1 The Emerson Grant Application Emerson Ecologics is proud to offer The Emerson Grant as part of our effort to support integrative medicine in the United States. This year, Emerson Ecologics will award grants totaling $25,000 to support projects and initiatives that improve, expand, or support the practice of integrative medicine. Objective The Emerson Grant program will provide funding to non-profit organizations in the form of a one-time grant to support specific projects and initiatives which will add value to the integrative medicine community. Projects could include legislative efforts, public awareness campaigns, or enhancements to education or clinical training, but all projects are welcome. Eligibility Grant requests from $500 to $10,000 will be accepted. Partial grants may be awarded. In order to be eligible to receive a grant, your organization must meet the following criteria: 1. Organization must be an incorporated nonprofit organization run by or primarily benefiting the integrative medicine community. 2. Organization s gross annual revenue must not exceed $350, Organization s mission must be aligned with expanding, supporting, or growing the practice of integrative medicine. 4. Grant request must be for funds to complete a specific project or part of a project. (This may include projects such as the design and printing of a new member packet, which has a discrete start and end date, or it may include a portion of a larger, ongoing project, such as paying for a lobbyist for six months of a state licensure effort.) Requests for funding to be pooled into an organization s general budget will be declined. 5. Project (or part of project being funded) must be completed within one (1) calendar year of receipt of grant. 6. Organization must be willing to recognize Emerson Ecologics and The Emerson Grant as supporting the project. In addition, the organization must be willing to allow Emerson Ecologics to use the organization s name and project description in their own promotional materials (e.g., press releases, website, etc.) 7. One calendar year from award date or at project completion (whichever comes first), the organization must provide a written report of project progress with details of how funds were used and, when applicable, access to view the finished product. Emerson Ecologics reserves the right to request documentation for use of grant funds.

2 Evaluation Criteria Grant recipients will be evaluated by a panel of Emerson Ecologics employees from numerous departments. Each grant application will be evaluated for merit based on the following: 1. Reach of the project: the number of people benefiting from the project 2. Project plan is complete, accurate, and feasible. 3. Application materials completed thoroughly and accurately. 4. Grant request, project budget and timeline are appropriate for the project. a. Project budget and timeline are realistic and accurate. b. If grant will not fund the entire project, a plan to raise additional necessary funding is in place to ensure completion of the project. 5. Established need for funding (funding is not currently available to fund the entire project) 6. Organizational structure is sufficient to achieve the desired outcome 7. Project (or portion of project being funded by grant) is likely to be completed in one year from receipt of funding.

3 Please ensure your organization and project meet the criteria described within Eligibility and Evaluation Criteria to ensure your best chances of receiving your requested funding. The Emerson Grant Application Must Include: Cover letter Grant application form Proposal narrative Current/final 501(c)(3) or 501(c)(6) IRS determination letter List of board members, including their credentials and role on the board Organization s most recent financial statement, including previous year s Profit and Loss (showing all revenue and expenses) and balance sheet (showing current assets, such as bank account balances and any assets owned) Organization s current operating budget with revenue and expenses Cover Letter Include a basic one-page cover letter that states the amount requested and provides an abbreviated description or overview of the project. Grant Application Form See attached form. Proposal Narrative Submit a proposal narrative, not exceeding two pages or 1,000 words. Use 1 inch margins, point font, and single line spacing. The narrative should be clear and concise, focusing on the grant request, and should include the following major points (if applicable): 1. Title and Purpose/Overview of Project Description of the project/program and its relation to the organization Discuss the need/issue/problem project will address Indicate the organization s relevant experience to addressing this issue or need Describe the targeted population and geographic area, as well as any special efforts to involve the constituents in the planning or implementation of the project Describe the goals/objectives of the project and its overall impact. How will this grant strengthen the organization, address the issues, make improvements, or achieve success? How will this be achieved? 2. Timeline and Implementation Plans of Project Outline the plans, timeline (including project start and end dates), and proposed staffing for achieving the desired results Indicate who is/would be responsible for managing the project, including potential collaborators

4 Indicate other public or private organizations that are involved in this project and the nature of their involvement Proposal Narrative, continued 3. Budget, Fundraising Strategy and Financial Need Discuss the projected expenses and other sources of committed and anticipated revenue (may be attached separately as a table) Projected timeline for use of funds Discuss fundraising and revenue generating strategies 4. Monitoring, Assessment and Evaluation of Project Indicate the criteria for success/progress and the process for measuring effectiveness Discuss the organization s plan for assessing progress toward goals and overall projected outcomes 5. Sustainability and Future Goals Explain how the organization intends to continue the project (or its impact) after the granted money has been used Describe how the organization will build upon and sustain what is learned after the grant period has ended Indicate any plans to share with other organizations the knowledge gained and to engage them in future efforts Deadlines and Submission Please ensure materials are submitted on time; if mailing the application, it must be postmarked by the submission deadline. Applications received after the deadline will be automatically considered for the next award cycle. Proposals that did not receive an award may be resubmitted for the next award cycle via written request from the organization.
