Included in this packet are the following documents: Academic and Dorm Supply List School Calendar Uniform Policy School Closings & Early Check-Out Student Computer and Internet Safety Policy The following forms must be completed and returned to the school at check-in (please note that if you have already completed these forms for the school year, you do not need to resubmit). 5 Parents/ Guardians: After careful consideration, The SEED School of Maryland has made the decision to allow students to wear khaki uniform pants (only) from outside vendors. All other required uniform items shirts, skirts/skorts, sweaters, etc. must be purchased from our vendor, Arthur s Uniforms, as they will bear the official The SEED School of Maryland logo unique to our school. If khaki uniform pants are purchased at an outside retailer (such as JC Penny, Walmart, Old Navy, Target, Gap, etc.) please visit the Uniform section of the retailer to purchase the items. As a reminder, The SEED School of Maryland uniform consists of: SEED logo shirts (short- or long-sleeved, obtained through Arthur s Uniforms only) Khaki pants/shorts (no cargo pockets) from the uniform section of stores Skirts/skorts for girls (obtained through Arthur s Uniforms only) Black belt; black shoes Pants worn during the school year that are in violation of the school uniform include: Skinny pants Cargo pants Any color other than khaki Students are to wear their uniform throughout the academic school day and change into their PE uniform as directed. PE uniforms worn under their academic uniform is strictly prohibited and considered a violation of the student dress code. SEED encourages parents to put your child s name on his/her uniform clothing. 6 Computer/Internet Acceptable Use Policy (AUP) Computer Use is a Privilege, Not a Right Student use of The SEED School of Maryland (SEED- MD) computers, networks, and Internet services is a privilege, not a right. Unacceptable use or activities may result in the limitation, suspension, or cancellation of privileges as well as additional disciplinary and/or legal action as listed in SEED- MD Student Code of Conduct. Student access to SEED- MD computers, networks, and Internet services is provided for educational purposes. Students may not use the SEED- MD s computers for any personal reasons not connected with the educational program or for SEED- MD assignments without permission from the supervising staff member. Students are expected to comply with these rules and all specific instructions from the teacher, other supervising staff members, or volunteers when accessing SEED- MD computers, networks, and Internet services. The same rules and expectations govern student use of computers as apply to other student conduct and communications. Responsibilities of Students ALL students using SEED- MD technology must comply with the following: 1. Students are responsible for taking reasonable precautions to protect SEED- MD-owned technology equipment against damage and/or theft. 2. Students are responsible for using SEED- MD technology, whether onsite or remotely, in an ethical, responsible, and legal manner. 3. Students will not engage in unauthorized activities. These include, but are not limited to: a. Sharing their password or accessing any other user s account; b. Accessing data for which students do not have privilege or attempting to circumvent the SEED- MD Internet filters; c. Knowingly deploying computer viruses or software with malicious intent; d. Violating copyright laws or the privacy rights of others; including the download or sharing of any copyrighted materials such as music, films, or software; e. Plagiarism; f. Circumventing or disabling technology protection measures put in place by the Information Technology Department; g. Using technology on the SEED- MD network that is not in compliance with current SEED- MD computer standards unless approved in writing by the Director of Academics/Designee; h. Unauthorized access to chat rooms, news groups, or social media sites (e.g. Facebook, MySpace, Twitter, YouTube). Students should never meet people they have contacted through the Internet without parental permission;
7 i. Monopolizing systems (i.e. logging in to more than one computer at a time or spending an inordinate time on the computer), overloading networks, storing excessive and unneeded resources on the network (e.g. music/games), and printing excessive materials; 4. Students are responsible for backing up work stored on SEED- MD technology. Students will be notified in advance when network folders will be emptied so that items may be stored on an alternate device or Internet storage service (e.g. DropBox); 5. A student shall not reveal his/her full name, address or telephone number on the Internet without prior permission from a parent and/or teacher/counselor. Students should inform their supervising teacher/counselor if they access information or messages that are dangerous, inappropriate or make them feel uncomfortable in any way. 6. Students will not create, access, download, store, publish, forward, or print content that: a. Depicts profanity, obscenity, the use of weapons, or violence; b. Promotes use of tobacco, drugs, alcohol, or other illegal or harmful products; c. Contains sexually suggestive messages, is sexually explicit or obscene; d. Depicts gang affiliation; e. Violates local, state, or federal laws or regulations or violates other SEED- MD procedures or rules; f. Contains language or symbols that demean an identifiable person or group or otherwise infringe on the rights of others; g. Causes or is likely to cause a disruption to SEED- MD activities or the orderly operation of SEED- MD; h. Contains rude, disrespectful, or discourteous expressions inconsistent with civil discourse or behavior; i. Constitutes bullying, cyberbullying, harassment, or intimidation in violation of The SEED School of Maryland Bully Policy. 