3 What is Lockout / Tagout? Specific practices and procedures used to safeguard individuals from the unexpected energization or startup of machinery and equipment or the release of hazardous energy during service or maintenance activities. Regulated by DOC under OSHA s Control of Hazardous Energy Standard. Waukesha County Technical College 3

4 Employee Classifications Authorized Employee - A person who locks out or tags out machines or equipment in order to perform servicing or maintenance on that machine or equipment. Affected / Other Employee - An employee whose job requires him/her to operate or use a machine on which servicing/maintenance is being performed under lockout or tagout, or whose job requires him/her to work in an area in which such servicing or maintenance is being performed. 5 Facilities Worker I - II Authorized Employees Not allowed to perform lockout/ tagout Facilities Worker III Authorized to lockout ballasts, outlets and switches Facilities Worker IV, Facilities Specialist, Facilities Lead Worker, Director of Facilities and Assistant Facilities Director Authorized to lockout equipment they are certified to work on Waukesha County Technical College 5

6 Covered Applications Control of energy during servicing and/or maintenance of machines and equipment. Normal production operations are not covered by this standard Servicing and/or maintenance which takes place during normal production operations is covered by this standard only if: An employee is required to remove or bypass a guard or other safety device; or An employee is required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. 7 Applications Not Covered Work on cord and plug equipment where the plug is under the exclusive control of the employee performing the servicing or maintenance. Hot tap operations involving transmission and distribution systems for substances such as gas, steam, water or petroleum products when they are performed on pressurized pipelines, provided that the employer demonstrates that continuity of service is essential; shutdown of the system is impractical; and documented procedures are followed, and special equipment is used which will provide proven effective protection for employees Waukesha County Technical College 7

9 Energy Control Procedures Procedure outlines steps for: Shutting down, isolating, blocking, and securing machines to control energy sources Placement, removal, and transfer of lockout devices or tagout devices, Testing a machine to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control. 10 Energy Control Procedures EXCEPTIONS: No potential for stored, residual energy or re-accumulation of stored energy after shut down Single energy source which can be readily identified and isolated Locking out energy source will completely de-energize and deactivate the machine Single lockout device will achieve a locked-out condition; Lockout device is under the exclusive control of the authorized employee performing the servicing/maintenance Servicing / maintenance does not create hazards for other employees There have been no accidents involving the unexpected activation or re-energization of the machine during servicing/maintenance. However, energy sources of these machines are still required to be isolated and locked out prior to any service/maintenance work. Waukesha County Technical College 10

13 Isolating Energy Sources 1. Only trained, authorized personnel can perform lockout / tagout. 2. Notify affected employee(s) that servicing or maintenance is required on the machine and it must be shut-down and locked out. 3. The machine must be turned off and the operating controls placed in the off position. 4. Apply lockout devices to all required energy sources (see machine specific procedure). A. Each authorized employee shall place his/her own personal lockout or tagout device on each of the energy isolating device(s). B. Lockout devices when used must be affixed in a manner that will hold the energy isolating devices in a "safe" or "off" position. C. Stored or residual energy must be released or restrained by methods such as grounding, repositioning, blocking or bleeding down. Waukesha County Technical College 13

14 Isolating Energy Sources 5. Verify the piece of machine is de-energized. 6. Operate controls to make sure it will not operate. Once you have verified the machine will not operate make sure to return the controls to the neutral or OFF position. 7. The machine is now locked out and the service or maintenance may be performed. 15 Restoring Machines to Operation 1. Inspect the work area and the immediate area around the machine to ensure tools and nonessential items have been removed and that the components on the piece of machinery are operationally intact. 2. Notify the affected and other employees in the area of impending restart and make sure they are safely positioned away from the equipment 3. Verify the controls are in the neutral or OFF position. 4. Remove your lockout devices and energize the machine. The removal of some forms of blocking may require energization of the machine before safe removal of the blocking devices. 5. Notify the affected employees that the service or maintenance is complete and the machine is now ready to operate Waukesha County Technical College 15

16 Testing or Repositioning Machines 1. Machine must be cleared of tools and materials. 2. Employees must be removed from the machine area. 3. Lockout or tagout devices may then be removed. 4. Authorized employees may then proceed to energize and test or position the machine. 5. Following testing or positioning, all systems must be deenergized and energy control devices reapplied to continue the servicing and /or maintenance. Waukesha County Technical College 16

17 Group Lockout 1. Notify affected employee(s) that servicing/maintenance is required and the machine will be shut-down and locked out. 2. Turn-off the machine and place operating controls in the off position. 3. Each authorized employee performing service/maintenance on should isolate and lockout the required energy sources. 4. Once all required energy sources are locked out, the group of authorized employees should verify each authorized employee in the group has secured a lockout to the required energy sources. 5. Authorized employee in charge will verify the equipment or machine is de-energized. per the machine specific procedure. 6. Authorized employee in charge will operate controls to make sure machine will not operate. Once you have verified the machine will not operate make sure to return the controls to the neutral or OFF position. 7. Machine is now locked out and service/maintenance may be performed. 18 Shift / Personnel Change 1. Notification / communication of lockout / tagout transfer between authorized employees at the machine. 2. Removal /replacement of locks from the energy sources. 3. In the event that maintenance is not completed by the end of a shift and no authorized employees will be taking over, the lockout device of the last authorized employee performing the work should stay attached to the affected energy sources. 19 LO/TO Device Removal Only the authorized employee who applied the lockout or tagout devices is authorized to remove it. 1. Identify and contact the authorized employee who applied the lockout device. 2. Determine why the lockout is still on the piece of machinery. 3. If unable to identify or contact the authorized employee, then contact Supervisor. It will be their responsibility to determine the status of the job. Only after the above steps have been taken may the Supervisor authorize the removal of the lockout or tagout device. Waukesha County Technical College 19

20 Periodic Inspections Conducted at least annually Ensured machine specific procedures are accurate Employees are familiar with their responsibilities Deviations or procedure deficiencies are corrected Person conducting inspection should be an authorized employee not involved in the energy control procedure being inspected Authorized employees not able to adequately perform the periodic inspection, will be required to successfully complete re-training Waukesha County Technical College 20

21 Lockout vs. Tagout If an energy isolating device is not capable of being locked out, then tagout procedures shall be utilized. If an energy isolating device is capable of being locked out, then lockout procedures shall be utilized. After January 2, 1990, whenever replacement, major repair, renovation or modification of a machine is performed, and whenever new machines or equipment are installed, energy isolating devices for such machine shall be designed to accept a lockout device. 22 Lockout / Tagout Equipment Durable and capable of withstanding the environment Standardized Identifiable, identifies the employee applying the devices Substantial enough to prevent removal without the use of excessive force Only used for lockout purposes Waukesha County Technical College 22

23 Tagout Equipment Constructed so the environment will not cause deterioration. Standardized in print and format. Substantial to prevent inadvertent or accidental removal. Secure attachment means (i.e. nylon cable tie) Warn against hazardous conditions if the machine or equipment is energized. Include a legend such as: Do Not Start, Do Not Open, Do Not Close, Do Not Energize, or Do Not Operate. Waukesha County Technical College 23

24 Contractors Comply with all aspects of the College s Lockout/Tagout Program. Copies of program will be provided by the EHS Dept upon request. Ensure employees are trained and authorized in lockout tagout. Comply with any machine specific lockout procedures Violations Contractor and employee will be notified and/or may be removed from the jobsite. Responsibility of the College employee supervising the Contractor to ensure compliance. All have responsibility to report! Waukesha County Technical College 24
