I was worried that I couldn't afford to pay for my treatment. However when I was offered financing by it meant I could get my treatment right away. I m delighted with my vision and I m paying less per month for my laser vision correction than I was for my contact lenses and glasses. S. L., Contruction Worker Are You a Candidate? Laser vision correction can now treat a very high degree of nearsightedness, farsightedness and astigmatism. Your eyes are as individual as you are. In order to assess your eligibility for laser vision correction, we invite you to attend a free consultation with one of our experienced Doctors of Optometry. This in-depth eye examination, which includes additional tests that go beyond the standard eye examination as well as assessments of your expectations and lifestyle, will determine if you are a suitable candidate. Nearsightedness (Myopia) If you need vision correction to see things clearly at a distance, you are nearsighted. Without glasses or contact lenses your vision is blurred. Farsightedness (Hyperopia) If you need vision correction to read or see things close up, you are farsighted. Depending on your age you may have problems focusing on distant objects too. Astigmatism Astigmatism is a condition in which the cornea is more egg-shaped than round. Most astigmatic corneas have two curves a steeper curve and a flatter curve. This causes light to focus at more than one point in the eye, resulting in blurred vision. Nearsighted and farsighted people often have a degree of astigmatism. Reading Glasses (Presbyopia) Presbyopia should not be confused with farsightedness. In middle age, the natural lens can no longer quickly alter its focus from far away to close up. By their 40s and 50s, whether they have laser surgery or not, most people require reading glasses, even if they have never had to wear glasses before. Treatments will soon be available to eliminate the need for reading glasses. If you are unsure whether or not this treatment would be appropriate for you, please contact us. A number of alternative solutions, such as an Intraocular Lens Implant, may be a good choice for you. 7

Your Consultation Your in-depth consultation includes preliminary diagnostic tests, an examination by your optometrist, and a follow-up discussion with your patient counselor. This comprehensive consultation takes approximately 90 minutes. An optometrist will determine whether you are a suitable candidate for laser vision correction, explain the procedure which best fits your individual needs, and answer all of your questions to enable you to make an informed decision. Even if you are not a candidate now, you may become one in the future as new technology emerges. The following tests are used to establish your eligibility for laser vision correction. The information gathered from the WaveScan identifies and measures optical imperfections, providing the surgeon with a detailed, accurate prescription and optical map of your eyes. Lensometry: We check your eyeglasses to determine your current prescription strength. Autorefraction: An automated refractor measures how light passes through your eye in order to help determine your baseline prescription. Tonometry: Used to measure your eye pressure. It is a key indicator in diagnosing the eye condition glaucoma. Pentacam: Gives detailed information on the shape of your cornea (the outer surface of your eye). It provides the optometrist and surgeon with accurate information about the surface shape and thickness of your cornea and how this varies from the center to the periphery. CustomVue WaveScan : Identifies and measures imperfections in your eye 25 times more precisely than standard methods used for glasses and contact lenses. It provides the essential data used in a WaveFront treatment. Visual Acuity Testing: Measures the eye s ability to see details at near and far distances. These tests usually involve reading letters or looking at symbols of different sizes on an eye chart. Your uncorrected visual acuity (UCVA) is what you see without glasses or contact lenses while your best corrected visual acuity (BCVA) is what you see with the optimum correction in place. Refraction: Your optometrist verifies your current prescription and compares this to the lensometry readings and previous prescription details to ensure stability. Cycloplegic Refraction: Eyedrops are used to relax your eye muscles and allow your optometrist to determine how your prescription may change over time. Pupillometry: This test takes a measurement of the size of your pupils in the dark when they are at their largest, using infrared technology. Pachymetry: The Pachymeter is an ultrasound device used to determine the thickness of your cornea in any given location. The thickness of your cornea is very important when determining whether you are a suitable candidate for laser vision correction. Slit Lamp Exam: This test uses a microscope and a small beam of light to examine every part of your eyes. Eye Dominance: This test measures which of your eyes is the dominant, or stronger, eye. Retinal Exam: The optometrist will assess the health of your internal eye, particularly the retina. The drops that were used for the cycloplegic refraction aid in this exam as well, providing an excellent comprehensive view of the retina. Our optometrists have extensive training and experience in the management of refractive surgery. Annual continuing education, peer review, and ongoing assessment are non-negotiable protocols for maintaining the high standard expected at. Only after a comprehensive examination can we determine if laser eye surgery is right for you. 9

