1 Request for Quotation Bridge Construction The Comox Valley Regional District (CVRD) is seeking quotations from contractors for the construction of two footbridges and some chainlink fencing at Masters Greenway and Wildlife Corridor near Courtenay in the Puntledge - Black Creek (Electoral Area 'C') of the Comox Valley. The services required under this request for quotation (RFQ) are described in the attached scope of services and the contractor, by submitting a quotation, undertakes to provide the services for the price quoted and as specified. Quotations clearly marked Bridge Construction will be received at the office of the Comox Valley Regional District at 600 Comox Road, Courtenay, BC V9N 3P6, until 2:00 p.m. Pacific time on Tuesday, April 14, All prices must be quoted in Canadian dollars with taxes shown as a separate item. Quoted prices are to remain valid for a period of 60 days from date of submission. Written quotations, which should include the attached quotation form completed and signed, may be submitted by mail or in person to 600 Comox Road, Courtenay, BC V9N 3P6 or by to communityservices@comoxvalleyrd.ca. In order to be considered, the contractor should ensure the quotation is received by the CVRD before the submission deadline. A contract will be issued to the contractor or contractors complying with the provisions of the RFQ offering the best value to the CVRD. The following criteria will be used to select the quotation offering the best value: a) Contractor s reputation, qualifications, equipment and experience; b) Ability to meet the requirements or specifications provided for in the RFQ; c) Lowest price to the Comox Valley Regional District Contract award is subject to regional board approval of 2015 financial plan in March. Contractors obtaining the RFQ documents from on online source assume sole responsibility for checking back to determine if any addenda have been posted that may affect this RFQ and any response they wish to submit for consideration. 2 Request for Quotation Bridge Construction Scope of Work Background The Comox Valley Regional District (CVRD) manages and operates the trails within Masters Greenway and Wildlife Corridor and is seeking the services of a contractor to construct two footbridges. Masters Greenway features forested trails with walk-in access from Powerhouse Road, Mellifera Place and First Street. There is no vehicle access to the work sites. It may be possible to use an ATV and trailer to carry supplies at least part way to the work sites. Contractors are welcome to submit a quotation for all or part of the services. The CVRD may award a contract to more than one contractor in order to ensure the most efficient delivery of the services. 1. GENERAL INFORMATION The successful contractor(s) will be required to provide comprehensive commercial general liability insurance in an amount not less than $3,000,000 per occurrence and provide a certificate of insurance showing the CVRD added as an additional insured. The successful contractor(s) will also be required to provide evidence of automobile liability on all vehicles used in the provision of the services in an amount not less than $2,000,000 each. Contractors are required to: 1) be registered with WorkSafeBC and maintain coverage for their employees 2) serve as prime contractor for the work site 3) comply with all applicable health and safety regulations 4) sign the CVRD safety agreement; and 5) provide to the CVRD prior to the start of work both evidence of WorkSafeBC coverage and copies of safe work policies and procedures The CVRD will: 1) arrange all permits and site inspections 2) arrange for a WCB clearance letter 3) provide signage and safety/security fencing for Contractor to install, and advise users of temporary trail closure 4) ensure the project engineer visits the project as required 5) ensure CVRD staff are available as needed 6) supply a portable toilet upon request Page 2

8 The Contractor shall: Request for Quotation Bridge Construction Construction Specifications 1. Close those trails leading directly to the bridges to ensure public access to work sites is restricted to prevent injuries to people that may attempt to use the trail and bridges during construction. 2. Develop and follow a work plan with excellent practices for avoiding erosion and minimizing sediment release. Commence work at the site only when the creek has gone dry. Even if the creek is dry, prevent debris from falling into in the creek channel. Any debris that falls into the channel will need to be removed without disturbing the creekbed. No materials or equipment are to be dragged through the creek channel. Complete the project quickly. Ensure no debris enters the creek during removal of the structure. Review the Best Management Practices attached to this RFQ and at 3. Take apart the existing bridges. All lumber should be removed from the greenway and disposed by the contractor. The logs beneath the decking can be moved to an unobvious location within the forest in the greenway. 4. Supply, deliver and install all materials. 5. Take apart the existing chainlink fence at the top of the bank as needed to reposition the fence in a wide U shape as marked on the photo attached to this RFQ and as marked in the greenway. Remove the post protruding in front of the fence. 6. Be registered with the WorkSafeBC and maintain coverage for the duration of the project. Any approved sub-contractors must also be registered or covered under the contractor s coverage. 7. For the purposes of the Workers Compensation Act, and for the duration of this work be the prime contractor for the worksite, and do everything that is reasonably practicable to establish and maintain a system or process that will ensure compliance with the Act and its regulations, as to ensure the health and safety of all persons at the worksite. As designated prime contractor, prepare a safety plan outlining safe work procedures for the crew and subcontractors. Potential hazards include: vehicle traffic along public roads and streets, moving equipment, eroding bank with steep drop-offs and slumping soils, and falling trees and branches. The contractor shall assess and deal with these hazards prior to beginning work and throughout the project as needed. This includes hiring a faller or tree climber to modify any trees that pose a hazard to workers or equipment. 8. Comply with any issues brought to their attention during site visits by CVRD staff or consultants. 9. Note that Onsite Engineering of Campbell River is planning to have an engineer inspect the bridges upon completion. They will attend the site on other days if pre-arranged by the CVRD. 10. Not receive compensation for any losses the contractor may incur as a result of work being delayed or suspended for reasons that are beyond the control of the CVRD. This includes being restricted from the work site due to water being present in the creek, extreme fire hazard or abnormal weather conditions. Page 7

