2 2 A. Educational & Other Technical Qualifications, Age for the various posts:- Group C Posts (1) Junior Medical Record Technician: Essential :- (a) (i) Matriculation or equivalent qualification. (ii) Should have attended at least 3 months training course for Medical Record Technician in a recognized institution Or should have at least 6 months experience of working in the record registration and statistical section of a hospital especially in coding and indexing work. (b) Age not exceeding 27 years. Relaxable upto 37 years for Govt. servants and (2 ) Staff Nurse: Essential :- (a) Matriculation, Registered Nurse and Midwife or equivalent qualification of the Male Nurse. (b) Age not exceeding 37 years. (3) Pharmacist/Ayurvedic Pharmacist: Essential :- (a) Degree in Pharmacy OR Matriculation or equivalent qualification from a recognized Board and qualified and registered Compounder/Pharmacist. (b) Age not exceeding 27 years. Relaxable upto 37 years for Govt. servants and (4) Junior Radiographer: Essential :- (a) Matriculation or equivalent from a recognized Board. Diploma or certificate in Radiography of a recognized institute. (b) Age years. (5) E.C.G. Technician: Essential :- (a) (i) Science Graduate preferably with one year experience of handling E.C.G. Machine. (ii) Matriculation or equivalent qualification from a recognized Board with one year experience of handling E.C.G. Machine. (6) Operation Theatre Assistant: Essential :- (a) Matriculation or equivalent from a recognized Board. OT Assistant Training or one year experience in the field. (b) Age not exceeding 27 years. 3 3 (7) Laboratory Assistant: Essential :- (a) Matriculation or equivalent qualification from a recognized Board with Diploma in Medical Laboratory Technology. (b) Age not exceeding 27 years. Relaxable upto 37 years for Govt. servants and (8) Physiotherapist : Essential :- (a) 1) Inter Science or Pre-medical. 2) Three years degree or diploma training course in Physiotherapy from a recognized institution. 3) Three months internship in the field. (9) Radiographer: Essential :- (a) Matriculation or equivalent qualification from a recognized Board with Diploma (2 years course) in Radiography with one year experience. (10) Optometrist/Refractionist: Essential :- (a) Matriculation or Higher Secondary with Diploma in Optometry/Orthoptics. (11) Audiometrist: Essential :- (a) 1) B.Sc. Speech & Hearing or equivalent from a recognized University OR 2) Senior School Certificate with DCD (Diploma in Communication Disorders) with 2 years experience in Audiometry. 3) 2 years experience in handling Audiometer and hearing aids. (b) Age years. Relaxable upto 35 years for Govt. servants and employees of ESI Corporation. (12) Dental Technician: Essential :- (a) Matriculation or equivalent from a recognized Board with 2 years experience in Prosthetic laboratory work and Registered Dental Hygienist. (b) Age years.

4 4 (13) Medico Social worker: Essential :- (a) Degree/Diploma in social work from a recognized institution preferably with experience in Family Planning/Social work/ Health Education/Teaching. (b) Age not exceeding 37 years. (14) Boiler Attendant: Essential :- (a) (1) Matriculation. (2) Boiler Attendant certificate class II from the Delhi Administration or any other State. Desirable Experience in working in disinfectors and electorade boiler or similar electrical equipment. Group D Posts (1) Nursing Orderly/Stretcher Bearer/Attendant: Essential :- (a) Middle Standard Elementary knowledge of first aid. One year experience in handling or dressing wounds for Nursing Orderly. One year experience in hospital work for Stretcher Bearer and Attendant. (b) Age between years. Relaxable upto 37 years for Govt. servants and Relaxation in age: (a) 05 years in case of SC/ST candidates. (b) 03 years for other backward classes recognized by the Govt. of India. (c) In case of Ex- Servicemen, Service rendered in the Armed forces plus 03 years. (d) In case of Physically Handicapped, 10 years. B. Fee & Mode of Payment:- Posts in the Pay Scale (ranging from) Exam fee+ Postal Charges for General and OBC candidates Rs. 2550/- to 4500/- Rs =75/- Rs. 4500/- to 5500/- Rs =125/- Rs. 5501/- to 6500/- Rs =225/-

5 5 a) Candidates belonging to SC/ST/Ex-Servicemen/PH shall submit DD for Rs. 25/- (Rupees twenty five only) towards postal charges. Departmental candidates are exempted from payment subject to enclosing certificate of service from the ESI Hospital/Dispensary. b) The fee is required to be paid by all candidates through Demand Draft issued in favour of Medical Superintendent, ESI Corporation Model Hospital, Nanda Nagar, Indore, payable at State Bank of India, Indore. c) Demand Draft drawn on or after the date of publication of advertisement will be accepted. Payment in other form will not be accepted. C. Examination Centre-Written examination ( Objective type) and interview shall be held in Indore. D. Instructions to candidates - How to apply 1) The age will be calculated as on 31/10/2008 which is the last date for receipt of application. 2) The application in the prescribed proforma given below duly filled in English or Hindi language, duly supported with clear/legible attested copies of the relevant certificates and marks statements ( in English or Hindi) should be sent by registered post/ speed post suitably superscribed on the envelope Application for the post of.. to the following address: Medical Superintendent, ESI Corporation Model Hospital, Nanda Nagar, Indore (M.P.) 3) Candidates have to fill in the application form in their own handwriting with blue or black ball point pen. 4) Original documents/ certificates should not be sent with application. 5) A candidate should submit only one application in one envelope. Candidature of those candidates who submit more than one application in one envelope is liable to be rejected. 6) The initial place of posting will be Indore (M.P.). However, incumbents are liable to be transferred anywhere in India, as and when required. 7) ESI Corporation reserves the right to call candidates for written examination or interview or both.

6 6 8) Application not in prescribed proforma as appended below Or incomplete (without photograph also) applications Or unsigned applications Or applications received after the last date of the receipt of applications Or without required enclosures are liable to be rejected and no reason for rejection will be communicated. 9) Applications not in accordance with the above-mentioned instructions are liable to be rejected. 10)Candidates already in Govt. Service must specifically mention the details of employment and should submit their application through proper channel. However, they may send an advance copy of their application along with Demand Draft, other certificates and testimonial so as to reach this office on or before the stipulated last date for receipt of application. 11) Mere submission of application does not confer any right of the candidates to be called for examination or interview. 12) No interim correspondence/enquiry will be entertained. 13) The last date for receipt of applications is 31 st October, ESI Corporation will not be responsible for postal delays. 14) No TA shall be paid to any candidates including SC/ST candidates for appearing in written examination. TA shall be paid to SC/ST candidates for interview only as per Govt. of India orders. E. Mode of Selection: Written examination (Objective type) or interview or both. F. Documents required to be attached with the application:- 1. Attested photocopies of certificates in support of (a) Date of Birth (b) Educational and Technical Qualification certificates (along with marks statements) (c) Experience certificate (d) Caste Certificate (e) Discharge Certificate in case of Ex-Servicemen (f) Disability Certificate in case of Physically Handicapped candidates. 2. Two recent pass-port size photographs duly attested by Gazetted Officer. One photo should be firmly pasted (not stapled or pinned) in the space provided in the application form. 3. Demand Draft: a) The Demand Draft should not be tagged or stapled, but should be pinned or clipped at the top of the application form. b) Fee once paid will not be refunded under any circumstances. c) Candidate must write his/her name and address on the back of the Demand Draft.
