitable separate arrangements for relatives. There is an effective system in place to ensure care plans are reviewed six monthly. Relatives interviewed said that their views were asked for in a number of different ways. There was evidence from care plans that active use is made of records about visit from doctors and other health professionals. There were also records of contacts where relatives were being informed and updated about their family members care. Areas for improvement The content in the written recording of six monthly reviews could be improved. This is the subject of Recommendation below. Recommendation with reference to Theme 1, Statement 1: The provider should improve the recording of its six monthly reviews. This is to ensure recording of all the topics covered at the meetings as well as the contribution of the different participants in the meeting and how any changes are to be actioned. 10 Statement 3 We ensure that service user's health and wellbeing needs are met. Service strengths Care plans showed a wide range of relevant assessments for such as nutrition, mobility and continence. Systems for management of continence ensure equipment such as pads, aprons and gloves are readily available so the needs of individual service users could be met. There was evidence of positive input from activity organisers and discussion with residents about their preferences. The home aims to promote service user stimulation and sense of well being from a programme of outings from the home. Several service users spoke of their enjoyment of trips to places like the theatre and the coast. Areas for improvement The service is continuing to audit moving and handling risk assessments to ensure safe practice. Where restraint measures are in place, the service should ensure review dates are in place and subsequently met. Restraint interventions could also be improved by more closely relating risk assessment to the system of physical checks and spacing measurements on items such as bedside rails. Recommendation with reference to Theme 1, Statement 3: The provider should ensure restraint procedures cover all methods of restraint and include risk assessments and review dates for these. This should be aligned to the system for regular physical checks and safety measurements for this equipment. 12 Quality Theme 3: Quality of Staffing Grade awarded for this theme: 4 - Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of staffing in the service. Service Strengths The service has its own quality assurance systems,including the use of questionnaires. These questionnaires invite responses on a wide range of issues including food and drink, staff attitudes, care plans and activity programmes. There is an active and engaged Relatives Group which recently invited the Care Inspectorate to make a presentation on its work and there was discussion about service improvement in general terms. Recruitment of staff includes questions formulated in discussion with service users. The service allies its face to face forums with service users and relatives to other mechanism such as suggestions boxes and informal contacts to maximize the opportunities for comment and opinion to be heard. Areas for improvement The service wants to further develop service user's involvement with prospective staff. 13 Statement 3 We have a professional, trained and motivated workforce which operates to National Care Standards, legislation and best practice. Service strengths Staff were aware of National Care Standards and the role of the Care Inspectorate. There was evidence from front line care staff that they felt supported in their work and that the overall ethos of the service was to do the best possible for service users. Staff presented as well motivated and knowledgeable about any service users for whom they were the assigned key worker. Staff were able to connect positive motivation to good communication between staff team members. There is an ongoing programme of SVQ training and staff were keen to improve their qualifications whenever there are opportunities. Areas for improvement Arrangements for staff supervision are being consolidated. The service is planning to train more staff in the Liverpool Care Pathways approach to palliative care. Action Plan Failure to submit an appropriate action plan within the required timescale, including any agreed extension, where requirements and recommendations have been made, will result in SCSWIS re-grading the Quality Statement within the Management and Leadership Theme as unsatisfactory (1).
