1 SCOPE OF WORK FEE PROPOSAL City of Ormond Beach Water and Reclaimed Water Storage Tank Capital Improvements Program Preliminary Engineering Services BACKGROUND The City of Ormond Beach (CITY) owns and operates four (4) potable water storage tanks and six (6) reclaimed water storage tanks of different sizes, configurations, and construction types at various locations throughout the city. Chapter of the Florida Administrative Code (F.A.C) requires that all finished drinking water storage tanks be inspected for structural and coating integrity at least once every five (5) years by personnel under the responsible charge of a professional engineer licensed in Florida. In compliance with this regulatory requirement, the CITY contracted Underwater Solutions, Inc. (USI) to perform a visual inspection and cleaning of their potable water storage tanks in January In addition, the reclaimed water storage tanks were inspected and cleaned in March The inspections involved visual observation of structural and coating integrity of the tanks, tank walls, access ladders, fill line pipes, pump suction pipes, vortex covers, air vents, overflow pipes, roof hatches, manways, catwalks, ladders, support legs (on elevated tanks), and any mechanical fasteners for ladders or level controls. Each inspection includes a written narrative report with photographs. Table 1 shows a list of the potable water and reclaimed water storage tanks inspected under this program along with their capacity and location. 2 SCOPE OF SERVICES Hazen and Sawyer (CONSULTANT) agrees to perform engineering services related to the development of a 5-year Capital Improvement Program (CIP) to address the repair or rehabilitation of the CITY s potable water and reclaimed water storage tanks. The project will involve a review of the inspection reports prepared by USI, site visits, additional structural analyses, preparation of cost estimates for repairs, and a letter report outlining the repairs and improvements required for each storage tank. The services will also include the development of renderings of a variety of proposed logos to be placed on the City s storage tank(s). The engineering services performed under Scope of Work will include the following tasks: Task 1: Review Tank Inspection Reports CONSULTANT will review the inspection report for each of the storage tanks and evaluate the condition of the items identified in the report narrative and the photographs included in the reports. The following terms will be used to categorize the conditions of the various items: Good Observed to be in good condition. Minor defects may be present, but do not present an obstruction to operation. Fair Less than good condition. This item has not failed, but it is in a state of degradation that will likely require repair or replacement in the near future. Poor The item has failed or is near failure. CONSULTANT will review other existing available information, record drawings, shop drawings, studies, and reports regarding the storage tanks that were inspected. The CITY will provide the requested information if available. In cases where significant structural deficiencies were noted by USI, CONSULTANT will visit the site to make additional observations to determine the severity of the structural damage. It is anticipated that up to three site visits will be made to observe those structural deficiencies. For storage tanks where the structural design was performed by the manufacturer and the design information is not contained on design drawings, CONSULTANT will coordinate with the manufacturer, the structural design engineer, and/or a tank subconsultant to assess the structural condition and develop the necessary repair recommendations. Task 2: Structural Testing and Analysis For structures showing significant signs of structural distress, CONSULTANT will retain the services of a subconsultant (Tank Engineering and Management Consultants, Inc.) to assist CONSULTANT perform structural testing and analyses as necessary. Structural tests may include the following: Pachometer (cover meter) testing Non-destructive testing to confirm remaining metal thickness, etc The results of these tests and analyses will be used for prioritizing repairs and designing the required rehabilitation. 3 Task 3: Prepare Renderings for Tank Logos The CITY desires to place a logo on the elevated storage tanks at the WTP and at the Breakaway Trails reclaimed water storage site. The CITY will provide several options for the two elevated storage tanks. CONSULTANT will prepare dimensionally correct renderings of the logos displayed on the two elevated storage tanks for presentation to the City Commission. CONSULTANT will prepare for and attend one meeting with the City Commission for the purpose of presenting the alternative logo configurations for their selection. Task 4: Prepare Letter Report and Estimates of Probable Repair Costs CONSULTANT will prepare estimates of probable repair costs for each of the water and reclaimed water storage tanks based on the information and recommendations presented in the inspection reports, additional structural testing for severely deteriorated structures (if required), and on the CONSULTANT s observations of each facility. Where alternative methods of repair or rehabilitation are warranted, CONSULTANT will prepare a cost estimate of each alternative for comparison. The recommended repairs and the estimated costs associated with each storage tank will be presented in a letter report to the CITY. CONSULTANT will meet with CITY staff to review the letter report and the estimated cost of repairs. SUBCONSULTANT