1 Job Description Job Title: Responsible to: Minimum Qualifications: Job Summary Liaises with: Clinical Unit Manager (In Patient Unit) Clinical Lead First Level Registration with recent management experience and post-basic experience in palliative care. (Minimum of 5 years) The post holder will provide clinical management of the Hospice Inpatient Unit. S/He will provide a visible, accessible and authoritative presence in order to ensure specialist palliative care is delivered through effective leadership, monitoring and maintenance of end of life care and professional standards. S/He will effectively and efficiently oversee the daily operations of the 10 bed Inpatient Unit. The post will be divided 60% management, 40% clinical. Clinical Lead St Rocco s Clinical Staff (multi professional team) Hospice Educator Hospice at Home Co-ordinator St Rocco s Hospice Non Clinical Staff NHS Clinical Staff Social Services Other independent Hospices Other individuals/organisations the clinical team liaise with to provide patient care, staff training and service improvement Key Strategic Responsibilities To support the Clinical Lead in the development and delivery of the strategic business plan objectives aligned to the strategy of the Hospice. Key Responsibilities Responsibility for the quality of the care delivered within the Inpatient Unit by the clinical nurse team and for demonstrating the quality standards applied. Responsibility for leading, managing and organising the inpatient services. Responsibility for the line management and development of the nursing team on the unit. Responsibility for ensuring collaborative and co-operative working with others to develop services provided for patients within the Inpatient Unit. Participate in the out of hours on call rota for nursing staff on the Inpatient Unit. Responsibility for ensuring staffing levels and skill mix are adequate to meet the dependency of the patients in line with the budget. 1

2 OPERATIONAL RESPONSIBILITIES Key Responsibilities and Tasks 1 Clinical 1.1 Responsibility for ensuring patient records are maintained and that documentation reflects care provided. 1.2 Responsibility for establishing and maintaining effective communication with patients and carers/relatives including sharing sensitive information. 1.3 Responsibility for ensuring patients and carers/relatives are involved in the planning and delivery of care. 1.4 Work collaboratively with other professionals and agencies to ensure patients needs are met especially in relation to discharge planning. 1.5 Act as a clinical expert and resource for others in relation to specialist palliative care and nursing issues. 1.6 Responsibility for ensuring the safe administration of medicines, infusions and blood transfusions as prescribed. 1.7 Recognise and respond appropriately to urgent and emergency situations. 1.8 Capture regular service feedback and develop action plans to ensure service development that meets patients needs 2 Managerial/Leadership 2.1 Responsibility for leading, managing and organising the inpatient services and for developing and amending policies and procedures, ensuring compliance with them on the Inpatient Unit to support the delivery of high quality care and compliance with statutory requirements, e.g. CQC 2.2 Responsibility for monitoring and evaluating patient flow and take responsibility for initiating improvements, including contributing to the maintenance of accepted bed occupancy and patient length of stay levels. 2.3 Responsibility for developing the services within the Inpatient Unit and implementing service improvement initiatives. 2.4 Responsibility for ensuring staffing levels and skill mix are adequate to meet the dependency of patients, within overall financial resources. 2.5 Responsibility for leading the recruitment and retention of staff within the Inpatient Unit. 2

3 2.6 Ensure compliance with statutory requirements, statutory notification and risk management. 2.7 Participate in clinical on call rotas as determined by the on call policy. 2.8 Manage incident reporting and be aware of responsibilities relating to Complaints Policy, celebrate and share success with the whole team. 2.9 To act on concerns reported by team members and service users in a timely manner To ensure safe keeping and administration of drugs in accordance with national standards and legislation Manage volunteers who are working on the IPU and liaise with the volunteer co-ordinator to develop or grow the volunteer workforce Foster good working relationships with the Medical Director and medical team, AHP Pharmacist and other key stakeholders. 3 Governance and Quality 3.1 Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of the same. 3.2 To work with the Clinical Lead and members of the Hospice Audit Group to develop and review audit programmes and action plans to continually monitor standards of care and lead working groups to demonstrate improvement of specific areas of care through audit, benchmarking, surveys and/or research. 3.3 To produce as required reports for the Clinical Lead in relation to progress/evidence against a specific element. 3.4 To manage verbal complaints in a professional manner in line with Hospice Complaints Policy. 3.5 To work with the Clinical Lead and others e.g., Facilities Manager, Health and Safety Lead, to ensure that effective emergency plans and procedures are in place and that staff are able to respond appropriately in emergency situations and work within the Health and Safety policy. 4 Education, Training and Development 4.1 To initiate and participate in orientation and induction programmes for new staff, teach students and initiate mentorship for new staff. 4.2 To take responsibility for ensuring that all new clinical staff on the Inpatient Unit complete their induction programme, working with the Hospice Educator and delegating such responsibility for aspects of induction as appropriate. 3