7. Reasonable exceptions to this provision may be made for students conducting educational research under the direction of a teacher/counselor. Specific permission must be granted regarding the nature of the research to be conducted and the type of files related to that research which might be accessed/created. Documented permission must be kept on file. 8. Be responsible for the replacement cost of misused equipment. Computer privileges will be revoked until compensation is made. No Expectation of Privacy The SEED- MD retains control, custody and supervision of all computers, networks and Internet services owned or leased by the SEED- MD and reserves the right to monitor all computer and Internet activities by students. Students have no expectation of privacy in their use of SEED- MD computers, networks and Internet services, including and stored files. Student Laptop Computers Additional rules and agreement for laptops issued to students must be completed before students will be provided a laptop when implemented by The SEED School of Maryland.
8 Student Computer/Internet Use Acknowledgment Form No student shall be allowed to use The SEED School of Maryland (SEED- MD) computers or the Internet until the student and parent/guardian have signed and returned this acknowledgment to SEED- MD. Student: I have read the Student Computer/Internet Acceptable Use Policy (AUP) and agree to comply with the rules. I further understand that violation of the policy and/or rules may result in the limitation, suspension, or revocation of computer privileges and that I may also be subject to further disciplinary and/or legal action. 9 BLANKET FIELD TRIP PERMISSION FORM TO WHOM IT MAY CONCERN: (Name of student) has my permission to participate in off-campus field trips to be taken by The SEED School of Maryland during the school year. Transportation for these trips will be provided by an approved school bus/charter company. As parent/guardian I acknowledge the following: 1. School officials are authorized to obtain emergency medical treatment for this student as necessary. 2. During field trips, The SEED School of Maryland will not be liable for injury to this student as result of the negligence, errors, and omissions of others (i.e., charter bus owners and drivers, or amusement park owners or workers), their agents, heirs, employees or assigns either through their action or inaction. 3. If your child takes personal belongings on field trips, he or she will be responsible for them. The SEED School of Maryland accepts no responsibility for personal items, such as watches, purses, money, cameras, and wallets, etc. If a student stores personal items in a locker at the designated field trip location, that entity may be responsible for any loss or damage. 10 PHOTO & MEDIA RELEASE FORM I hereby grant permission to The SEED School of Maryland to photograph, interview or videotape my child and/or display my child s work for the school year. I understand that photographs/interviews/video tapes, student work or portions thereof may be used for public viewing [including but not exclusive to: school news programs, slide shows, presentations by school or The SEED Foundation employees at conferences and/or workshops, web sites, distributed print media, displays/publication, and local newspapers]. I give my permission for my child s work and/or photograph to be posted on The SEED School of Maryland and The SEED Foundation web site. I agree to my child's participation without financial compensation, and I understand that this releases any photographer/interviewer from the school, The SEED Foundation, or authorized media personnel from any future claims, as well as from any liability arising from the use of said photograph/interview/video tape or portions thereof used for public viewing. 