Learning Objectives This course is directed at neurologists in practice and those practitioners with interests in neuromuscular disorders. Participants will gain expertise in the evaluation and management of chronic inflammatory demyelinating polyradiculoneuropathy, myasthenia gravis, and ALS. They will additionally receive clinically useful instruction in the electrophysiologic assessment of radiculopathies. The differential diagnosis, laboratory evaluations and treatments will be reviewed. Recent findings in the placebo-controlled trial of indinavir in ALS, designed at Beth Israel, will be summarized. 3 About Neuromuscular Division The neuromuscular division at offers expert consultation for a wide range of neuromuscular diseases. These include entrapment and other mononeuropathies (ie carpal tunnel syndrome, ulnar neuropathy, and tarsal tunnel syndrome), radiculopathies, amyotrophic lateral sclerosis (Lou Gehrig s disease), polyneuropathies, myopathies, muscular dystrophies, myasthenia gravis, and other neuromuscular junction disorders. Electrodiagnostic testing, which assesses the function of peripheral nerves and muscles, is conducted by Drs. Scelsa or MacGowan on site. Routine electromyography (EMG) and nerve conduction studies as well as specialized studies- single fiber EMG, repetitive stimulation, and sympathetic skin responses- may be requested to determine quantitatively the site and severity of any neuromuscular disorder. The ALS Center The Amyotrophic Lateral Sclerosis Center (ALS) at Beth Israel Medical Center is supported by the Greater New York Chapter of the ALS Association. The Center provides clinical care for patients with ALS using a multi-disciplinary team approach. Patients with suspected ALS are referred to one of the treating neurologists for a confirmatory consultation. Patients with ALS are then seen quarterly for team evaluations. At each visit the patients are seen by a neurologist, physical therapist, occupational therapist, speech pathologist, nutritionist, respiratory therapist, nurse, and social worker, all with expertise in the management of ALS. Pulmonary and gastroenterology evaluations are readily available. Two studies funded by the National Institute of Health (NIH) will commence this Fall. One will assess early BiPAP use in ALS. The other is a clinical trial of IGF-1, insulin-like growth factor in ALS patients. An additional NIH-supported trial of ceftriaxone in ALS will begin in 2005, after proper dosing and safety are established. The goal of the center is to maximize patient s functional independence, quality of life, and comfort utilizing current therapies. The New York Chapter also provide patient and family support groups, information about ALS and available services, patient transportation, and equipment loans.