When you make the decision to have laser vision correction, your focus should be on quality care, experienced surgeons, and outstanding, life-changing results. You will find all this and more at. Why Choose? If you are considering laser vision correction, this is an important question. Not all lasers are the same. Yesterday s laser gives yesterday s results. At, we use only the technologically advanced VISX S4IR laser. This laser currently treats over 60% of patients in the US, where most of the laser procedures in the world take place. It is proven to deliver SAFE, ACCURATE, and PRECISE results. This is why athletes and celebrities like Tiger Woods and Cindy Crawford choose to be treated with this laser. VISX Lasers: What are they? The VISX excimer laser used in refractive surgery is a gentle, cool beam laser that reshapes the cornea at the front of the eye. The VISX laser removes less tissue from the cornea per diopter of prescription than most of the lasers used by other providers. Each laser pulse removes 0.25 microns of tissue (1 micron = 1 thousandth of a millimeter). This is the same as removing: 1/0 of a human hair, or 1/39,000,000 of an inch in 4 billionths of a second (Shown above - Laser ablations on a human hair) To ensure the highest standards of quality vision, all lasers are equipped with advanced eye-tracking technology. The 3-D eye tracker works by guiding the beam as it moves over the cornea, ensuring the laser treats only the section of the cornea that requires reshaping. The 3-D eye tracker also ensures that even if your eye moves, the laser beam tracks the movement thus enabling a safe and precise treatment. As laser vision correction continues to evolve, constantly provides our teams with the best resources and the latest technology to remain a global leader in the industry. 10 "I had LASIK Advanced CustomVue WaveFront after 10 years of wearing contacts and glasses. I can now see better than /. The whole experience has been life-changing. I can now enjoy a night out, go swimming, and drive without the inconvenience of glasses and contacts. Would I recommend? I have six friends and family who have already booked their consultations! L. W., Dentist

The entire staff, the people, the science, the results I can see! At last after 32 years of wearing eyeglasses, I now have the best eyes in the world!!! Now I have the self-confidence I never had before, both in personal and professional life. Thank you for my newly-found self-esteem. F. A., Teacher Available Treatments and Results A Look at LASIK LASIK (Laser-Assisted-In-Situ Keratomileusis) is currently the most frequently performed elective procedure in the world and at. It is a highly effective procedure that is suitable for most low, moderate and higher prescriptions. LASIK uses a cool beam of light from the excimer laser to gently reshape the front surface (cornea) of your eye. The entire procedure takes about 10 minutes. LASIK patients experience less post-procedural discomfort than others, and both eyes can be treated simultaneously. Most patients experience a significant improvement in vision within a few hours, and the majority are able to return to work after 48 hours. Following treatment, your eyes may feel light-sensitive, gritty and watery. We recommend that you bring sunglasses with you. Some patients may feel slight irritation for a few hours, but most are quite comfortable after a rest. Visual recovery is faster with the LASIK procedure. You will be given light, transparent eye shields to wear for 5 nights after treatment; they protect your eyes while you sleep. Your optometrist may decide that LASIK is not the best option for you and may recommend Photorefractive Keratectomy (PRK) instead A Look at PRK PRK (Photorefractive Keratectomy) is an alternative laser refractive procedure to correct nearsightedness, farsightedness, and astigmatism. PRK is sometimes referred to as LASEK or EpiLASIK. It involves removing the top layer of the cornea before reshaping the cornea with the excimer laser. Then a soft contact lens is inserted for a few days following the surgery The PRK Procedure 1. Anesthetic drops are applied to the eye. 2. The surface of the outer layer of the cornea is gently removed by the surgeon. 3. Computer-controlled pulses of cool laser light are applied to the surface layers of the cornea. As the laser pulses, it makes a ticking sound. The outer cornea layer is re-shaped with the laser to correct nearsightedness, farsightedness, or astigmatism. 4. A bandage contact lens is placed on the eye to enhance healing and comfort. The LASIK Procedure 1. Anesthetic drops are applied to the eye. 2. A protective flap is created and gently lifted by the surgeon, revealing the inner corneal tissue. 3. Computer-controlled pulses of cool laser light are applied to the inner layers of the cornea. As the laser pulses, it makes a ticking sound. The inner corneal layer is reshaped with the laser to correct nearsightedness, farsightedness or astigmatism in your eye. 4. The Surgeon gently replaces the flap in its original position. It heals naturally and securely. Statistically, 85% of all treatments performed in the US and Europe are LASIK treatments; they are more convenient and afford a faster recovery time. However, some patients may have thin corneas or be involved in occupations or activities that would preclude LASIK and make PRK a more appropriate procedure. After a short period of time, the visual outcomes are identical. Our optometrists will recommend the most appropriate treatment for your unique needs

LASIK Surgery and its follow-up care have absolutely been a life-changing experience. Thanks to everyone involved for their dedication and 5-STAR customer service. I will recommend this procedure to everyone who is interested. Thank you and God bless. Sincerely, Commander C.M., United States Navy Advanced CustomVue WaveFront at At we offer two types of procedures: Conventional and Advanced CustomVue WaveFront. While conventional LASIK and PRK have been revolutionary in vision correction, also offers the latest in laser vision correction technology; Advanced WaveFront CustomVue is a treatment that allows the surgeon to further customize the conventional procedures to your eyes. Higher-Order Aberrations Several types of visual imperfections, referred to as lower- and higher-order aberrations, exist within the eye and may affect your vision (the degree of these aberrations varies from person to person.) Previously, only lower-order aberrations (such as nearsightedness, farsightedness and astigmatism) could be measured and treated. Advanced CustomVue WaveFront WaveFront laser vision correction is an advanced form of excimer laser technology. Just like a fingerprint, each person s vision is unique. Before the recent advances in laser technology, surgeons would perform LASIK and PRK using standard measurements to correct vision and achieve excellent results. Now the new WaveFront technology can measure and correct the unique imperfections of each individual s vision in order to provide them with the potential to experience better vision than they had with glasses or contact lenses. This technology was originally developed for use in high-powered telescopes to reduce distortions when viewing distant objects in space. Now surgeons can identify, measure, and correct imperfections in the eye 25 times more precisely than with conventional methods used for glasses and contact lenses. Safety and Accuracy CustomVue WaveFront using Iris Registration technology takes safety and accuracy to the highest level. Twenty-four unique reference points in your eye are identified by the WaveScan, automatically transferred to the laser, and matched to your eye. The laser will compensate for any alterations in pupil size or eye position, ensuring optimum precision of treatment. LASIK and PRK can be performed using the WaveFront laser technology. Glare Halos Normal Now Advanced CustomVue WaveFront LASIK may help to further customize your treatment of these higher-order aberrations that in the past could not be adequately treated with glasses, contact lenses or conventional treatments. Higher-order aberrations may also have a significant impact on vision and have been linked to glare and halos which can cause more visual impairment at night than during the day. Your optometrist and surgeon will recommend the best procedure for you based on your unique prescription and aberrations your optical fingerprint The Technology of Advanced CustomVue WaveFront Together with WaveFront technology, our ActiveTrak 3-D eye tracker actively follows the tiniest movements of the eye in all three dimensions, keeping the laser centered to within 50 microns (about the width of a human hair) even if your eye moves during treatment. Due to demanding visual requirements and rigorous standards of vision, all service members in the US Navy who require laser vision correction are treated using the most advanced technology the VISX S4IR with WaveFront. Advanced CustomVue WaveFront Procedure 1. A WaveScan capture of your visual system is created which displays all of your higher-order abberations. 2. This information is automatically transferred to the laser platform and used when your eye is being treated. 3. Lower- and higher-order aberrations have been eliminated from your eye. WaveFront eliminates night vision problems and offers an improved overall quality of vision. WaveFront makes the difference! 14

Advanced CustomVue WaveFront Results The VISX S4IR excimer lasers have been used successfully to treat over 10 million eyes in the U.S. and over million eyes globally. Patients enrolled in a clinical study of the new WaveFront procedure showed that: Prior to surgery 80% of patients could not read the large E when viewed from feet without their glasses or contact lenses. Following surgery 100% of study participants could see well enough to drive a car legally without glasses or contact lenses.* Following surgery 98% of study participants achieved / vision or better.* Following surgery 70% of FDA study participants were /16 or better.* E H N D F N P T X Z U Z D T F D F N P T H P H U N T D Z N P X T Z F H The benefits of Advanced CustomVue WaveFront customized laser treatment include: WaveScan measures and calculates the imperfections in vision 25 times more precisely than standard methods, potentially giving better vision after treatment than before treatment with glasses or contact lenses. Improved quality and consistency of vision in different light levels. In a clinical study, four times as many people were very satisfied with their night vision after the procedure compared to their night vision before with glasses or contact lenses. Iris registration provides you with improved accuracy and clinical outcomes. IntraLase What is IntraLase? IntraLase is an alternative method of creating the protective flap, using a precision laser. For over ten years standard mechanical methods have provided excellent outcomes. However, through IntraLase precision laser technology we can now offer you an alternative to the mechanical method. How it works 1. The IntraLase laser is precisely focused to a point within the cornea, where thousands of microscopic bubbles are formed. 2. IntraLase enables the surgeon to precisely design and control many aspects of the flap its thickness, its circumference, and the angle of its edges effectively tailoring the protective flap to the eye. 3. In the second part of the procedure, the surgeon gently lifts the flap to allow for the excimer laser treatment. The procedure is complete when the protective flap is securely repositioned on its beveled edge. The Benefits IntraLase enables even faster healing and recovery rates than standard methods of creating the protective flap. IntraLase can improve the precision and visual results of LASIK, whether you choose to have a standard or CustomVue procedure. IntraLase allows surgeons to tailor the corneal flap for each individual patient and each individual eye. IntraLase is of particular benefit to patients with steep, flat, or thin corneas, some of whom are not eligible for LASIK/PRK surgery when performed with a standard microkeratome. Laser eye surgery has a complication rate of less than 1%. However, these complications are even further reduced by choosing the IntraLase precision laser procedure. *Advanced medical optics FDA clinical study 05 WaveFront is clinically proven to be superior to standard procedures in terms of vision potential 17

