4 Drug and Alcohol Assessments and Treatment Resources Reach For Youth, Inc. Perry Township Office The Alcohol and Other Drugs program is for any youth who abuses alcohol and/or other drugs, has a family member who abuses alcohol and/or other drugs, or is referred by the school or Juvenile court for an alcohol and/or other drug offense. A formal referral is not necessary and family, friends, churches or agencies may refer anyone age 7-17 into the program. Sliding fee scale, co-pays range from $5 & up. Medicaid and some insurance accepted for payment. Fairbanks 8102 Clearvista Parkway Indianapolis, IN Phone: Services include: free drug and alcohol assessments, inpatient and outpatient therapy. Ala-Teen and Al-Anon Programs -Offers a fellowship of relatives and friends of persons with addiction to share their experience, strength and hope. Health Dept-Nursing Division Offer dental care, physicals and physicians visits for sick children. Also free immunizations for infants, children and adolescents. Access Johnson County Offers community wide transportation to jobs, services, community resources and appointments. Fees vary, call for information and to arrange pick up and return times. Medi-Cab or call Hoosier Healthwise directly for more information Offers non-emergency transportation to physician appointments. Accepts Medicaid, private insurance and personal pay.
