The federal Trade Commission (FTC), the New York State Department of Health and the New York City Department of Consumer Affairs require funeral providers to make certain disclosures on the General Price List. These disclosures are similar, but are not exactly the same. This General Price List contains the disclosures required by the New York State Department of Health and the New York City Department of Consumer Affairs. *The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral service and merchandise you selected. *This list may not include prices for certain items you select, such as cemetery charges or crematory services, flowers and newspaper notices. The estimate or final prices for those items will be shown on the itemized statement describing the funeral services and merchandise you selected. We charge you for our services in buying certain items. *Your authorization is required for a funeral home to obtain custody of a body, and a body must be released promptly upon request. *The provider of funeral services is required to give price information in person and over the telephone. * A provider of funeral services is prohibited from representing that the deterioration of human remains can be prevented for more than a limited time by embalming, caskets, vaults or outer interment receptacles. 2 ALTERNATIVE SERVICES: The following charges for direct burial and direct cremation do not include additional items of services, merchandise, facility use and livery that the customer may wish to select. PRICES RANGE FROM DIRECT CREMATION TO $ 3, $ 3, ( Direct cremation is the disposition of the remains without any attendant rites or ceremonies as is schedule at the convenience of the funeral home.) Direct cremation includes the local transfer of remains to the funeral firm, the services of the funeral director and staff, securing of necessary authorizations, basic local transportation to the crematory, an alternative container for cremation and the return of the cremated remains to the funeral firm. If you want to arrange a direct cremation, you may use an unfinished wooden box or an alternative container. Alternative containers can be made of heavy cardboard pressed wood or composition materials (with or without an outside covering), or may be pouches do canvas or other materials. We offer 2 basic package services for direct cremations: The first package includes the local transfer of remains to the funeral firm, the services of the funeral director and staff, securing the necessary authorizations, basic local transportation to the crematory, an alternative container for cremation, and the return of the cremains to the funeral firm. $ 3, Direct Cremation with a cremation container supplied by the purchaser. (All such containers must meet any applicable state or crematory requirements.) $ 3, The direct cremation prices do not include the crematory charge. PRICES RANGE FROM DIRECT BURIALS: TO $ 3, $ 3, ( Direct Burial is the disposition of remains without any attendant rites or ceremonies and scheduled at the convenience of the funeral home) We offer two basic package service for direct burials: The first package includes the local transfer of remains to the funeral firm, the services of the funeral director and staff, securing of necessary authorizations, basic local transportation to the $ 3, cemetery, and an alternative container. The second package includes the services as described above but with burial container supplied by $ 3, the purchaser. The direct burial prices do not include cemetery charge. TRANSFER OF REMAINS TO THE FUNERAL FIRM INCLUDING: PERSONNEL, EQUIPMENT AND VEHICLES. Local within 10 mile radius $ Charge per mile beyond local range $ 4.00 Additional Assistance $ Securing Non Electronic Death Certificate $ PREPARATION OF REMAINS: Embalming ( including use of Preparation Room) $ Embalming of autopsied remains $ In New York State, embalming is not required by law if you do not want embalming, you have the right to choose an arrangement which does not require you to pay for embalming such as direct cremation or direct burial. If you select certain funeral arrangements such as viewing or an open casket funeral, embalming may be required by the funeral firm. Page 2 PJCC GPL 2012

3 OTHER PREPARATION (Excluding Embalming) Preparation of the remains, unless selected, is not required for direct cremation or direct burial. However, this charge will not be added to the cost for direct cremation, direct burial, forwarding of remains or receiving of remains because the prices for those include this charge. 5 FORWARDING REMAINS TO ANOTHER FUNERAL FIRM: This charge includes the transfer of the remains to funeral firm, staff services, securing necessary authorizations and local transportation to the point of shipment. The charge does not include shipping charges or additional items of services, $ 3, merchandise, facility use, and livery that the customer may wish to select. Transportation beyond local point of shipment will be billed either as livery or common carrier charges depending upon the forwarding location. Local point of shipment is up to or within 10 miles. RECEIVING REMAINS FROM ANOTHER FUNERAL FIRM: This charge includes the basic transportation of the remains to the funeral firm and/ or place of local disposition staff services, securing necessary authorizations. The charges do not include additional items of services, merchandise, facility use, $ 3, and livery that the customer may wish to select. Transportation beyond local point of shipment will be billed as livery charges. Local place of disposition is up to or within 10 miles CASH ADVANCES: Services and merchandise provided as cash advance items will be billed at the same amount paid by the funeral firm.
