1 2014 HEALTH CENTER AND PUBLIC HOUSING NATIONAL SYMPOSIUM ALEXANDRIA, VA JUNE 10-12, 2014 CALL FOR ABSTRACTS ENGAGING RESIDENTS OF UNDERSERVED COMMUNITIES: FROM OUTREACH TO QUALITY CARE
2 Overview There are approximately 2.2 million people living in public housing units today. Near these units are health centers that provide comprehensive primary care to residents in the surrounding communities. In 2012, health centers offered primary care services to more than 21.1 million patients. These health centers are vital to all neighborhood communities because they provide comprehensive, culturally competent, quality primary health care services to medically underserved communities and vulnerable populations. The Health Center and Public Housing National Symposium is hosted by the National Center for Health in Public Housing (NCHPH) and Community Health Partners for Sustainability. The 2014 annual symposium, Engaging Residents of Underserved Communities: From Outreach to Quality of Care, will take place at the Westin Alexandria. This year s symposium will examine aspects of federal grant compliance, clinic management, service delivery, and program planning critical to the foundation and long-term sustainability of health centers. This two and a half day training is perfect for clinicians, public housing advocates, and others involved with or interested in the provision of safety-net care for public housing residents and other at-risk populations. Presentations should address issues relevant to the health of low-income populations including: health issues affecting vulnerable populations; clinical, supportive, and community services for underserved communities; and wellness promotion through outreach and education to low-income populations. Symposium Goals/Purpose This event is an excellent opportunity to network with other health professionals and public housing health advocates to discuss issues that directly affect low-income, disadvantaged populations such as public housing residents. We encourage health center administrators, clinicians, and others who are interested in providing better quality health care to the underserved public housing population to submit an abstract.
3 WORKSHOP TRACKS This annual meeting will present subject matter experts, evidenced-based practices, resources and tools focusing on the following three (3) workshop tracks: 1. Fiscal and Program Management Collaborative Care Relationships Financial Management Emergency Preparedness Planning Health Care Workforce 2. Performance Improvement Primary Care and Enabling Services: Replicable practices of preventive, intervention, chronic and specialty care delivery models for public housing residents. Oral/Behavioral Health Violence and Trauma Prevention, Wellness, Disease Management Chronic Disease, Nutrition Mental Health, Substance Use
4 Operational and Administrative: Issues pertaining to the operation and management of programs or health providers delivering services to public housing residents. Quality Improvement, Case Management Innovative Models of Care Rural Health 3. Program Development and Analysis Grant Funding, Audits Research and Data Community Outreach and Enrollment
5 Who should submit? We encourage the following to submit abstracts for workshops: Health centers providing health care to low-income populations and public housing residents Primary care associations Public Housing Authorities All health providers Social service agencies Non-profit community based organizations, faith based organizations Research organizations Community centers and organizations Submission Guidelines We encourage presentations that are interactive and participatory. Abstract submissions must include the following information: 1. Workshop title 2. Workshop description (not to exceed 200 words) 3. Three workshop learning objectives. Objectives should: a. Contain measurable action words (i.e. define, list, explain, name, compare, etc.) b. Include only one action per objective c. Identify what the workshop participant will be able to do after your presentation 4. Select one (1) workshop track and write a brief description as to how this session is relevant to this year s symposium theme 5. Each presenter s name, title, address, telephone number, fax number, and e- mail 6. A brief bio for each presenter please limit to half a page a. Please identify the primary presenter to be contacted. This person will receive all correspondence and will be responsible for sharing all information with their co-presenters b. We recommend no more than three (3) presenters per workshop 7. Incomplete abstracts will be returned! Submissions should be ed to Devon LaPoint at communications@namgt.com. 6 Criteria for Judging Relevance/Originality: Topic is of vital concern to practitioners and those offering services to low-income populations and/or public housing residents. Topic is an emerging issue with broader implications for the healthcare and/or public housing community. Workshop Objectives: Objectives to be met during the workshop are clearly stated. Implications: Implications of findings to the field, policy theory, or areas of further research are discussed. Adaptability of innovation to other practice sites or settings is discussed. Interactive: The workshop format is interactive in nature and will involve audience members as full participants in the learning process. Notification Date You may submit your abstract at any time for review until the final submission deadline, Wednesday, February 19, You will be notified in March 2014 of the status of your submission. Symposium Registration and Accommodations: All presenters must register for the symposium. Registration information will be available shortly. The registration fee will be reduced for presenters. Honoraria and compensation for travel are not available.
