6 Recovery Worksheet Relationships & Recovery Do I need to change my relationships with using friends of family? This worksheet is designed to help you consider what, if any, relationship changes you need to make to support your recovery. In column #1 list all the people you used drugs or alcohol with, n column #2 indicate the type of relationship you had with the person, in column # 3 indicate what if any changes you thing you need to make in this relationship, and in column # 4 indicate if you need to send them a letter describing your intention to avoid using people and using settings. 1-Names of People You Used Drugs or Alcohol With List the names of the people you used drugs or alcohol with 2-Type of Relationship Friend, Associate, Dealer, Family Member, Significant Other 3-Setting Boundaries changes I need to make. 1) Avoid Entirely 2) See only in sober setting 3) Make no changes at all 4) Other indicate what you mean 4 Communication of Boundary How will you inform this person that your relationship with them needs to end or change.