11 Student Computer and Internet Safety Policy The SEED School of Maryland (SEED- MD) provides computers, networks and Internet access to support the educational mission of the school and to enhance the curriculum and learning opportunities for students and school staff. SEED- MD believes that the resources available through the Internet are of significant value in the learning process and preparing students for future success. At the same time, the unregulated availability of information and communication on the Internet requires that schools establish reasonable controls for lawful, efficient and appropriate use of the technology. Student use of school computers, network and Internet services is a privilege, not a right. Students are required to comply with this policy and the accompanying rules (Computer/Internet Acceptable Use Policy). Students who violate the policy and/or rules may have their computer privileges revoked and may also be subject to further disciplinary and/or legal action. SEED- MD expects that all users will operate technology in a responsible, civil, ethical, and appropriate manner. All SEED-MD computers remain under the control, custody and supervision of the school. SEED- MD reserves the right to monitor all computer and Internet activities by students. Students have no expectation of privacy in their use of school computers, network and Internet services. Students are not allowed to use privately-owned computers at school, connect any personal device to the network, or access any wireless system by bypassing the network. It is the policy of SEED- MD to a. Prevent users from accessing of transmitting inappropriate material over its network via the Internet, electronic mail, or other forms of direct electronic communication; b. Prevent unauthorized access and other unlawful online activity including hacking ; c. Prevent unauthorized online disclosure, use, or dissemination of personal identification information of minors; d. Promote the safety and security of users when using electronic mail, chat rooms, instant messaging, and other forms of direct electronic communications; e. Use filtering technology designed to block materials that are obscene or harmful to minors and child pornography; and f. Comply with the Children s Internet Protection Act [Pub. L. No and 47 USC 254 (h)]. SEED- MD takes precautions to supervise student use of the Internet and also educate students about appropriate use of the Internet including interacting with other individuals on social networking sites and in chat rooms; however, parents should be aware that the school cannot reasonably prevent every instance of inappropriate use, including access to objectionable materials and communications with persons outside of the school in violation of School policies, procedures, and rules. SEED- MD is not responsible for the accuracy or quality of the information that students obtain through the Internet.
12 Before a student is allowed to use school computers and Internet services, the student and the student s parent/guardian must sign and return the Student Computer/Internet Use Acknowledgment Form. Students and parents shall be informed of these policies and procedures on an annual basis through handbooks and/or other means selected by the Director of Academics. Parents may request alternative activities for their children that do not require Internet access through an opt-out process. The Director of Academics shall be responsible for overseeing the implementation of this policy and the accompanying rules and for advising the School Media & Technology Advisory Committee of the need for any future amendments or revisions to the policy or rules. The Director of Academics may develop additional administrative procedures or rules governing the day-to-day management and operation of the school s computer systems as long as they are consistent with current policies and rules. The Director of Academics may delegate specific responsibilities to others as he/she deems appropriate.
13 UNIFORM POLICY The SEED School of Maryland has a dress code that is strictly enforced. During the academic day students are expected to be in uniform. Student may wear regular clothing following the academic day. Uniforms must be purchased from Arthur s Uniforms with the exception of uniform pants and students must be in uniform by the 1 st day of class. No Exceptions. Uniforms should be clean and neatly pressed with appropriate footwear. Students are required to wear solid black shoes (to include tennis shoes). The school emblem is the only permitted symbol on uniform shirts. Uniform shirts are maroon or white. Shirts should be tucked in pants or skirts. All pants must have black belts and must be worn around the hips. Underwear cannot be showing. Skirts must be the appropriate length (knee length or no more than one inch above the knee). Girls can wear tights or knee highs. They can only be one of the following colors: white, brown, black or grey. Leggings are not permitted! Students can only wear stud earrings. Brushes, combs and picks are not part of the dress code and must not be in pockets or in student s hair at any time. Hair must be neatly combed. Hats and scarves cannot be worn during the school day Students are not permitted to wear t-shirts or other clothing with profanity, endorsement of drugs and/or alcohol or displaying any negative comments. Jeans may not have holes or tears. Midriffs, halter tops and other tops that are similar in style are prohibited. Short skirts and inappropriate shorts are also prohibited.
14 I have received a copy of the SEED approved dress code and will adhere to it. I understand that if my child is not in compliance he or she can possibly be sent home and I will be asked to come in to meet with school management.
