3 Abstract This white paper outlines a vendor-neutral medical image archive solution that integrates IBM Scale Out Network Attached Storage (SONAS) and TeraMedica Evercore running on an IBM x86 server. It describes the configuration that was used to test the TeraMedica Evercore software with IBM SONAS. This was a minimal configuration to ensure that everything would function with even the smallest of configurations, and is available as a bundled solution in the channel from participating IBM value-added distributors and their network of IBM Business Partners. Introduction In the present clinical environment for health care providers, most of the medical imaging data is stored in individual silos. Each of those silos must be individually maintained and managed. This type of storage infrastructure has the tendency to cause a duplication of some or all of the following resources. Available disk space IT resources Environmental resources When utilizing an infrastructure that contains individual silos, it becomes increasingly difficult for a physician to view the media from each of the different silos. In most cases, the media has to be obtained from each of the individual silos. IBM and TeraMedica have now teamed up with a joint offering that addresses these data storage and usage needs. IBM SONAS delivers seamless scalability for the high performance and massive capacity that healthcare providers require. Its distributed architecture reduces management complexity, speeds up processes, and eliminates any single point of failure (SPOF) that would impede data availability. The TeraMedica Evercore Clinical Enterprise Suite provides scalability and vendor-independent interoperability to bridge the highly-specialized and departmentalized structures of modern healthcare organizations. Modular solutions connect and manage the healthcare system s digital image infrastructure from the imaging device to the picture archiving and communications systems (PACS) and from electronic health records to the clinical desktop. Together, the comprehensive IBM and TeraMedica capabilities support the complete collection, use, and archiving of digital records and clinical workflows with ubiquitous sharing that can enhance complex relationships in today s advanced medical environments. When utilizing the TeraMedica Evercore software in conjunction with IBM SONAS, the silos are removed. Evercore provides a single interface for all Digital Imaging and Communications in Medicine (DICOM) data to be archived to. Evercore can also be configured to work with departments that utilize non-dicom data. Utilizing this type of configuration allows for the management of just one storage platform across several departments. This reduces the need for redundant free space to be available across multiple silos, the need to manage individual storage platforms, and the overall environment needs. Lab configuration This section outlines the lab configuration at the time of running the test. 1
4 Figure 1: Illustration of configuration used The hardware definitions are as follows: Client server - This is the server where all of the DICOM data is sent from (simulating a PACS server in a customer environment). Application server This is the IBM System x 3650 M3 server, where the TeraMedica Evercore application resides Database server This is the IBM System x3650 M3 server, where the Microsoft SQL database resides. Cisco Nexus 5020 This is a 40-port Ethernet switch that provides 10 Gigabit Ethernet (GbE) connectivity between all the devices in this test configuration. SONAS This is the IBM storage platform. IBM SONAS is a network-attached storage (NAS) based storage appliance that internally utilizes IBM General Parallel File System (IBM GPFS ), and is capable of presenting multiple NAS mounts through multiple NAS protocols. A SONAS can independently scale capacity (up to 14 PB, all under a single global name space for ease of management and implementation) or performance by adding additional interface nodes (up to 30 nodes) 2
5 Figure 2: Block diagram representing data flow with the solution Servers Three individual Intel based IBM System x3650 M3 servers, with the following standard configuration have been used. Intel Xeon CPU X5650 at 2.66 GHz (two processors) 64 GB of memory 10 Gb Ethernet Adapter Microsoft Windows Server 2008 R2 (Service Pack 1) Microsoft SQL Server The disk configuration for each of the servers varied slightly. The following configurations have been used for each of the servers. Client Server Three 300 GB SAS drives configured as a RAID 1E. This array contained the operating system (OS) as well as the tools used to run the tests. Application Server Two 146 GB SAS drives configured as RAID 1. This array contained the OS as well as the Evercore software. Database Server Two 146 GB SAS drives configured as RAID 1. This array contained the OS as well as the Microsoft SQL Server 2008 R2. This server also had four 73 GB SAS drives configured as RAID 0. This array contained the SQL database. 3
