2 Cataract surgery has evolved over the years and is one of the safest and most successful procedures performed today. There are choices to consider in how your surgery is performed and what technology is used to give you the visual outcomes you desire. The goal of this cataract surgery booklet is to describe choices you can make regarding your procedure. Please review all the options and come prepared to your surgical consultation with what option you feel best suits your goals, needs, and budget. Our goal at Moyes Eye Center is to provide a premium cataract experience to all our patients undergoing cataract surgery regardless of what option you choose. These cataract options have options to improve your vision with and without glasses. We recommend you ask yourself how important is it after surgery to be less dependent on glasses? Your doctor will discuss which options are best for you. It is important to understand that the upgrade cost associated with the options outlined within this booklet are not covered by your medical insurance. We recommend that you check your insurance for any copays, coinsurance or deductibles that will be due as part of your surgery. YAG laser capsulotomy is a laser polishing of the lens implant that is needed in about two thirds of patients following cataract surgery. The in-office procedure is performed when your doctor determines that the lens implant requires the treatment. When performed, YAG laser capsulotomy will be billed to your insurance company.

4 1. Standard Option This option is designed for those who select standard cataract surgery. Patient will be responsible for co-pays and deductibles. Standard Option Includes: A. Cataract Removal with a Phacoemulsifier Unit» The standard of care for several years. Our surgeons have thoroughly evaluated all commercially available phacoemulsification units and use technology that delivers the most consistent results. B. Cataract Incisions Created with Standard Operating Scalpel C. Monofocal Lens Implant» Monofocal lens implants have been the standard of care throughout the evolution of modern cataract surgery. Monofocal lens implants focus at a single location. (distance, intermediate, or near)» The monofocal lens implant can be targeted to leave your vision more for distance or near. Astigmatism will not be corrected with a monofocal lens implant.» Glasses will be needed for distance, intermediate, and near vision.

5 2. LenSx All Laser Incision Option Upgrade cost: $895 per eye This option includes laser assisted cataract surgery as an upgrade to standard cataract surgery. Medicare or other insurances will cover the standard portion of your cataract surgery. Patients will be responsible for co-pays and deductibles. The LenSx All Laser Option Includes: A. Laser Assisted Cataract Removal with Astigmatism Reduction» The LenSx laser technology provides laser precision for making corneal incisions, the lens capsulotomy, and breaking the cataract in exact pieces.» The LenSx laser will be used to reduce astigmatism. B. Monofocal Lens Implant» Monofocal lens implants have been the standard of care throughout the evolution of modern cataract surgery. Monofocal lens implants focus at a single location. (distance, intermediate, or near)» The monofocal lens implant can be targeted to leave your vision more for distance or near.» Glasses will be needed to fine tune your vision for distance, intermediate, and near vision.

6 3. LenSx LASIK Option Upgrade Cost $1,895 per eye This option is designed specifically for individuals who wish to reduce dependence on glasses for distance vision. This option includes laser assisted cataract surgery as an upgrade to standard cataract surgery. Medicare or other insurances will cover the standard portion of your cataract surgery. Patients will be responsible for co-pays and deductibles. The LenSx LASIK Option Includes: A. Laser Assisted Cataract Removal with Astigmatism Reduction» The LenSx laser technology provides laser precision for making corneal incisions, the lens capsulotomy and breaking the cataract in exact pieces.» The LenSx laser will be used to reduce astigmatism. B. Monofocal or Toric Lens Implant» Depending on your particular eye measurements, your surgeon will choose either a Monofocal or Toric lens implant to correct your vision. Monofocal and Toric lens implants focus at a single location. The Toric lens implant has built in astigmatism correction for those who have higher degrees of astigmatism.» Glasses will be needed to fine tune your vision for intermediate and near. C. Laser Vision Correction (LASIK or PRK)» 15% of patients may need additional correction after cataract surgery to maximize their distance vision. If necessary, laser vision correction will be performed no sooner than 2 months after cataract surgery.

7 4. LenSx LASIK Multifocal Option Upgrade Cost $2,895 per eye This option is designed specifically for individuals who wish to reduce dependence on glasses for distance, intermediate, and near. This option includes laser assisted cataract surgery as an upgrade to standard cataract surgery. Medicare or other insurances will cover the standard portion of your cataract surgery. Patients will be responsible for co-pays and deductibles. The LenSx LASIK Multifocal Option Includes: A. Laser Assisted Cataract Removal with Astigmatism Reduction» The LenSx laser technology provides laser precision for making corneal incisions, the lens capsulotomy, and breaking the cataract in exact pieces.» The LenSx laser will be used to reduce astigmatism. B. Multifocal Lens Implant» Multifocal lens implants are the latest advancement in modern cataract surgery. Multifocal lens implants focus at multiple locations. (distance, intermediate, and near)» Minimal glasses will be needed to fine tune your vision for intermediate and near vision. C. Laser Vision Correction (LASIK or PRK)» 15% of patients may need additional correction after cataract surgery to maximize their vision. If necessary, laser vision correction will be performed no sooner than 2 months after cataract surgery.
