4 THE LEAPFROG GROUP FOR PATIENT SAFETY The Leapfrog Group is a voluntary program aimed at mobilizing employer purchasing power to alert America s health industry that big leaps in health care safety, quality and customer value will be recognized and rewarded. Among other initiatives, Leapfrog works with its employer members to encourage transparency and easy access to health care information as well as rewards for hospitals that have a proven record of high quality care. The Leapfrog Hospital Quality and Safety Survey asks hospitals to report on the steps they take to improve the quality of patient care. Survey results are available for public viewing on Leapfrog s web site, The Leapfrog Hospital Quality and Safety Survey asks hospitals if they adhere to the quality and safety practices on the following pages. The Leapfrog Group works with over 50 leading medical experts to identify solutions that will improve hospital quality and safety and then gather this information from hospitals around the country.

5 OVERALL PATIENT SAFETY RATINGS Safe Practices Select a hospital that has a full Leapfrog Safe Practices Score. This means it has put in place 17 procedures to reduce preventable medical mistakes. Preventing Medication Errors Patients should choose a hospital that uses computerized prescriber order entry (CPOE) systems to order medications, tests, and procedures. CPOE systems are computer systems designed to alert doctors and other prescribers to potentially serious problems with their instructions. Managing Serious Errors Patients should choose a hospital that has implemented Leapfrog s policy on managing serious events ( or never events ) such as surgery on the wrong body part or death due to contaminated drugs or devices. HIGH-RISK TREATMENT SAFETY RATINGS Select a hospital with a lot of experience and the best results for specific procedures, surgeries, or conditions. This is known as Evidence Based Hospital Referral (high-risk procedures). The scores for the following high-risk treatments are based on a combination of quality and costs: Aortic Valve Replacement Abdominal Aortic Aneurysm Repair Pancreatic Resection Esophageal Resection High-risk Deliveries Appropriate ICU Staffing Patients should choose a hospital with an intensive care unit (ICU) that is staffed by doctors and other caregivers who have special training in critical care medicine. These doctors are called intensivists.

14 IN SOUTH CAROLINA HEALTHY BABIES ARE OUR BUSINESS Over the course of the last 3 years, South Carolina s Birth Outcomes Initiative has substantially improved infant health. This collaborative effort allowed the state to be recognized as a national leader in birth outcomes and reducing the number of low birth weight infants. The SC Birth Outcomes Initiative is dedicated to making a significant impact for all moms and babies: All birthing hospitals state-wide have committed to end non-medically necessary deliveries before 39 weeks gestation. There has been a 51% decrease of early elective births across the state. In South Carolina, babies are 3 times more likely than the national average to be born in a Baby-Friendly hospital. 3) Greenville Memorial Hospital 4) Medical University of SC 5) Roper St. Francis Mount Pleasant Hospital 6) Waccamaw Community Hospital Birthing hospitals state-wide have pledged to reduce the number of cesarean sections that are performed for first time low-risk moms. South Carolina is dedicated to making a difference in the health of women and babies in our state. Babies with a low birth weight (less than 5 and a half pounds) are more likely to need special care in a newborn intensive care unit, have developmental and educational delays, and have chronic diseases such as high blood pressure and diabetes later in life. Map: SCDHEC The map above highlights areas of our state where there is a higher proportion of babies being born at a low birth weight. The darker color, the higher the proportion (i.e. Chester and Hampton). Low birth weight can be prevented by women and men, years before the baby is even conceived by not smoking and controlling chronic conditions like hypertension and diabetes. Let s work together to make sure the babies in our state have a healthy start! The darker spots on the map above are the places in our state where African American babies are much more likely to be born at a low birth weight (less than 5.5 pounds) than Caucasian babies. In some areas of the state, African American babies are more than 10 times likely than Caucasian babies to be born at a low birth weight, which means, for example, that if 2 out of 100 Caucasian babies are born at a low birth weight, 20 or more out of 100 African American babies are born at a low birth weight in the same area.
