2 A Letter from the President DAVID J. HARTMAN, President We Value Your Support and Your Partnership Dear Friends: As summer has come to a close, we happily look back and reflect on some of the amazing accomplishments and milestones achieved this season. Our annual Transitional Care Symposium was a tremendous success, and was a sell out for the 3rd year in a row. We were extremely excited to have Dr. Joseph Ouslander join us for 2 days for speaking engagements and educational sessions on the INTERACT Program and Fall and Continence Management Program with our Administrators and DON s. Dr. Ouslander also joined us in a Post Acute Network meeting with Advocate Healthcare. In addition, he was our keynote speaker for the Transitional Care Symposium, speaking on the INTERACT Program. Thank you to all who supported and attended this incredible event. The Transitional Care Symposium also served as our official launch of the Symphony Post Acute Network. This new network is a representation of all our member facilities and is based around our belief of the four key groups of people we bring together - Residents, Staff, Community and Family. Summer also brought the very first Grand Re-Opening as a result of renovations that have been ongoing at a number of facilities. Symphony of Crestwood celebrated its Grand Re-Opening and Open House on August 1, This inaugural event for the network showcased the new amenities, clinical programming and renewed brand at Symphony of Crestwood, located in the southwest suburbs of Chicago. This edition of the quarterly report will focus on the best practices and initiatives that our network members are collectively working on - the Rapid Response Program from Symphony of Mesa in Arizona, The Claremont of Hanover Park being the 1st in the Nation to receive Joint Commission certification for Rehabilitation and Advanced Care, the Shining Stars that have touched our hearts such as Jacob, Erica Riley, Randi, Claudia and the Seven Oaks Music & Memory Program. All of these and so much more are the testament as to why we do what we do and we are proud to share it all with you. David J. Hartman President, Symphony Post Acute Network Symphony of Crestwood celebrates its official Grand Re-Opening! August 1, 2013, was a very exciting day for Symphony of Crestwood. All the renovations, facility upgrades and new clinical programming were honored at the Grand Re- Opening and Community Open House. The event began with a ribbon cutting ceremony, lead by the Mayor of Crestwood himself, Lou Preska, pictured to the left with village trustees as well as Symphony s Chairman of the Board, President, CEO, COO, SVP and the facility Administrator. The event drew over 200 guests from nearby hospitals, businesses as well as physicians, nurses, case managers and many members of the community. 1 QUARTERLY REPORT FALL 2013

3 Spotlight On... Announcing the On August 29, 2013 the Symphony Post Acute Network was officially unveiled. With this new network, comes a new philosophy with a harmonious approach to post acute care. All NuCare clients are now members of the Symphony Post Acute Network. Together, this network of locations share the same goal to provide the best outcomes for our patients. Working in collaboration with local physicians, nurses and social workers, we are proud to offer the communities we serve the highest level of post acute care. Our Philosophy The four rings in our logo represent the important role these four groups play in our approach to providing the best possible care. Community Family At Symphony, we are proud to be a part of the communities we serve. Working with local physicians, nurses and therapists we have made it our goal to provide our communities with the finest healthcare services. At Symphony, we know that family plays an important role in our guests recovery process. We are dedicated to providing your loved one with the best possible recovery. Residents The care and comfort of our residents is our highest priority. To us, they are more than patients; they are our guests. That is why we do everything we possibly can to make them feel relaxed and at home. Staff Our Symphony staff is made up of well-trained medical professionals who live and work in the communities we serve. 5 Program Successes CareBridge Success Renaissance at 87th Street Success An 81 yr. old Male Patient had a 4 day hospital stay with CHF exacerbation, patient was discharged home directly with Home Health Care. On the home care nurse s 5th visit, she noticed a weight gain and shortness of breath. The patient could barely walk up stairs, and couldn t move around the house well. There could also have been medication non-compliance and diet non-compliance. Intervention - patient was admitted to SNF for IV Lasix, re-education on diet and medication compliance and physical therapy. Patient was discharged back home after 10 days with home health care. ER was avoided the entire 30 days post hospital stay. Aria Success A 76 yr old female patient post gall bladder surgery returned home with home health care and a caregiver. The caregiver called the home care team on day 2 saying the patient was complaining of pain and that she could no longer safely care for the patient and was taking her to the ER. The home care team directed the caregiver to Aria where IV antibiotics, pain medication and wound care incision site treatment were managed. The ER was avoided the entire 30 days post hospital stay. Forest Villa Success Forest Villa was able to work with Advocate Lutheran General Hospital on a patient that had recently been discharged home, per the patient s request. This patient was discharged with a new wound vac and was a new outpatient dialysis recipient. The patient was not managing well at home. Case Management at Lutheran General and the admissions team at Forest Villa were able to get the patient admitted immediately to the facility and avoided rehospitalization. The Rapid Response Program The Rapid Response Team is a highly specialized trained group of medical professionals consisting of Registered Nurses, Licensed Nurses, and Respiratory Therapists. The members on the Rapid Response team are ACLS (Advanced Cardiac Life Support) certified. The Rapid Response Team is deployed for a significant change of condition of a patient. Symphony of Mesa s Rapid Response Team has the capacity to capture vital data (such as, but not limited to EKG rhythms) allowing the Physicians to keep the patient at Symphony of Mesa safely. Symphony of Mesa s Rapid Response Team concentrates on educating staff about the distinctive characteristics of a particular diseases. The Rapid Response Team is able to aide in identifying the signs and symptoms the patient is experiencing and relay those indications to the Physician providing for a much clearer picture for the Physician. A six-month study proved that 70% of all patients that Rapid Response was deployed for stayed at Symphony of Mesa. With continuing education, cutting edge technology, and a facility that is willing to adapt to a higher acuity demand, Symphony of Mesa and its Rapid Response Team has made it possible for the road to recovery to occur with significantly fewer returns to the hospital. 