1 Toronto Catholic District School Board Catholic Education Centre 80 Sheppard Avenue East North York, Ontario M2N 6E8 Attention: Re: Ms. Kathleen Wilson Safety Officer, Accident Prevention, Occupational Health and Safety Department Mould Investigation and Remediation St. Patrick Catholic Secondary School, Rooms 153, 154, 154A, 154B and 156 Closet 49 Felstead Avenue, Toronto, Ontario Dear Ms. Wilson: Decommissioning Consulting Services Limited (DCS) was retained by the Toronto Catholic District School Board (the Board) to provide engineering assistance associated with the investigation and removal of mould-impacted materials from Rooms 153, 154, 154A, 154B and 156 Closet in St. Patrick Catholic Secondary School. 1.0 INTORDUCTION Mr. Wayne Kesteven of DCS attended the site on 12 August 2014 to perform a visual inspection for mould of readily accessible areas of the above-noted rooms located in the lower level of the school. Board staff, along with representatives from Biggs & Narciso Construction Services Inc. (B& N) were present at this time and pointed out rooms and locations that appeared to have been impacted by a water leak from the floor above (on going at the time of the inspection). 2.0 SUMMARY OF INVESTIGATION FINDINGS A summary water and/or suspect mould impacts observed during the inspection on 12 August 2104 is outlined in Table 2.1 below.

PATRICK CATHOLIC SECONDARY SCHOOL 12 AUGUST 2014 Location Room 153 Room 154 Room 154A Room 154B Comments The room contained furniture and fabric dividers. Water stained ceiling tiles observed. No suspect mould observed. Water and mould impacts observed along the lower east wall approx. 12 to 18 high Water impacted delaminated 12 x12 vinyl tile observed along the east wall. The water leak from the floor above was ongoing at this time. Water was being caught in plastic barrels. Some furniture and sundry items were present in the room. Water impacted ceiling tiles had been removed. The ceiling was open to the plenum and the concrete floor slab above. Piping in the plenum was exposed and appeared to be possible source of the water leak. Lighting fixtures were water impacted. No suspect mould observed in the upper ceiling plenum. No water damage or suspect mould impacts observed on the visible front surfaces of the lower cabinet work along the east wall of Room 154. Standing water, water impacts and staining observed on the vinyl floor tile. No suspect mould observed. The room was empty. Water and suspect mould impacts observed on the underside of the north bulkhead. No water or suspect mould impacts observed on the upper gypsum board ceiling. No water damage or suspect mould impacts were observed on the front surface of the upper painted gypsum board walls or the front surface of the vinyl covered foam padded walls units. Standing water, water impacts and staining observed on the vinyl floor tile. The room was empty. Water and suspect mould impacts observed on the underside of the north bulkhead. No water or suspect mould impacts observed on the upper ceiling tiles.
3 Page 3 Location Room 156 Closet Comments Water damage and suspect mould impacts were observed on the front surface of the lower east painted gypsum board wall. No water or suspect mould impacts observed on the vinyl floor tile. Musty mould-like odour was detected. The room contained numerous sundry items. Open to above. No water suspect mould impacts observed. Water staining observed on all walls. Suspect mould observed on the backside of the gypsum board at the lower east closet wall. Possible suspect mould observed on cement pipe fitting (possible ACM) No water or suspect mould impacts observed on concrete floor. Dust and debris observed on the floor. Photographs are provided in Attachment A. 3.0 RECOMMENDATIONS We recommend the following on the basis of the visual site inspection carried out by DCS on 12 August the source(s) of moisture should be rectified in order to prevent potential future mould growth; humidity levels in the subject area of the school be lowered. locations where water damage and mould growth have occurred should be removed using Level II remediation procedures in accordance to the Mould Guidelines for the Canadian Construction Industry, Standard Construction Document CCA , Canadian Construction Association, as a precautionary measure: 4.0 MOULD REMEDIAL WORK B&N were retained by the Board to complete the recommended mould remedial work. DCS was retained by the Board to supervise and ensure mould remedial work was undertaken using CCA Level II in accordance to the Mould Guidelines for the Canadian Construction Industry, Standard Construction Document CCA , Canadian Construction Association, as a precautionary measure.
