1 OAK HILL SUPPORTIVE EMPLOYMENT JOB DESCRIPTION JOB COACH GENERAL PURPOSE The Job Coach Oak Hill (OH) develops and maintains the vocational abilities of individuals with disabilities by creating and maintaining a safe environment for program participants; completing all programming activities specified in Overall Plan of Service/Individual Educational Plan; supervising program participants and performing all other duties specified in that program, in keeping with Oak Hill s mission. SUPERVISORY AND OTHER CONTACTS The Job Coach works under the general supervision of the Supported Employment Manager or Day Program Manager, and the Program Supervisor. This individual does not exercise supervision of employees. Job Coaches on the job will come in contact with any individuals who fall within the range of programs, including parents and guardians, medical personnel, other staff, employers, and other people in the community. DUTIES AND RESPONSIBILITIES 1. Becomes familiar with and implements Oak Hill s policies and procedures designed to fulfill specified goals and objectives of vocational programs for individuals with disabilities by providing guidance, instruction and assistance. 2. Promotes self-reliance by working directly with program participants/clients on assigned tasks, decreasing direct assistance in proportion to client's demonstrated increase in proficiency. 3. Maintains the comfort and well-being of participants, and promotes the development and maintenance of self-help and daily living skills by assisting them in the completion of tasks related to personal hygiene and other related activities, according to individual need.

2 4. Controls/manages/modifies participants' behavior by implementing individual behavior management programs, as specified in OPS/IEP, recording relevant observations as required. 5. Ensures continuation of programs by implementing the day components of individual programs, following relevant policies and procedures. 6. Ensures the safety of program participants and the completion of assignments by overseeing and monitoring their activities at all times. 7. Maintains the health and well-being of program participants by promoting/encouraging self-administration of medication; administering medication; documenting medication administration, and any errors which may occur, as specified in Oak Hill s policies and procedures. 8. Protects program participants by applying Emergency First Aid and CPR procedures when necessary, and implementing other emergency procedures such as evacuation in the event of a fire, heating failure or other situation; using initiative and good judgment in emergencies where policies and procedures may not be specified. 9. Implements programs by transporting program participants to and from work, volunteer activities and community trips. 10. Assists program participants by lifting and transferring as needed, using a back brace and any other applicable equipment to maintain safety. 12. Provides guidance and support, and facilitates learning and task completion by assisting participants in all aspects of vocational and volunteer activities. 13. Promotes the interest of program participants and OH regarding vocational and community contacts by advocating for participants, and acting as liaison on job sites; interacting with participants, employers, supervisors, co-workers, parents/guardians and others in the community in a professional manner. 14. Ensures safe and hygienic conditions in participants' work areas by maintaining a clean, orderly and hazard-free environment. 15. Facilitates the implementation of programs by preparing materials for participant related activities, as required. 16. Ensures that tasks are completed at the required standard, within required time frames, and ensures that OH s obligation to employers is fulfilled by observing quality control procedures, monitoring the completion of work assignments, and completing tasks as needed, when program participants are unable to do so.

3 17. Develops, enhances and maintains participants self-esteem; teaches them by interacting in a professional and age appropriate manner, consistent with the idea of role modeling. 18. Provides information for program maintenance and evaluation, to meet OH policies, procedures, licensing and certification requirements by collecting, charting and recording data. 19. Obtains information regarding new developments specific to individual participants, groups, or the field in general; meetings training guidelines, certification requirements, and ensures continued professional development by attending general and group specific team meetings and training programs. 20. Facilitates evaluation and intervention, brings relevant concerns and information regarding participants and vocational tasks to the attention of the supervisor by making notes in appropriate logs and discussing issues as they arise 21. Handles information regarding program participants with discretion, respecting confidentiality at all times. 22. Ensures consistency in programs during transition by assisting in the orientation of new staff. 23. Contributes to team effort by completing related tasks as needed. QUALIFICATION REQUIREMENTS Experience, and/or training which provides the knowledge, abilities and skills listed below. Knowledge, Ability, and Skills Knowledge of the needs of individuals with disabilities; knowledge and ability to maintain a safe environment; ability to train program participants in self-care habits, personal hygiene and proper social conduct. Knowledge and ability to apply behavior management skills; knowledge and ability to recognize and respond appropriately to physically abusive behavior (to self, staff, and others), and to accidents related to bodily functions. Ability to exercise common sense and good judgment at all times; ability to act quickly and appropriately in crisis or emergency situations; ability to understand and carry out oral or written instructions; ability to organize and maintain accurate records; ability to maintain a safe and secure environment.

4 Ability to lift and transfer individuals with physical disabilities using safe and appropriate methods; ability to work with minimal supervision. Ability to successfully complete training leading to certification for Emergency First Aid, Cardiac Pulmonary Resuscitation (CPR), Physical Management Training (PMT), Passenger Assistance Training (PAT) and all other training related to resident care. Good interpersonal skills. Ability to meet the requirements for Public Service Licensing. PHYSICAL DEMANDS

5 Persons in this job must possess the physical and emotional health needed for effective and efficient completion of duties. Persons in this job are likely to be required to perform lifting and transferring activities and may be required to restrain residents/program participants; may be expose to risk of injury from abusive/assaultive residents. Using arms in upward, inward, outward, and down ward motions in bent and extended positions. 2. Bending knees while sustaining body weight. 3. Maintaining standing balance during transfer in pivot and with knees bent. 4. Bending at waist while standing or kneeling. 5. Maintaining firm hand grasp. 6. Rising from crouch. 7. Turning head to side and lower chin toward chest. 8. Using arms in bent and extended positions to reach tiedowns and safety restraints, push/pull wheelchair, raise wheelchair over curbing/ramp. 9. Using leg, ankle and foot to apply leverage to wheelchair rear frame bar. 10. Using arms and hand to manually pump hydraulic lift. Pounds required to handle while providing physical assistance to residents including handling occupied wheel chairs:
