1 New Employee Safety Orientation The Washington State Industrial Safety and Health Act (WAC ) requires that this information be provided to all new employees including hourly, temporary and student employees. The plan is posted on the HCDE web page in MyHCDE and in a binder in HCDE main office near the Fiscal Specialist s desk in 423 Sieg Hall. Evacuation procedures: A Building Evacuation Plan (floor plan) posted on each floor and on the Safety Bulletin Board in 422 Sieg Hall. The floor plan locates safety equipment and exit pathways for each office. Please note your office location and suggested evacuation route(s) in case of fire, bomb threat, or earthquake. See pages in HCDE Safety Plan book. HCDE assembly point for Sieg Hall is the HUB lawn (north & east of Sieg Hall). Methods for accounting for faculty, staff, students, visitors: 1. Conduct a headcount by using a checklist to account for all occupants. 2. Report any missing persons on your list and their last known location. 3. Give special attention to needs of persons with disabilities. 4. Phone, or send a runner to, UW Emergency Operations area in the UW Police Department on Boat Street or HUB Emergency Station if there are injured or if there is a dangerous situation. 5. Begin implementation of your family emergency plan, if possible. HCDE areas of refuge for mobility-impaired occupants are the landings inside the stairwell on each floor. 2
3 3. Local Fire Alarm Signaling System Employees must leave the building immediately upon hearing a fire alarm, closing doors behind them. When employees discover a fire they should first pull the nearest fire alarm pull station and then exit the alarmed area. If possible, employees should follow up with a telephone call to from a safe location to provide more details. On Campus: The activation of a fire alarm pull station also sends a signal to the UW Police and Seattle Fire Department showing the location of the emergency. Portable Fire Extinguishers You may use a portable fire extinguisher only if: you have been trained to use them, the fire alarm has been sounded first, the fire is small (waste basket size), and you have a clear evacuation route. 4. Accidents: a. Medical Emergencies: All medical emergencies must be reported to the nearest Emergency Medical Services (EMS), usually 911. Our department uses the following method to summon EMS help. UW Police 911 b. Report form to supervisor and EH&S: All accidents and near misses must be reported to the employee s supervisor and EH&S as soon as possible. Near misses are valuable opportunities to correct unsafe situations, which under slightly different circumstances, would result in serious injury. A report may be filled out by the employee, the supervisor, or both using the Online Accident Reporting System (OARS) at: Copies of this department s completed forms are distributed to the following people: Lisa Yamasaki, Safety Committee Team member and DJ Miller, Administrator. c. Investigation: All accidents and near accidents must be investigated by the supervisor who then summarizes the details and corrective measures in the above report. EH&S and the department s organizational safety committee review the report. Assistance from EH&S is available by calling Reporting Unsafe Conditions and Acts Employees are encouraged to report safety concerns to their supervisor. If employees do not feel they can do this, or have done so and do not feel the problem has been resolved, they may discuss the situation directly with their safety coordinator or safety committee representative. Assistance from EH&S is available, if needed, to resolve a problem. Safety problems may be reported online using OARS as you do for accidents/incidents. 6. First Aid 3
4 HCDE s First-Aid Kit is located above the student assistant desk (third upper cabinet from front) in 423 Sieg Hall. First-Aid Kits are inspected periodically so they can be restocked before running out of an item. If another employee is injured, assist them with First-Aid, or contact UW Police. Do not hesitate to call 911 in case of an emergency. HCDE First-Aid/CPR certified staff, located in Sieg Hall are listed on Reference Information Page. First-Aid/CPR certification classes are offered by UW Environmental Health. See Safety Bulletin Board in 422 Sieg Hall or Environmental Health web page [ 7. Hazard Communication (Chemical Safety) (Worker Right-to-Know, HazCom) Hazard hand-out materials used in our offices include office supplies (e.g. white out, liquid paper, rubber cement, spray glue, etc.), cleaning products (e.g. spray window and desk cleaners), and toner for printers, Fax and copier. Material Safety Data Sheets (MSDS) on these products are kept in the back pocket of the Safety & Health Plan book. Be sure to use these products with adequate ventilation. Ask your fellow employees if they have (health) problems with the product you are about to use. (Note: 1 or more of our employees are chemical sensitive.) If any hazardous materials emergencies, such as spills or releases too big for you to clean up report it to HCDE s Administrator and/or: What Who Phone Small Spills General Campus Large Spills or releases General Campus Worksite Warning Signs and Labels/ Protective Equipment No worksite warning signs or protective equipment are needed or used in HCDE offices. 9. Safety Bulletin Board Our departmental safety bulletin board is used for posting DOSH (formerly WISHA) posters, safety notices and safety newsletters. Safety committee minutes, training schedules, safety posters, accident statistics, and other safety education material may also be posted. The bulletin board is located in 422 Sieg Hall where all employees, students, and visitors can see it (WAC ). 4
5 The HCDE departmental safety bulletin is currently integrated with the Union bulletin board. The bulletin board must display the following posters:- UW HazCom Poster State Labor and Industries Posters "Job Safety and Health Protection" "Notice to Employees" "Your Rights as a Worker" Other safety notices, newsletters, safety and health committee minutes, etc. will be posted here also. 10. Employee Safety and Health Training The following checklist indicates the safety and health training classes the new employee is required to take for their job. Recommended classes could also be marked but priority must be given to arranging the required health and safety training classes. New employees should register for EH&S courses as soon as they are aware of their start date since many required courses fill early. 5