4 Page 4 DCS staff was on site on 19 August to 2014 to perform a pre-contamination inspection of the work area setup. The work area setup was completed with no deficiencies noted. At this time B&N recommend a second decontamination chamber be erected in Room 154 for access purposes. This was completed the following day prior to the start of mould remedial work. A summary of the mould remedial work completed at the time of the final visual inspection on 19 August 2014 is outlined in Table 4.1 below. TABLE 4.1 SUMMARY OF MOULD REMEDIAL WORK ST, PATRICK CATHOLIC SECONDARY SCHOOL FINAL VISUAL INSPECTION 19 AUGUST 2014 Location Room 153 Room 154 Comments Furniture and sundry items in the rooms were HEPA vacuumed, damp wiped with a detergent solution and covered with polyethylene sheeting and store in the room or adjacent hallway. All surfaces inside the work area were HEPA vacuumed and damp wiped with a detergent solution after tear out was completed. Water stained ceiling tiles were removed bagged and disposed. Water and mould impacted gypsum board was removed bagged and disposed. All water and mould impacted metal and/or wood framing were wire brushed and/or sanded HEPA vacuumed and damp wiped with a detergent solution or removed if not practical to clean. Water impacted delaminated vinyl tiles were removed bagged and disposed. Floor surface was HEPA vacuumed and damp wiped with a detergent solution. Furniture and sundry items in the room were HEPA vacuumed, damp wiped with a detergent solution and covered with polyethylene sheeting and store in the room or adjacent hallway. All surfaces inside the work area were HEPA vacuumed and damp wiped with a detergent solution after tear out was completed. Water leak from the floor above continued to be caught in plastic barrels. Water stained ceiling tiles were removed bagged and disposed. Water and mould impacted gypsum board was removed bagged and disposed. All water and mould impacted metal and/or wood framing were wire brushed and/or sanded HEPA vacuumed and damp wiped with a detergent solution or removed if not practical to clean. Lower wood cabinet work along the west wall was removed and inspected. Cabinets with water and mould impacts not practical to clean were broken down bagged and disposed. Remaining cabinets were HEPA vacuumed damp wiped with a detergent solution covered

All surfaces inside the work area were HEPA vacuumed and damp wiped with a detergent solution after tear out was completed. Water and mould impacted gypsum board removed from north bulkhead was bagged and disposed. Metal framing was HEPA vacuumed and damp wiped with a detergent solution. Vinyl covered foam padded walls units removed from the west wall were inspected and found to have water and mould impacts along the bottom. These units were HEPA vacuumed and washed and damp wiped with a detergent solution. Water and mould impacted gypsum board along the lower west wall was removed bagged and disposed. All water and mould impacted metal and/or wood framing were wire brushed and/or sanded HEPA vacuumed and damp wiped with a detergent solution or removed if not practical to clean. Floor surface was HEPA vacuumed and damp wiped with a detergent solution. All surfaces inside the work area were HEPA vacuumed and damp wiped with a detergent solution after tear out was completed. Water and mould impacted gypsum board removed from north bulkhead was bagged and disposed. Metal framing was HEPA vacuumed and damp wiped with a detergent solution. Water and mould impacted gypsum board along the lower east and south wall was removed bagged and disposed. All water and mould impacted metal and/or wood framing were wire brushed and/or sanded HEPA vacuumed and damp wiped with a detergent solution or removed if not practical to clean. Floor surface was HEPA vacuumed and damp wiped with a detergent solution. All non porous sundry items in the closet were HEPA vacuumed and damp wiped with a detergent solution and stored in Room 156. Porous items were bagged and disposed. Open to above. No remedial work undertaken. Inside surfaces of painted concrete and concrete block walls were HEPA vacuumed and damp wiped with a detergent solution. Minor suspect mould observed on the backside of the gypsum board at the lower east closet wall was removed and by HEPA vacuum and damp wiping. Dust and debris observed on the floor was removed by HEPA vacuuming and mopping with a detergent solution..
6 Page 6 Photographs are provided in Attachment A. Mr. Dwayne Kellyman of DCS revisited the site on 20 August 2014 to perform a final visual inspection. The mould remedial work had been completed with no deficiencies observed with the exception of a minor amount of dark water staining on a wood stud and heavily rusted metal sill plate in Rom 153 that DCS staff request be removed as a precautionary measure. 5.0 USE AND LIMITATIONS OF THIS REMEDIATION VERIFICATION REPORT This verification report, prepared for the Toronto Catholic District School Board, does not provide certification or warranty, expressed or implied, that the remediation verification program uncovered all mould locations at the site. The material in the report reflects DCS best judgement in light of the information available at the time of completion of remediation, 20 August Mould growth can occur following the date of remediation. Any use which a third party makes of the report of parts thereof, or reliance on, or decisions based on, is the sole responsibility of such third parties. The mould remedial work described in this report was limited to Rooms 153, 154, 154A, 154B and 156 Closet. Mould growth may be present in other areas of the school which were not included in the scope of the remedial work referred to in this report. 6.0 CLOSURE We trust that the enclosed is suitable for your current purposes.
