1 Executive Summary 36.0 MIA Safety Services - April 2014 Proper first aid training that meets OSHA compliance requirements is an absolute must in order to protect the lives and health of your employees. It s also vital to the health of your organization. The National Safety Council estimates 25% of all emergency room visits could be avoided if people knew basic first aid and CPR. First aid refers to medical attention that is usually administered immediately after the injury occurs and at the location where it occurred. It often consists of a one-time, short-term treatment and requires little technology or training to administer. The purpose of this Toolbox Talk is to raise awareness among workers and employers about the hazards faced every day in the stone industry. Two toolbox talks are enclosed: one reviewing an investigation from OSHA files and another reviewing basic first aid instruction. Also provided are a standard employee meeting sign-in sheet and a safety related Product of the Month feature. 2 36.1 MIA Safety Services - OSHA Accident Summary Refer to sign-in sheet for attendees (note: a separate meeting should be held with all absent employees). Agenda Accident Type: Type of Operation: Size of Work Crew: 6 Safety Monitor on Site: Struck by Cut Stone & Stone Products No Safety & Health Program: No Regular Worksite Inspections: Training Provided: No No Brief Description of Accident Employees #1 and #2 and coworkers were unloading over 4,000 lbs. of stone slabs from an enclosed truck. They were lifting the slabs manually so a coworker could position a two-wheel hand truck under them. After they moved the first slab, the rest of the load became unstable and started to fall. The two employees close to the open end of the truck were able to get out. Employee #1 was at the closed end and became caught under the slabs. His right leg was seriously lacerated at the knee. Employee #2 was wedged in the left corner of the truck suffering a laceration to the body. Employees #1 and #2 were transported to the hospital. Employee #1 underwent surgery and stayed in the hospital for a few days. Employee #2 was treated and released. Inspection Results Investigators cited the employer for failure 1) to post and enforce unloading procedures, 2) to mandate appropriate Personal Protection Equipment (PPE), and 3) to store material in a non-hazardous manner ( b). Accident Prevention Recommendations The employer must initiate and maintain a safety and health program. Provide training to employees in the correct loading and unloading procedures and to identify fallen or shifted slabs that may present a hazard. Recap & Review Has anyone seen or experienced a similar situation? What happened? How could this accident been prevented? Reminder: Safety is the responsibility of both management and employees! The next safety meeting is scheduled for and the topic will be. NOTE: This case was selected as being representative of injuries caused by improper work practices. No special emphasis or priority is implied nor is the case necessarily a recent occurrence. The legal aspects of the incident have been resolved, and the case is now closed.

3 36.2 MIA Safety Services - Basic First Aid Instructions Refer to sign-in sheet for attendees (note: a separate meeting should be held with all absent employees). Agenda 1. Overview: This safety meeting will cover only basic first aid instructions. Your workplace may already have a trained, designated first response team. However, by reviewing these procedures, you will be better prepared to help if one of your co-workers gets injured on the job. In all cases requiring emergency medical treatment, immediately call 911 to request emergency medical assistance. 2. What is First Aid? First aid refers to medical attention that is usually administered immediately after the injury occurs and at the location where it occurred. It often consists of a one-time, short-term treatment and requires little technology or training to administer. 3. Wounds: Minor: Cuts, lacerations, abrasions, or punctures Wash the wound using soap and water; rinse it well. Cover the wound using clean dressing. Major: Large, deep and bleeding. Stop the bleeding by pressing directly on the wound, using a bandage or cloth. Keep pressure on the wound until medical help arrives. 4. Broken Bones: Do not move the victim unless it is absolutely necessary. If the victim must be moved, "splint" the injured area. Use a board, cardboard, or rolled newspaper as a splint. 5. Burns: Thermal (Heat) Rinse the burned area without scrubbing it and immerse it in cold water. Do not use ice water. Dry the area by blotting and cover it using sterile gauze or a clean cloth. Chemical Immediately flush the exposed area with cool water for 15 to 20 minutes. 6. Eye Injury: Small particles Do not rub your eyes. Use the corner of a soft clean cloth to draw particles out, or hold the eyelids open and flush the eyes continuously with water. (continued next page)

4 36.2 MIA Safety Services - Basic First Aid Instructions 6. Eye Injury (continued from previous page): Large or stuck particles If a particle is stuck in the eye, do not attempt to remove it. Cover both eyes with bandage. Chemical Immediately irrigate the eyes and under the eyelids, with water, for 30 minutes. 7. Neck and Spine Injury: If the victim appears to have injured his or her neck or spine, or is unable to move his or her arm or leg, do not attempt to move the victim unless it is absolutely necessary. 8. Heat Exhaustion: Loosen the victim's tight clothing. Give the victim "sips" of cool water. Make the victim lie down in a cooler place with the feet raised. 9. Discussion: Identify any employees who may have prior first aid and/or CPR training. Review burns procedure. Review where first aid kits are located in your shop and office. 10. Recap & Review: What is the first thing to do if a co-worker is suffering from heat exhaustion? What is the best way to stop the bleeding of a large wound? How should you proceed if a person appears to have a neck or spine injury? 11. Reminder: Safety is the responsibility of management and employees! 12. 5 36.3 MIA Safety Services - Checklist - First Aid Kit Contents The American National Standards Institute (ANSI) revised the standard relating to first aid kits in the workplace. ANSI Z replaces ANSI Z The 2003 standard required eight items as minimum for workplace first aid kits. The new standard brings the required item total to ten. The other noteworthy change is that latex is to be completely eliminated from kits. The new (2009) required items are: Quantity Description 1 First Aid Guide 1 Absorbent compress 4 x 8 minimum 16 1 x 3 adhesive bandages 1 Adhesive tape yard roll 10 Antiseptic treatment applications gram each 6 Burn treatment applications gram each 4 3 x 3 sterile gauze pads 2 Pair medical exam gloves 1 Triangular bandage - 40 x 40 x 56 min 6 Antibiotic ointment applications gram each. Additionally, there are now ten items listed as Recommended Supplies: Analgesic (oral, non-drowsy formula) Bandage compress, 2 x 2 min. Breathing barrier, single use Burn dressing, 12 square inches Cold pack, 4 x 5 min. Eye covering, 1/4 thick min. Eye/face-wash sterile, 4 oz. min. Roller bandage, 2 x 4 yards min. Hand sanitizer Employers who have unique or changing first-aid needs in their workplace may need to enhance their first-aid kits. The employer can use the OSHA 300 log, OSHA 301 log, or other reports to identify these unique problems. Consultation from the local fire/rescue department, appropriate medical professional or local emergency room may be helpful to employers in these circumstances.
