Dear Sir, T-17/ Tender for providing wall mounted medicine storage unit at Office Dispensary and built in furniture items in Security Officers flat in the Annexe Building Belapur, Navi Mumbai. Having examined the drawings, specifications, designs and schedule of quantities relating to the works specified in the memorandum hereinafter set out and having examined the site of the works specified in the said memorandum and having acquired the requisite information relating thereto as affecting the tender. I/W hereby offer to execute the works specified in the said memorandum within the time specified in the said memorandum at the rates mentioned in the attached schedule of quantities (Part II) and in accordance in all respects with the specifications, designs, drawings and instructions in writing referred to in Conditions of Tender, the Articles of Agreement, Special Conditions, Schedule of Quantities and Conditions of Contract and with such materials as are provided for, by and in all other respects in accordance with following memorandum and such other conditions so far as they may be applicable. Memorandum (a) Description of work Providing wall mounted medicine storage unit at Office Dispensary and built in furniture items in Security Officers flat in the Annex Building Belapur, Navi Mumbai. (b) Estimated cost (Rs.) 2,63,000/- inclusive of all taxes/charges (c) Earnest Money (Rs.) Rs. 5,260/- to be deposited along with tender document. (d) Percentage of retention money, if 5% from each paid bill (e) any, to be deducted from bill Time allowed for completion of the works from tenth day after the date of written order to commence work 6 weeks from the 10 th day of issue of work order for the work

3 Should this tender be accepted, I/we hereby agree to abide by and fulfill the terms and provisions of the said condition of contract so far as they may be applicable or in default thereof to forfeit and pay to the Reserve Bank of India the amount mentioned in the said conditions. 3. We have deposited a sum of Rs. 5260/- (Rupees five thousand two hundred sixty only ) as earnest money with the Reserve Bank of India, which amount is not to bear any interest. Should we fail to execute the Contract when called upon to do so, we do hereby agree that this sum shall be forfeited by us to the Reserve Bank of India. 5. The Tender is submitted in two parts in separate sealed envelopes. Terms and Conditions 1. The two part sealed quotation should be addressed to Shri S N Panda Chief General Manager, Reserve Bank of India, Sector-10, Plot No.3 H.H. Nirmala Devi Marg, CBD, Belapur, Navi Mumbai (by name) and super scribed Tender for providing wall mounted medicine storage unit at Office Dispensary and built in furniture items in Security Officers flat in the Annexe Building, Belapur, Navi Mumbai and sent so as to reach him not later than 2.00 pm on April 04, The Part-I (Technical Bid) of the tender will be opened on the same day/date at pm. The date of opening of Part-II of the tender document will be intimated later to eligible bidders only 3. The Reserve Bank of India does not bind itself to accept the lowest or any other quotation and reserves the right to reject any or all the quotations without assigning any reasons there to. 4. Each of the quotation documents is required to be signed by the authorized person or persons. If any of the documents is missing or unsigned the quotation will be considered invalid. 5. The quotation forms must be filled in, in English and all entries must be made by hand and written in ink. All erasures and alternations made while filling the quotations must be attested by initials. 6. Intending tenderers shall pay as earnest money deposit a sum of Rs.5,260/- (Rupees five thousand two hundred sixty only) by way of demand draft or Bank Guarantee issued by Scheduled Bank drawn in favour of Reserve Bank of India, Belapur. The Demand Draft or Bank Guarantee may be directly attached to the tender document. A tender which is not accompanied by such Demand draft or Bank Guarantee as Earnest Money, will not be considered. The earnest money will be refunded to the tenderer if his tender is not accepted but without any interest. 7. Under no circumstances earnest money deposits will be accepted in the form of fixed deposit receipts or Insurance guarantee or cheque.