Be aware that should an activity or event occur which contravenes these terms, the CVRD may issue a Stop Work order directing the immediate suspension of all or a portion of the activities of concern, and may undertake or order remedial measures to be conducted. The cost of any work stoppages and/or remedial works shall be the responsibility of the contractor. 12. Attain Substantial Performance of the Work by no later than September 10, Page 8

15 General Best Management Practices Weather Work during dry weather conditions when there is no water at the work site. Follow rainfall shutdown procedures if the intensity and duration of rain is such that there is a risk of erosion and sediment transport: Create temporary settling ponds, swales and/or sumps as required to control water at the site. Avoid leaving piles of loose soils exposed to rain in close proximity to the ditches. Vegetation Minimize the footprint of all project works. Do not cut or remove vegetation unless required. Prune the lower branches from trees to improve sightlines instead of removing the entire trees. Top hazard trees at 3 5 m or higher, rather than completely removing them. Plants should be salvaged and replanted after construction. Vegetation in adjoining areas is not to be disturbed. Retain high tree stumps near ditches, when possible, to promote re-sprouting, soil stability and maintain habitat. Retain large woody debris and the stubs of large diameter trees where it is safe to do so. These are important for wildlife populations. Re-vegetate exposed areas quickly to prevent surface erosion and promote stability. Use a combination of grass seed, straw and replanting of salvaged vegetation. Fall trees away from watercourses. Erosion &Sediment Control Develop a work plan that minimizes traffic up and down unstable soils. Complete the works as quickly as possible once they are started. Keep equipment away from open water and particularly soft ground. Operate machinery from the top of bank. Divert flows to vegetated areas where flows can slowly infiltrate. Create an off-line detention or retention ponds. Begin work at the upper end of the drainage system so that the existing downstream vegetation cover can help to filter sediments. Install temporary sediment fencing. The fence may be constructed of silt fencing or geotextile fabric staked into the ground. Grade disturbed areas to a stable angle of repose. When material is moved offsite, dispose of it in a manner that prevents its entry into any watercourse, floodplain, ravine or wetland. Brief all crew members on the site specific erosion controls. Complete post-construction monitoring. Ensure revegetation meets full survival. Remove any remaining sediment and erosion control measures (i.e., silt fence). Ensure all equipment, supplies, and non-biodegradable materials have been removed from the site. Fuels and Hazardous Materials The accidental release of petroleum, oils, hydraulic fluids, lubricants, concrete additives, anti-freeze or other hazardous materials onto land surfaces or into waterbodies is an offence under the Federal Fisheries Act and may result in degradation of habitat quality and could be a threat to human health. Environmental protection procedures for handling and storage of fuels and hazardous materials include the following items: 1. Ensure all construction equipment used in conjunction with the project is in good repair, has undergone a recent mechanical inspection and shall be inspected daily to ensure it is free of leaks Page 11
16 which could release fuel, oils, hydraulic fluids, antifreeze or other substances deleterious to aquatic life and to the marine environment. Any leaks shall be reported immediately and repaired prior to continuing work. 2. A spill kit of appropriate capacity will be on hand at all times gas powered equipment and tools are in use during construction. 3. All identified spills will be cleaned up immediately, and contaminated soils and vegetation will be removed for appropriate disposal. 4. Refueling of equipment is to occur only at designated fuelling stations and located at least 30 m from all waterbodies. 5. All fuel, chemicals, and hazardous materials will be clearly marked. 6. Pumps and jerry cans are to be placed on poly sheeting and sorbent pads to contain spills. 7. All equipment maintenance with the potential for accidental spills (e.g., oil changes, lubrications) will be done on a designated area at least 30 m from any waterbody. Tarps should be laid down prior commencement of work to facilitate clean up. 8. In the event of a spill, the following guidelines should be followed: - Spills to the receiving environment are to be reported to the BC Provincial Emergency Program ( ) if they exceed the reportable limits (e.g. 100 liters of fuel or oil). - Apply sorbent pads and booms as necessary. Dispose of all contaminated debris in an approved disposal site. 9. Cut treated timber at least 5m from the bridge site and watercourse. Page 12
