1 Sample Program Parent Survey My child is (children are) currently in gifted programs in grade(s) (select all that apply): Purpose: The purpose of this survey is to gain insights about the gifted program that will guide school improvement. Responses are anonymous. The results will be used collectively to understand the perceptions of the program from community stakeholders. Directions: If you have more than one child who is participating or has participated in the past, in the first section you will be provided spaces for responding for up to three different children. You will be asked to select the current grade Child #1 is in, and you will respond for that child for that grade. You will also be asked to respond to the items in relation to the previous grade for that same child. Then you can enter the current grade for Child #2, and you will respond for that child for that grade. You will also be asked to respond to the items in relation to the previous grade for that same child. Finally, you can repeat the process for Child #3, indicating your responses for that child for the current grade as well as the prior grade. If you have more children in the program, please make comments about their experience in the comment section. If clarification or further explanation is warranted, please use the comment sections provided. For all questions, there will be a scale for you to provide your ranking such as: If you just answer each question with the scale items, the survey will likely take about 10 minutes to complete. However, if possible, it would be helpful if you could provide comments to help explain your rankings. Curriculum and Instruction 1.