5 8. The Earnest Money Deposit of Rs. 5260/- paid by the successful tenderer shall be held by the Reserve Bank of India as security for the execution and due fulfillment of the contract. No interest shall be paid on the said deposit. 9. In addition to the Earnest Money Deposit under clause 6 and as a further security for the due fulfillment of the contract by the Contractor, an amount at the rate of 5% of the value of the work done will be deducted by the Bank from each payment to be made to the contractor towards retention money This total amount (EMD +RMD) will be termed as a Security Deposit. On virtual completion of the works, the Contractor would be paid Earnest Money Deposit and the Retention Money will be released by the Employer after rectification of the defects pointed out during the defects liability period of 12 months. The amounts retained by the employer shall not bear any interest. 10. In case if the contractors so request, the Retention Money Deposit will be held in the form of a Bank Guarantee of an approved Scheduled Bank in proforma to be got approved by the Bank. 11. The Contractor should note that unless otherwise stated, the tender is strictly on item rates basis and his attention is drawn to the fact that rates for each and every item should be correct, workable/reasonable and selfsupporting. The quantities in the Schedule of Quantities approximately indicate the total extent of work, but may vary to any extent and may even be omitted thus altering the aggregate value of the Contract at the sole discretion of the Bank, the contractor shall not be entitled to any extra claim on the account. 12. Wherever basic rates of materials specified in the Bill of Quantities, the contractor shall provide the Bank actual paid bills for perusal. In case Bank s approved quality priced differently, the difference including taxes excluding overhead charges will be considered for payment/recovery. 13. Contractors should examine the site and should thoroughly acquaint themselves with the nature of work to be done. 14. The work shall have to be carried out including Sundays and holidays with a minimum of disturbance to the occupants/office. 15. The rates shall be inclusive of all taxes, duties, levies & ST/SBC etc. The rates shall be firm and shall not be subject to any variations, labour conditions or any conditions whatsoever. 16. The work shall be commenced within one week from the date of receipt of work order. 17. The work shall be completed in all respect within 6 weeks from the 10 th day of issue of work order. No extensions of time will normally be allowed except for conditions considered by the Bank. 18. If the contractor fails to complete the work by the date stated or within any extended time, the contractor shall pay to the employer a sum of Rs.200/- (Rupees two hundred only) per day as liquidated damages for the period during which the said works shall so remain incomplete and the employer may

6 deduct such damages from any money due to contractor, subject to a maximum of 10% of the value of contract. 19. Payment will be made after satisfactory completion of the work on the basis of actual work measured at site. 20. Any defects noticed during defect liability period will be rectified free of cost, failing which the cost of rectification of the defects will be deducted from the retention moneys. 21. All the damages to the walls, floors, false ceilings, ducts etc. during the execution of work, must be repaired by the contractor to match the original at his own cost. 22. The successful renderer shall take out all necessary insurance policies at his own cost such as Contractor All Risk Policy for total value of work; Third Party Liability (Minimum Rs 2.00 Lakh (Rupees two lakh only) per person for any one accident or occurrence and Rs 5.00 Lakh (Rupees five lakh only) in respect of damage to property for any one accident or occurrence) and Workmen compensation Policy to be valid till date of completion of the contract period. All policies shall be taken in joint name of Reserve Bank of India and contractor, the Reserve Bank of India name shall appear first in all the policies. 23. In all matters of dispute arising on the work, the Bank s decision will have to be taken as final and will be binding on the contractor. 24. The estimated cost of work is Rs 2.63 Lakh (Rupees two lakh sixty three lakh only) approximately. 