The Day of Your Procedure On the day of your procedure, allow a half day for your visit to the clinic. This ensures ample time for discussion and for any final measurements to be taken. You will be unable to drive after the procedure, so be sure to make arrangements for your transportation home. Since a local (not general) anesthetic is used, you can eat and drink normally on the day of your visit. Wear loose, comfortable clothing. Do not wear any make-up, creams, mascara, eyeshadow, eyeliner, deodorant, or perfume on the day of treatment. Contact lenses can change the shape of your cornea. Since laser vision correction treats your cornea, it is important to allow time for your cornea to return to its normal curvature by not wearing contact lenses prior to your consultation and procedure. You should stop wearing them for at least one week prior to treatment. If you wear rigid gas permeable lenses, you should stop wearing them one month prior to your procedure. If these guidelines are not followed, it could affect the results of your treatment. The success of your laser treatment depends on several factors, including the expertise of the surgeon and team and the advancement of the technology used. At, our priority is to bring you optimal clinical results while maintaining the highest level of safety. After Your Procedure There are two stages to your procedure: The surgical stage and the recovery stage. Both are just as essential to your outcome. With over 24 million successful procedures already performed worldwide, experience teaches us that how well you manage your recovery stage significantly affects your results. Your treatment will be most successful if you: Do Wear sunglasses if your eyes feel light-sensitive Use your prescribed eye drops Be gentle when washing your face and avoid using washcloths, etc. around the eye area Avoid smoky or dusty environments Relax and enjoy your new visual freedom Do Not Do not wear eye make-up for one week Do not swim under water for two weeks Do not use tanning beds for two weeks Do not participate in contact sports that could result in an eye injury for a minimum of one month. Do not rub your eyes 19

Frequently Asked Questions During your consultation, you will have the opportunity to spend ample time with an optometrist and counselor who can provide you with all the information you need to make an informed decision regarding your treatment. These are some of the more frequently asked questions from previous patients. Q.Will I be a good candidate for treatment? A. For the best possible results, we are able to treat most people over 18 years of age who have had a stable prescription for 12 months. A consultation with an optometrist will determine your suitability for treatment based on your personal prescription, lifestyle, and expectations. Q.Why is laser vision correction less expensive at than at many of its competitors? A. Since is a world leader in laser eye treatment, we benefit from economies of scale and in turn pass that savings on to you. Other laser providers must charge more to make up for their lower volume. Furthermore, employs optometrists and surgeons instead of having to refer our patients elsewhere. Q.Will I ever need glasses again? A. Fewer than 1 in 1,000 patients requires glasses for distance after laser vison correction. The majority of those who do require them only for specific tasks. However, as we grow older a condition called presbyopia usually develops, and most people will eventually require reading glasses, whether they have treatment or not. However, new treatments that may eliminate the need for reading glasses are currently undergoing clinical trials. When my husband proposed, I was determined not to be wearing my glasses on my wedding day, so I went for laser vision correction at. I wish I had done it sooner, as it really is amazing to be able to see without my glasses. It is the best thing I have ever spent my money on! M. W., Business Analyst Q.Does offer patient financing? A. Yes. At, we try to make laser vision correction affordable for every patient. We are committed to preventing finances from becoming a barrier to laser vision correction. 13 Frequently Asked Questions Q.Do I need to leave my contact lenses out prior to treatment? A.Yes. Depending on the type of lenses you wear, you will need to leave them out (and wear glasses) prior to consultation/treatment. Contact lenses may distort the cornea and lead to inaccurate measurements and treatments. Lens Type Soft Hard/GP Prior to assessment consultation Leave out for 1 day (minimum) Leave out for 1 week (minimum) Prior to treatment Leave out for 1 week (minimum) Leave out for 1 month (minimum) Q.Will I experience discomfort during or after treatment? A. We use anesthetic eye drops that completely numb your eye so you remain comfortable throughout the procedure. After LASIK treatment, patients may feel some grittiness, light sensitivity and watering for a short while. With PRK treatment, you may feel some mild discomfort after the treatment, potentially in the first hours. Q.What if I close or move my eye during my treatment? A. Drops are used to eliminate the blink reflex, and an eyelid holder keeps your eye open. There is a very sophisticated eye tracker which follows your eye as it moves during your treatment. If your eye moves out of line, the laser tracks the slightest of movements and stops pulsing. Q.How much can I expect my vision to improve? A. The final result depends on your original prescription. Although we are unable to guarantee you / vision, we can reasonably predict outcomes based on the original levels of prescription.
