8 Why Should I Use HIE-Bridge? To be compliant with Meaningful Use Stage 1 & 2 To be compliant with Minnesota s 2015 EHR Interoperable Electronic Health Record Mandate To increase patient coordination of care between providers To increase patient confidentiality by eliminating Protected Health Information shared via FAX To quickly access crucial medical records for better treatment decisions To increase office efficiency (less time spent on the phone looking for clinical information from others)
9 Use Cases Memory care patient moves to Montevideo to live with her son. Patient has 5 years of clinical records in Duluth. Patient needs to be seen by your neurologist. Provider logs into HIE-BridgeConnect and (with patient consent) immediately has access to clinical records from hospitals and memory care facility in Duluth, as well as list of current medications patient is taking. Cancer patient has been treated by local oncologist. Patient goes to Arizona for winter and contacts office asking for clinical information sent to oncologist in Mesa. Local oncologist logs into their EHR, which automatically launches HIE-BridgeDirect and all needed information is instantly sent to provider across the country. Unresponsive patient presents in ED. Besides a drivers license, there is no other information on the patient. While ED team is working to stabilize patient, simultaneously HUC launches HIE-Bridge Connect to find that patient is allergic to many meds that would otherwise be given. Correct meds are now able to be given and patient is admitted to hospital.