4 4.3 To work with the Hospice Educator to ensure compliance of all inpatient staff with mandatory training and personal development plans aligned to the business plan and the maintenance of records. 4.4 To support staff to access and participate in clinical supervision. 5 Professional Responsibilities 5.1 Ensure appropriate professional registration is up to date at all times and produce the appropriate supporting documentation on request. 5.2 To maintain professional awareness of current developments in clinical care and evidence based practice in order to remain an effective practitioner. 5.3 Undertake continual personal development needs to maintain professional registration. 5.4 Always act in accordance with the NMC Code of conduct. 5.5 Participate in clinical supervision. 6 Staff Management 6.1 Responsible for formally appraising nursing and associated staff and formulating a development plan beneficial to the needs of the unit and the individual. 6.2 To be responsible for the direct and indirect management of individuals and resources including managing rosters, sickness, absence, compassionate leave, study leave and return to work interviews 6.3 To take a leading role in performance management and the development of direct reports. 6.4 To ensure development of staff is in line with the team, department and organisational objectives. 6.5 Nurture innovation and support new ways of working for the benefit of patient care. 7 Financial Management 7.1 Management of unit services in line with organisational and departmental policy and budgets. 7.2 To supervise stock levels and supplies and to ensure unit resources are utilised efficiently and cost effectively. 4

5 8 Communications and Working Relationships 8.1 Develop and sustain good working relationships with allied health professionals and external agencies. 8.2 Participate and support Hospice Income Generation Lead in marketing Hospice services and celebrating success of clinical services. 8.3 Ensure satisfactory channels of communication are maintained, promoting a transparent, impartial and honest culture amongst all disciplines. 8.4 To act as a core member of the Multi-Disciplinary Team, ensuring that they have up to date knowledge of the patients condition and contribute to the discussion to determine with other members of the MDT future plans which promote patient centred care. 8.5 To have responsibility for ensuring that plans for future care are shared with staff who have responsibility for delivering care. 8.6 To attend 67% of MDT meetings and liaise with the individual named in the MDT membership to attend in their absence. 8.7 As a core member of MDT to attend one network audit meeting a year and to have responsibility for sharing this information within the wider clinical team within the Hospice.. 9 Information Systems 9.1 To ensure safe and secure management of information within the department and to manage compliance of other staff with information management policies in relation to both paper and electronic records. 10 Responsibility for Patient/Client Care 10.1 Ensure patient care is of the highest standard, is evidence based and significant advances are implemented early Advise and participate as appropriate in multi-disciplinary assessment and care planning and liaise with external agencies to facilitate safe and effective discharges within the relevant statutory frameworks. 11 Equality and Diversity 11.1 Act in ways that support equality and diversity and work within the spirit and detail of legislation including the Equality Act 2010 paying particular attention to how this impacts on patients and relatives and carers. 5

6 12 Health and Safety 12.1 All employees are responsible for taking reasonable care for both their own health and safety and also the health and safety of colleagues. Staff are responsible for ensuring that they do not intentionally misuse or interfere with anything provided in the interests of health, safety or welfare e.g. misuse of equipment Responsibility for ensuring patient equipment is in good working order and that staff are trained in its use. Report any faulty equipment and remove from use Follow all Hospice infection control procedures when dealing with patients, relatives/carers and that the ward team and ward volunteers are informed of any infection control measures required by individual patients. Safeguarding It is the responsibility of the post holder to be aware of and follow the legislation and guidance regarding Safeguarding Adults and Children as stated in the Hospice Safeguarding Policy. This applies to all staff regardless of which member of the family is the primary client. The post holder is responsible for ensuring they receive the appropriate level of safeguarding training according to their role.