6 Symphony s Shining Stars Bronzeville Park - Jacob Amgbey returned home to Ghana after more than 25 years, the last several of which have been spent in a skilled nursing facility. Staff at Bronzeville Park, where Amgbey has lived for the last 1½ years, have been planning to send him home since June and finally worked out the details. Joyce Molino, Director of Memory Care Programming at Bronzeville Park, eventually found a friend of a friend that the family and staff deemed trustworthy to bring Amgbey back to Ghana for the first time in 25 years. Molino worked with Amgbey s nephew to find the man who will accompany him on the trip. Amgbey has two sons in Ghana he plans to live with when he returns. The staff planned a small get-together at Bronzeville Park for family and friends to say goodbye to Jacob. They also had a stretch limo pick him up from Bronzeville Park and take him to O Hare Airport to catch his flight on Air Italia. At the gathering, friends and family gave Amgbey suitcases, sandals, clothes and other gifts for his trip. We miss everyone when they leave. Everyone should be home if it s an option, Molino said. Ghanaian Immigrant Finally Set to Return Home, With Help From Some Friends Molino helped raise $700 that Amgbey will be able to use as pocket change once he s home, and an anonymous donor paid the airfare for the trip. Geriatrics Saved His Life! A tribute to Erica Riley, LPN, a secret weapon who helps an older cancer patient who is currently residing in her skilled nursing facility on the south side of Chicago. As Erica tells it, He is one of the patient s that s dear to my heart. His appetite is extremely poor due to his diagnosis, but by sitting by his side, communicating with him, and encouraging him not to give up, it has made a tremendous change. The smallest acts of kindness can mean so much... He has now started to consume more food, his appetite has increased and he has gained weight. He has become a lot stronger. Which he has. We give him grilled cheese sandwiches all day long. He takes meticulous notes in his notebook, and he asks me questions each week. We have straightened out his finances, and he has decided to stay at our facility long term. We provide him transportation to get to his oncology appointments. And Erica makes sure everything is taken care of. A few weeks later, Mr. Moore returned to our Friday clinic to see the oncologist. I heard about it from my colleague, who came into the workroom, laughing and carrying on. Hey, Mr. Lessman is back. He looks fantastic! We re going to treat him! Hartford Foundation s 1st Prize in their Hero s in Long Term Care Essay Contest. Rockstar Randi Randi Wilsoncroft is a CNA at Springdale Village in Arizona. Randi sprung into action one day recently when a rehab resident stopped breathing. She administered CPR & the resident was revived and went to the hospital in stable condition. Saving lives is an amazing part of what the nursing staff does every day at long term care & rehab facilities. Randi, like many other healthcare professionals, is to be commended for her passion & caring for the residents. Congratulations to Seven Oaks Nursing & Rehabilitation Seven Oaks Nursing & Rehabilitation has been selected to participate in the Music & Memory Initiative. This is a Department of Health Services program (DHS) formed to provide 100 Wisconsin skilled nursing facilities the ability to bring enjoyment to their residents with Alzheimer s and other related dementias. The program is designed to bring renewed meaning and connection to the residents through personalized music. Live A Dream: Claudia s Story Claudia Greco was a resident in a skilled nursing facility in Arizona, thousands of miles away from her family located in Chicago s south suburbs. With the help of the executive director of the facility she was living in and the Arizona Health Care Foundation, Claudia and her family were granted financial support as a result of their application to Arizona s Live a Dream program. Claudia was able to move from Arizona to Symphony of Joliet. We welcome Claudia and her family to our Symphony family! A big thank you to Cory Fosco of Wound Rounds for his work with the Arizona Health Care Foundation and assistance in helping Claudia and her family with the travel! 7 Committed to our Communities & Excellent Care Back to School On August 23, Bronzeville Park helped the Bronzeville community kick off a terrific start to the 2013 school year. The residents, staff, along with members of the community enjoyed an afternoon full of fun in Bronzeville Park s parking lot. Amongst the enjoyment was a bouncy house, a dunk tank, carnival games with prizes, pony rides, a piñata full of candy, and a clown who performed magic tricks and made balloon animals. Even the activity director, Becky, joined in the fun and dressed up as Cupcake the Clown. Live entertainment included guests from the radio station V103, an additional DJ as well as performances from the Pebbles Foundation and the Chicago Boyz Acrobatic Team who are currently in the semifinals on the television show America s Got Talent. On top of all that fun, school supplies were distributed and free haircuts were given to young boys! County Care s Exclusive Skilled Nursing Facility Provider County Care is a Managed Care Medicaid program for adults. Members that qualify are eligible for certain health care services, such as skilled nursing facility rehabilitation and therapy.