25. The value of interim payment shall be Rs 1.25 Lakh (Rupees one lakh twenty five thousand only). Bill of Quantity I. no Description of item Qty Rate (Rs.) 1. Providing wooden wall mounted medicine storage unit made out with approved make M.R. Grade ply confirming IS 303 of different thickness i.e 12mm, 15mm & 18mm same as provided in existing dispensary. Rear side of the unit shall be provided with 12mm M.R. Grade ply. The shutters / shelves shall be made out with 15mm thick M.R Grade ply as per existing etc. All inner surface of the carcass shall have 1mm thick plain white laminate including inner surface of shutters, all exposed including shutters shall have 1mm thick colored/ designed laminate and the middle open shelves shall have sliding shutters of 12mm thick plain glass instead of existing 8mm thick plain glass both end of the unit shall have covered unit from floor to ceiling as similar as provided in existing unit (only one side) etc. The rate shall include for providing and fixing approved aluminum sliding channels for glass sliding shutters, heavy quality stainless steel auto closer hinges, SS handles, locks, heavy quality S S brackets with rollers for holding and moving the glass shutter, providing and fixing required wooden beadings / moldings, providing and applying French polishing to exposed wooden beadings / moldings etc all complete as directed and as per existing medicine storage unit in dispensary.(the basic rate of design laminate shall be Rs 115/- per sqft). Grade ply, rear side of the unit shall be provided with 12mm M.R. Grade ply, shutters / shelves shall be made out with 18mm thick M.R Grade ply etc. All inner surface of the carcass shall have 1mm thick plain white laminate including shelves and inner surface of shutters, the all exposed surface including shutters shall have 1mm thick colored/ designed laminate. The rate shall include for providing and fixing approved heavy quality stainless steel auto closing hinges, S S handles, locks,providing and fixing required wooden beadings / moldings, providing and applying French polishing to exposed wooden beadings / moldings etc all complete as directed (The basic rate of design laminate shall be Rs 115/- per sqft). The size of cabinet shall be:- (i) 0.80 m x 0.60 m 1 No (ii) 1.20 m x 0.60 m 1 No 1.20 Sqm 3 Providing wooden wall cabinet unit to kitchen made out with approved make M.R. Grade ply, shutters shall be made out with 18mm thick M.R Grade ply frame work with 5mm plain glass panels with approved design/pattern etching. The shelves shall be provided with 12mm thick plain glass resting/ fixed with the help of heavy quality approved stainless steel bracket etc. All inner surface of the carcass shall have 1mm thick plain white laminate including shelves and inner surface of shutters, the all exposed surface including shutters shall have 1mm thick colored/ designed laminate. The rate shall include for providing and fixing approved heavy quality stainless steel auto closing 0.84 Sqm

Grade ply, shutters/ shelves shall be made out with 18mm thick M.R Grade ply etc. All inner surface of the carcass shall have 1mm thick plain white laminate including shelves and inner surface of shutters, the all exposed surface including shutters shall have 1mm thick colored/ designed laminate. The rate shall include for providing and fixing approved heavy quality stainless steel auto closing hinges, S S handles, locks,providing and fixing required wooden beadings / moldings, providing and applying French polishing to exposed wooden beadings / moldings etc all complete as directed (The basic rate of design laminate shall be Rs 115/- per sqft). The size of unit shall be as follows :- (i) 1800 mm x 660 mm x 450mm- 2 Nos 5 Providing and fixing chimney, manufactured by M/S Glen, Faber or approved equivalent make with air suction capacity 800 Cum per hour. The shutters (3 Nos)/ shelves (6 Nos intermediate) shall be made out with 18mm thick MR Mr grade Ply etc. All inner surface including shelves & shutter shall have 1mm thick plain white laminate. All exposed surface of the unit shall have 1mm thick design laminate etc. The rate shall include for providing making 3 Nos of drawers at the middle of main wardrobe with 12mm MR ply and finished with 1mm thick white plain laminate all around, small portion of the wardrobe shall have 4 number of shelves excluding bottom., Main wardrobe shall have one shelve between bottom and drawer unit and one shelve above the drawer unit finished all round with 1mm thick white plain laminate; providing heavy quality S S hinges(4-4nos) to end shutter; 3 Nos 6 9 long decorative S S handles for shutter 3 Nos, 4 long decorative S S handles for drawers; top and bottom megnatic lock to each & every shutter; heavy quality locking shutter for all the shutters; including providing required thickness TW beading to all the open edges of MR ply incl French polishing etc all complete as directed and as per enclosed drawing.