6 Storage The IBM SONAS includes the following configuration. Two internal Ethernet switches Two internal InfiniBand switches Six interface nodes, each with a dual port 10 GbE Converged Network Adapter (CAN) One management node One storage pod consisting of two storage nodes and two enclosures of 60 hard drives each Software tools used TeraMedica TestSend TestSend is a TeraMedica-developed DICOM PACS simulator that was written for the specific purpose of performing DICOM functionality and load testing against DICOM image archive systems pertaining to Composite Store (C-Store) operations. It also captures client-side performance metrics (total duration in seconds, image count, data sent in MB, number of images per second, MB sent per second, and average file size for each association) into a log file. TestSend is capable of creating multiple simultaneous threads for sending and receiving data. TeraMedica Query/Retrieve Service Class User (QRSCU) QRSCU is a TeraMedica-developed DICOM PACS simulator that was written for the specific purpose of performing DICOM functionality and load testing against DICOM image archive systems pertaining to Composite Find (C-Find) and Composite Move (C-Move) operations. It also includes a C-Store listener that is used to process all C-Find and C-Move requests. It also captures client-side performance metrics (total duration in seconds, image count, data sent in MB, number of images per second, MB sent per second, and average file size for each association) into a log file. Testing performed Benchmark 1: Client-side baseline DICOM stream C-Store throughput determination The test team performed n (where n refers to the maximum number of TestSend instances, which consisted of six separate tests of 1, 5, 10, 15, 20, and 25 simultaneous threads) runs of TestSend storage, query, and retrieve scripts from a single, random client. Each run increased the number of TestSend threads on the machine to n in order to establish the maximum number of TestSend threads that can be run on each machine before saturating the client s network interface. Each of these runs began with a clean database. Benchmark 2: Client-side baseline DICOM stream C-Move throughput determination The test team performed n (where n refers to the maximum number of TestSend instances, which consisted of six separate tests of 1, 5, 10, 15, 20, and 25 instances) runs of the TestSend storage, query, 4
7 and retrieve scripts on the single application server. These tests began with a clean database to establish baseline metrics for each server. Benchmark 3: Client-side baseline DICOM stream throughput determination The test team ran 10 threads of TestSend storage clients. Also, simultaneously ran five threads of QRSCU retrieval clients. Each run began with a clean database and establish system C-Store and C-Move metrics. Benchmark 4: Client-side baseline DICOM stream C-Store throughput determination on a loaded database The test team performed n (where n refers to the maximum number TestSend instances, which consisted of six separate tests of 1, 5, 10, 15, 20, and 25 simultaneous threads) runs of the TestSend storage, query, and retrieve scripts on the single application server. Each run increased the number of TestSend threads on the machine to n in order to establish the maximum number of TestSend threads that can be run on each machine before saturating the client s network interface. Benchmark 5: Client-side baseline DICOM stream C-Move throughput determination on a loaded database The test team performed n (where n refers to the maximum number of TestSend instances, which consisted of six separate tests of 1, 5, 10, 15, 20, and 25 instances) runs of the TestSend storage, query, and retrieve scripts on each server machine sequentially from a single, random client. The estimated duration is 1 hour and 30 minutes. Benchmark 6: Client-side baseline DICOM stream throughput determination on a loaded database The test team ran 10 threads of TestSend storage clients. Also, simultaneously ran five threads of QRSCU retrieval clients. Additional information: Study size for storage ranged from 10 MB to 150 MB, with a 30 MB average. Studies were stored and retrieved in a random sequence of modalities and sizes. TestSend sent images to Evercore uncompressed in the DICOM default transfer syntax.(implicit- VR Little Endian) QRSCU retrieved images from Evercore compressed in the Evercore storage format, JPEG Lossless Process 14 (which is the default transfer syntax for Lossless JPEG image compression) Planning the IBM vendor-neutral archive (VNA) solution deployment The SONAS appliance can be configured as a complete Enterprise NAS solution, which can support multiple purposes, including, but not limited to, archive, databases, virtual machines, and so on. The SONAS storage that comes with the IBM VNA solution is configured only for use with the VNA application. 9 Summary The testing concluded in about 3 days without any performance problems. The IBM SONAS storage performed as expected, and even when tested with high simultaneous thread counts, the minimal configuration performed extremely well. This testing has shown that the IBM SONAS storage in conjunction with IBM System x3650 M3 hardware and the TeraMedica Evercore software can provide a solid solution for even small-scale customers.
