2 Proof of status. Notarized evidence of your legal status in US (Notarized copy of Green card, US visa, I-20, H1B approval notice. VisaHQ cannot assist US B1/B2 visa holders at this time.) A copy of the International Certificate of Vaccination (ICV) for yellow fever is required if the applicant has traveled within the last 90 days to any of the following yellow fever endemic areas. Passport MUST contain two blank visa pages side by side for the visa to be issued. If the child cannot be inoculated, please submit a notarized letter from the child's physician. Additional required documents for applicants of Brazilian Origin: Brazilian citizens, even those with dual citizenship, must enter and leave Brazil with a Brazilian passport.