Structural Testing: CONSULTANT will utilize the services of Tank Engineering and Management Consultants, Inc. (TEAM) to assist with the evaluation of the repairs required for the three elevated steel storage tanks. TEAM will assist CONSULTANT review the tank inspection reports, visually observe the defects, perform structural testing and analysis, and assist with the cost estimates for the elevated steel storage tanks. DELIVERABLES CONSULTANT will provide the following information, data, and documents to the CITY: Two (2) full size (22 x34 ) copies and ten (10) half size (11 x17 ) copies of the renderings of the logos for the two (2) elevated storage tanks. Five (5) copies of the letter report outlining the repairs required for the storage tanks. Electronic copies of all meeting summaries will be forwarded to all attendees. CITY REQUIREMENTS The CITY will provide the following: Existing available information for each storage tank including design drawings, record drawings, shop drawings, photographs, studies, and reports. Access to all water and reclaimed water storage tanks. Assistance in arranging meetings with CITY departments. COMPENSATION CONSULTANT shall be paid in accordance with the Time Basis Method of compensation as set forth in Section 5(b) of the Agreement. A budget with a Not-to-Exceed amount of $35,967 for the Task Authorization No. 1 (102014) Page 3 of 4

4 services under this Scope of Work is shown in the attached Fee Proposal, and this budget will not be exceeded without the CITY s written approval. Services to be rendered under this Scope of Work are provided in accordance with Section 1 of the Agreement. TIME SCHEDULE Services described herein will commence upon receipt of the executed Work Authorization Form by CONSULTANT which shall constitute CONSULTANT s Notice to Proceed. The project start date is anticipated to be November 15, 2014 as shown on the attached Project Schedule. The letter report regarding the repairs to the elevated steel tank at the WTP are anticipated to be completed by January 16, 2015 based on the Project Schedule. (TEAM Consultants), is pleased to furnish our proposal for performing inspection and engineering services in support of Hazen and Sawyer for the rehabilitation of the above referenced elevated water storage tanks. This project will include the following services: INITIAL ASSESSMENT Task 1 - REVIEW RECENT REPORTS: TEAM personnel will review the reports from a recent inspection contracted by the City. The purpose of this review is to begin establishing a scope of work for tank rehabilitation. Task 2 - ON-SITE INSPECTION: Each tank will be visited by a TEAM inspector in order to assess the tanks current condition and verify the conditions stated in the report. Our inspection will include the following, as applicable: Access to areas of corrosion may require a manlift. Complete visual inspection of the site and foundations. Inspection of the support tower including base plates and anchor bolts. Inspection of the platforms and handrails. Evaluation of the interior and exterior of the tank and stem plates supplemented by random non-destructive ultrasonic measurements, if necessary. Evaluation of the tank interior will be made with water in the tank and the water level lowered to approximately 50% capacity or lower. This will allow a general visual inspection of the interior from the existing interior ladder. Evaluation of metal for isolated or general deterioration, including structural damage due to metal loss. The average area pitted and depth of pits will be determined and recorded. Detailed measurements will be taken and recorded to assist in the design phase, including: UTM steel thickness, structural member sizes and spacing, and appurtenance details. Evaluation of accessories such as obstruction lights, conduit, manholes, overflows, vents, etc. Evaluation of OSHA-related accessories such as ladders, vents, manholes and balcony. Hazen and Sawyer City of Ormond Beach Three EWT s July 16, 2014 Collect coating samples from the exterior and have them laboratory tested for the presence of lead, cadmium, and chromium. STANDARDS - We intend to utilize the following standards for the inspection and report: The Engineering Report will utilize American Water Works Association (AWWA) Standards and Manual M42 as criteria for tank evaluation. Personnel access areas will be inspected and evaluated in accordance with Federal OSHA EXCLUSIONS - We do not include: Draining the tanks or opening manholes. Rigging the interior tank for access. The interior will be inspected using existing ladders for access. All work will be done under the direct supervision of a Registered Professional Engineer as required by AWWA M42, App. "C" and FDEP Rule Approximately one-half day of field time will be required for the assessment inspection. Task 3 - REPORT OF INSPECTION: Following the inspection, a Report of Inspection will be submitted for each and will contain detailed information on the condition of the site, structure, accessories, and appurtenances. We will include recommendations concerning any repairs, replacements, etc., with color photographs to illustrate the findings. Economic considerations such as replacement value, estimated cost of repairs and repainting, and the remaining life expectancy of the tank are also included. SCHEDULE: The Report Review can begin within two (2) weeks of your Notice-To-Proceed. The On-Site Inspection can be scheduled within one (1) week of the Report Review. The Report of Inspection will require approximately three (3) weeks to complete following the inspections.
