1 June 8, 2016 Dear CREST families: RE: CREST CLUB ENROLLMENT The 2016/17 school year is just around the corner and the City of Santa Monica is looking forward to offering your child(ren) a host of fun and interesting after school programs! After-school programs include: CREST Club: provides soon-to-be 4 th or 5 th graders with homework assistance, enrichment classes and organized games from school dismissal until 6:00 p.m. Registration begins on August 1, Homework Club: is an after school program that gives students academic support and a well structured environment to work on homework. The program is offered to children in grades 1-5 at Edison, Grant, John Muir, McKinley, SMASH, Roosevelt and Will Rogers. Registration is limited and begins on August 1, Playground Access: students in grades 1-5 engage in supervised, unstructured play in designated areas of the playground afterschool with basic game equipment available for check out. Registration begins August 1, PM Extended Care: provides students in grades 3 rd -5 th care from 5pm-6pm. Registration begins August 1, Youth Sports: is a non-competitive recreation program where everyone participates regardless of ability while giving athletes the opportunity to compete and learn about the skills and rules of each sport. Sports is offered for 3 seasons during the school year for students in grades K-8. Registration for the fall season begins on August 29, Enrichment: our school-based enrichment classes are an easy way for parents to add exciting learning adventures to their child's day. All classes are led by handpicked, professional instructors for youth in grades K-5. Registration for the fall session begins August 25, Registration for the following programs will be taken separately: Homework Club, Playground Access, PM Extended Care, Youth Sports and Enrichment. We recommend that you take advantage of the opportunity to register early for CREST Club to ensure a smooth first day of school. Attached you will find an enrollment packet for CREST Club; please read all instructions carefully and ensure that all sections are completed and that you have signed each page where necessary. Incomplete packets will delay your registration! A $50 registration fee as well as the August CREST Club prorated fee must be included at the time you submit your registration packet.

3 STEP-BY-STEP GUIDE CREST CLUB ENROLLMENT PACKET ENROLLMENT OPTION & PROGRAM FEES Please review the CREST Club options available. Select one based on your needs. All changes must be submitted in writing to the program supervisor. ACTIVITY REGISTRATION FORM Please make sure this form is completed fully with your child s name, date of birth, age and grade. Your contact numbers and child s medication/special needs information must also be included. Please make sure method of payment is clearly identified (with numbers and expiration date for credit card payments). Please fill in the details of the adults authorized to pick up your child(ren) in an emergency. Please print your name, sign and date the form. PROGRAM FEE AGREEMENT FORM Please read carefully and complete fully with your child s name and enrollment option. AUTO BILLING FORM Please complete fully and ensure credit card information is accurate. Please sign at the bottom of the page authorizing monthly payment. CHILD INFORMATION QUESTIONNAIRE In order to best serve your child(ren), please complete the forms attached, giving us as much information as possible. FINANCIAL ASSISTANCE APPLICATION The City of Santa Monica offers Financial Assistance to qualifying families (Santa Monica residents and non-residents) with moderate to low income whose students attend a Santa Monica-Malibu Unified School District School and participate in CREST after-school programs during the school year. Financial Assistance must be approved prior to registering for the CREST programs. Please note that financial assistance applications may take up to 15 business days to process. 4 CREST CLUB OPTIONS/FEES* Service Hours of Service Days of Service Fees CREST Club From school dismissal until 6:00 PM School year Monday-Friday when school is in session $300 per month AM Care with CREST Club registration From 7:00 AM until the start of the school day School year- Monday-Friday when school is in session $50 per month AM Care only From 7:00 AM until the start of the school day School year- Monday-Friday when school is in session $165 Per month Late Start AM Care only 7:00 AM until the start of the school day School year - 1 day a week at Grant, Roosevelt or Will Rogers $126 per year (Roosevelt, Grant and Will Rogers) Daily 8:30AM -5:00PM during Pupil Free Day and Camps. School Year / Camps $75 per day Registration Fee: A one-time $50 registration fee will need to be included with the first month s payment. (Exception: Those applying for Financial Assistance or receiving Connections for Children/CalWorks should contact the office with regards to their payments). *Pending City Council approval on June 14, 2016
5 CREST Club Program Fee Agreement will be participating in CREST CLUB for the school year. (Child s name) Payments are due on the 25 th of each month for the following month. Non-payment by the 1 st of the month will result in loss of program services. All students are expected to follow the established site rules. Progressive discipline is used and may include parental involvement. The CREST Club program ends at 6:00 p.m. Late pick-ups will be noted and charged a fee of $1 per minute. Late pick-ups will result in termination of services. Each child will be signed into the program DAILY by the CREST Staff. An authorized person must sign the child out. No child will be released to anyone who is not listed on their emergency card unless PRIOR, written permission is given by the parent. Emergency card information should be updated as changes occur. You have enrolled your child in the option of the program. Your child will be expected to attend according to this schedule.
